Nimmy James    2 posts   Re: Topic 8 DQ 1  Dependent variables are otherwise known as the outcome variables. The value of the dependent variable depends on the value of other variables. In research, the dependent variable takes on different values in response to the independent variable. The independent variable is otherwise known as the experimental or predictor variable. It is manipulated in the research to observe the effect on the dependent variable. The value of an independent variable does not rely on other variables as the independent variable can stand alone (Kusurkar, Ten Cate, van Asperen, & Croiset2011).  Using my EBP as an example, the dependent variable in the study is the rate of pressure ulcers in the facility. To measure this, I will need to collect data on the number of pressure ulcers in past three years. This data will help me to monitor how manipulating the independent variables affect pressure ulcer rate which is a dependent variable. The independent variables in this study include nurses’ knowledge about wound care and how effects of protein intake with wound healing. The EBP will manipulate theses independent variables by educating nurses on nutritional impact on pressure wound healing and by implementing extra protein into their daily diet. With a questionnaire, I will be able to collect data on nurses’ knowledge about the topic which help with planning the education and training.

Dorothy Troutman    4 posts   Re: Topic 8 DQ 1  For my evidence-based project I will need to collect independent variables including the nurses’ response to whether they are more comfortable recognizing and treating hemorrhage. Additionally, I must collect data on the dependent variables including hemorrhage rate and transfusion rate. I will evaluate if the nurses’ improved comfort level in recognizing and treating hemorrhage decreases the rate of hemorrhage and rate of transfusion. It is important to remember that the dependent variable depends on another variable (the independent variable). We must understand the relationship between the two in order to understand the outcomes of research (L. Flannelly, K. Flannelly, and Jankowski, 2014).  The goal of my project is to decrease the overall number of hemorrhages patients experience after giving birth, as well as to decrease the number of transfusions needed to treat hemorrhage. By decreasing the hemorrhage, and transfusion rates we will improve patient outcomes and decrease maternal morbidity. I plan to track hemorrhage rates post education and compare them to the pre-education hemorrhage rates. I will do the same thing with transfusion rates.

A variable is something that varies or changes. “A variable in research simply refers to a person, place, thing, or phenomenon that you are trying to measure in some way” (University of California Libraries, 2019, para. 5). Independent variables are stable and unaffected by other variables you are attempting to measure. These are often manipulated by the researcher. Dependent variables are those that depend on other factors and change as a result of an experiment. This is the response that is measured. Dependent and independent variables are important because they drive the research process. A variable as opposed to a constant is simply anything that can vary and that many researchers consistently look at the relationship between the two variables. In order to evaluate my EBP project, I will have managers audit charts to determine if education provided to the patient was effective by not being readmitted with the same diagnosis. A few independent variable are patient’s willingness to learn, age, and time. These are important factors to consider when performing the audits. Some patients are not willing to make a change to their health and are set in their ways. The time spent with the patient is also important to consider. If the patient is being discharged and is ready to go, they aren’t going to fully be attentive to the teaching as they are eager to leave. Timing is key and having enough time to perform the teaching is important as well. Some dependent variables to consider are the patients who are admitted with stroke and chest pain who are tobacco users. The outcome depends on the effectiveness of the teaching performed by the nursing staff. The variables need to be collected because they will affect the evaluation. If existing staff is unable to clearly explain the new way of caring for hemorrhage patients, they may instead teach the “old” way. If staff has worked in other labor and delivery units, they may be used to how their old facility cared for postpartum hemorrhage patients, and resort to how they have always done things.  Postpartum hemorrhage only happens in about 2% of births. Though nurses may have a strong desire to continue to provide evidence-based care, they may forget some of the information they learned during the implementation phase.  One way to overcome both concerns is to hold simulation trainings several times per year, and to re-educate as necessary. Additionally, including the training during new hire orientation will help new staff learn about the importance of evidence-based care for postpartum hemorrhage.  Clark and Marks-Maran (2014) discuss the importance of leadership engagement in sustainability. One critical component of Evidence Based Practice is to ensure that practice change is part of an organization’s culture so it will continue to impact outcomes over time. There are barriers that may prevent my Evidence Based Practice change proposal from continuing to obtain the same desired results six months to a year from now. My PICOT statement is, In adults with tobacco dependency (P), would providing teaching/research about tobacco cessation (I) compared to medications to assist in tobacco cessation (C) lead to a decrease in tobacco dependency and better health outcomes (O), after four weeks of intervention/quit date (T)? One barrier is new staff coming on board and not knowing the proper way to educate patients on tobacco cessation. The current staff will receive proper education on how to educate tobacco dependent patients. To assist new staff with the process change, we can create required computer based learning assignments (CBL’s) that have to be completed before coming out of orientation. This will allow all new nurses to be up-to-date on the education process. Another barrier is language. Not only is language a barrier in education, but other forms of communication as well. Upon communicating with a patient who does not speak English, the health care team must first decide what language the patient understands best, and in what way the patient learns best. Once the assessment is completed, the nurse can obtain an interpreter. At our facility, there are interpreters on wheels; this is an IPad that we bring to the patient and an interpreter comes on live to speak to the patient in their specific language. Not only can we use the interpreter, but provide written education in different languages as well. Throughout the education process, it is important to remember communication techniques. A few communication techniques are listening, asking open ended questions, and having an open posture (Elsheikh, 2018). Change initially can be a good thing and take off quickly. Anne Kolsky    3 posts   Re: Topic 9 DQ 1  Professional Presentations  Strength: I may not be the most detail-oriented writer or person in general, my writing strengths appeal more to the lay person than the technical side. I can convey messages well to those who do not understand the “nursing side” of nursing. In other words, if I were to be on a team of professionals seeking to make changes together, I would be put on the committee that sends out the messages to the community. Although I can convey my messages in print well, I struggle with speaking coherently. Joining toastmasters or taking classes that can improve my communication to all audiences would be beneficial.  Weakness: Conversely, my attention to details are not my strength, as evidenced by the inability to pick up mistakes in citations, etc. I could be employed to find the research, but probably not chosen to be the editor. Taking classes, attending journal clubs, and other similar activities could help me improve on the technical side.  Evidence-based practice for strong leadership includes role modeling and demonstrating strong commitment to research, developing skills through staff opportunities, meeting performance appraisal expectations, policy making based on solid research, encouraging others to do their best for the patients and each other, willingness to change policies and practices based on solid evidence, demonstrate transformational leadership based on openness to listen and implement based on true needs and evidence, and encourage the entire community to research and inquire how to do things better which is no easy task as it is often perceived as “more work piled on” for those who are already maxed out.
