                       Discussion: Scholarly Writing and Plagiarism

As a nurse embarking on an advanced degree, you are developing the characteristics of a scholar-practitioner, which includes strong communication skills. Writing in a scholarly manner involves supporting your thoughts with evidence from the literature and appropriately using APA formatting.

One of the challenges of scholarly writing is paraphrasing the thoughts of others in your work. Paraphrasing, and correctly citing the original author for his or her ideas, allows you to take the ideas of others, summarize them, and incorporate them into your own writing. 

When summarizing the ideas of others, it is important to avoid plagiarizing (copying the words and ideas of others as though they were your own). In addition to expanding your knowledge of APA, this week’s Learning Resources help you to distinguish between paraphrasing and plagiarizing.

Read the following paragraphs, which were written by Patricia O’Conner:

A good writer is one you can read without breaking a sweat. If you want a workout, you don’t lift a book—you lift weights. Yet we’re brainwashed to believe that the more brilliant the writer, the tougher the going.

 

The truth is that the reader is always right. Chances are, if something you’re reading doesn’t make sense, it’s not your fault—it’s the writer’s. And if something you write doesn’t get your point across, it’s probably not the reader’s fault—it’s yours. Too many readers are intimidated and humbled by what they can’t understand, and in some cases that’s precisely the effect the writer is after. But confusion is not complexity; it’s just confusion. A venerable tradition, dating back to the ancient Greek orators, teaches that if you don’t know what you’re talking about, just ratchet up the level of difficulty and no one will ever know.

 

Don’t confuse simplicity, though, with simplemindedness. A good writer can express an extremely complicated idea clearly and make the job look effortless. But such simplicity is a difficult thing to achieve because to be clear in your writing you have to be clear in your thinking. This is why the simplest and clearest writing has the greatest power to delight, surprise, inform, and move the reader. You can’t have this kind of shared understanding if writer and reader are in an adversary relationship. (pp. 195–196)

Paraphrase this passage from O’Conner using no more than 75–100 words. Remember that paraphrasing means summarizing the essence of the original text. It does not mean creating a thesaurus-based revision of the author’s original words or copying the piece, or any part of it, word for word. For this activity, do not use any direct quotes.

Kimberly Morris    1 posts   Re: Topic 9 DQ 2  One potential barrier that may prevent my evidence-based practice proposal from continuing its enthusiasm is the fact that the community in which I will be presenting may lose interest in the topic of lead. Because this topic has been brought to the forefront due to a recent crisis, once the initial public health hazard has been taken care of, the community may go back to being ignorant or not caring about the topic. One way in which I can battle this is to stress the fact that elevated blood lead levels are not an acute problem for the general public but instead they are a chronic problem. While the initial threat of elevated blood lead levels in children due to the public water change in Flint has passed, there are still may ways in which children can obtain lead. Pointing this out and stressing this in my education will hopefully allow me to continue an active interest within the community.  Another barrier I can see would be a one-size-fits-all approach to my chosen topic. Evidence-based practice changes are scientific based and often people take a strict adherence to the guidelines set forth within these proposals (Surface, 2009). These guidelines may not always fit with the audience you are working with and should be a revolving set of guidelines based on research and evidence. Just like my topic – 25 years ago obtaining elevated blood lead levels from imported toys and food items was not a big problem but in today’s global economy it has become an area of concern.  Reference  Surface, D. Advocacy for patients is important to ensure that they are receiving the proper care (Helbig, 2018), and when they can’t voice for themselves, we, as nurses, should stand up for them and make sure the proper care and needs of our patients are given if at any time it is denied or is in need of investigation or proper resources. Communication and collaboration are key in the work environment in order to provide patient-centered care (Helbig, 2018).

I had an experience where the patient was seen and the doctor determined she had not been taking her medication as prescribed and had not attended PT.  The doctor came out of the room and instructed me to document non-compliance and to discontinue her treatment at the office.  I asked him what the problem was and asked if I could talk to the patient and see what may be the cause.  He gave me permission and I was able to determine the issues within twenty minutes.  As he was seeing another patient, I waited until he was done and asked to speak to him.  I explained to him the issue was the patient was in the hole with medication through Medicare and could not afford it, and was also having problems with transportation.  He gave instruction to document the result and find the resources and samples of medications the patient needed and we would supply them until she was out of the hole. Had I not intervened or advocated for the patient, she would have been labeled non-compliant as well as had to go through the process of looking for another doctor.

