The instructor will assign you to a CLC group.

This assignment consists of both an interview and a PowerPoint (PPT) presentation.

Assessment/Interview

Select a community of interest. It is important that the community selected be one in which a CLC group member currently resides. Students residing in the chosen community should be assigned to perform the physical assessment of the community.

1. Perform a direct assessment of a community of interest using the “Functional Health Patterns Community Assessment Guide.”
2. Interview a community health and public health provider regarding that person’s role and experiences within the community.

Interview Guidelines

Interviews can take place in-person, by phone, or by Skype. Complete the “Provider Interview Acknowledgement Form” and submit with the group presentation.

Develop one set of interview questions to gather information about the role of the provider in the community and the health issues faced by the chosen community.

Compile key findings from the interview, including the interview questions used, and submit with the group presentation.

PowerPoint Presentation

Within your group, create a PowerPoint presentation of 15-20 slides (slide count does not include title and reference slide) describing the chosen community interest.

Include the following in your presentation:

1. Description of community and community boundaries: the people and the geographic, geopolitical, financial, ethnic, and phenomenological features of the community as well as types of social interactions, common goals and interests, barriers, and challenges.
2. Summary of community assessment.
3. Summary of interview with community health/public health provider.
4. A conclusion summarizing your key findings and a discussion of your impressions of the general health of the community.

APA format is required for essays only. Solid academic writing is always expected. For all assignment delivery options, documentation of sources should be presented using APA formatting guidelines, which can be found in the APA Style Guide, located in the Student Success Center.

With the client you selected in mind, address the following in a client termination summary (without violating HIPAA regulations):

· Identifying information of client (i.e., hypothetical name, age, etc.)

· Date initially contacted therapist, date therapy began, duration of therapy, and  

   date therapy will end

· Total number of sessions, including number of missed sessions

· Termination planned or unplanned

· Presenting problem

· Major psychosocial issues

· Types of services rendered (i.e., individual, couple/family therapy, group therapy, 

   etc.)

· Overview of treatment process

· Goal status (goals met, partially met, unmet)

· Treatment limitations (if any)

· Remaining difficulties and/or concerns

· Recommendations

· Follow-up plan 

· Instructions for future contact

· Signatures

You have been working as a veterinary assistant at a small animal clinic for the last two years. Part of your responsibilities involve assisting with client education when clients must provide care or treatment after minor problems with their furry companions.

Melanie is a long-time client of the practice. One summer day, Melanie brings her dog Zoey to the clinic because Zoey has been limping for the last several days. During the appointment, the veterinarian determines that Zoey has suffered damage to her distal and metacarpal pads from walking on hot pavement. You assist the veterinary technician clean up and bandage the pads. The appointment has run very long, so you are trying to free up the examination room for another patient. You walk out of the room with Melanie and explain to her that Zoey should be walked only in grassy areas during the day and that she should not walk on hot pavement. You also quickly explain that the bandages must be kept clean and dry; you let