Nimmy James    3 posts   Re: Topic 1 DQ 2  Providing quality care to the patient very important in nursing the Evidence-based practice (EBP) takes a major role for this outcome. “The EBP process is a method that allows the practitioner to assess research, clinical guidelines, and other information resources based on high quality findings and apply the results to practice” (Spyder, 2018).  In this study, 2100 questionnaires were given to registered nurses find out their thoughts regarding on EBP. The positive expression towards the EBP were more than 64% (Majid et al., 2011). But the nurse’s workload in daily routine causes many issues which they cannot keep up to date with new evidence. The nurses felt that EBP training, time availability, and mentoring by nurses with EBP experience would encourage them to implement EBP.  There are many issues and barriers that might hold nursing practice from achieving this goal. The first issue is the overload of information, means there are thousands of articles are published monthly which cause confusion and attain wrong data. Another issue is lack of time to adopt the EBP, lack of resources, and the difficulty in understanding statistical analysis (Majid et al., 2011).  Although nurses showed a positive attitude toward EBP, certain barriers were a delay. It is important that hospital management develop a comprehensive strategy for building EBP competencies through proper training and education.  Advocacy is as essential to the role of a nurse as any other aspect of nursing care. Interacting with patients more than any other healthcare provider, nurses are in the perfect position to serve as patient advocates (“Importance of Patient Advocacy in Nursing | UTA Online,” 2016) We as nurses are the ones who know the patient the best. We are at the bedside the majority of the time with these patients and families. We get to know them the best, so we as nurses have the responsibility to advocate for them.
I had a patient once who was pod #1 s/p laparoscopic gastrectomy sleeve. The patient seemed to be doing very well until he became very SOB, was complaining of chest pain, shoulder pain, he was tachycardic. This particular surgeon is very particluar with his patients and does not like to get anyone else involved. We strictly call him to his cell phone with any questions or concerns. However, in this case he did not reply promply to my call, so i had to call a RRT. Patient had to go to the ICU, and later found out that a patient had a leak. Had I waited for the surgeon to call me back or not called an RRT, the patient wouldnt have been treated promptly.
Advocating for our patients is crutial for them. We care for them in one of there most vulnerables times in their lives. They trust the heathcare professionals to do what is best for them.

When nurses advocate for patients, stand up for patients’ rights, or work to meet their need for care, that nurse is actively assuming the role of patient advocate. The nurse is using knowledge to care for the patient and to coordinate care for the patient (Dynamics in Nursing 2018). This is important because the nurse communicates what he/she believes to be the best methods of care for the patient based off the data in all forms.  This is then communicated by the nurse across all levels of care of the patient to ensure the best possible outcomes for the patient.

A situation I recall in my time of patient care in the Emergency Room to where I advocated for a patient was realizing that based off the data I collected when caring for a patient I suggested to the doctor to perform a CT scan that wasn’t previously done and hadn’t been ordered yet.  The patient was feeling a bit short of breath and was on oxygen and on the cardiac monitor with a slightly elevated heart rate.  We had already performed a chest x-ray and labs right away and cardiac monitoring.  Because I was the nurse caring for the patient and monitoring the results in all forms, my concern was that the patient had a Pulmonary Embolism and because the doctor hadn’t actually made it bedside to see the patient yet, my advance knowledge and awareness prompted me to relay to the doctor what I believed we needed to do.  Sure enough the patient did in fact have a Pulmonary Embolism and we were able to proceed faster with care and get the patient admitted and to the floor faster as well.  Had I not advocated for the patient the outcomes could have went in many directions but mostly would have delayed prompt patient care and potential adverse outcomes.

Accountability is defined as the concept of being responsible for one’s actions. This means being accountable for our work practice, environment and patient safety. The American Nursing Association’s (ANA) code of Ethics defines professional accountability as being “answerable to oneself and others for one’s actions.” In 2016 the Gallup poll rated Nursing as one of the most trusted profession in the United States for 15 consecutive years. It is of vital importance that we maintain this level of trust with each patient encounter. As nurses our patient’s healthcare depend on the decisions and actions we make. Sometimes our actions make a difference between life and death. As nurses we face daily time challenges as well as work demands. We often have to deal with staff shortages and the acuity of our patients. Even with all these road blocks, it is our responsibility to maintain certain ethical standards in order to obtain better outcomes. The ANA code of Ethics for Nurses was developed as a guide for carrying out nursing responsibilities in a manner consistent with quality in nursing care and the ethical obligations of the profession. It was created to help nurses “navigate ethical and value conflicts, and life and death decisions, many of which are common to everyday practice” (Ethics and Human Rights, 2018). It states that, “the nurse is responsible and accountable for nursing practice and determines the appropriate delegation tasks consistent with the nurse’s obligation to provide optimum patent care” (Code of Ethics for Nurses with Interpretive Statements, 2010)
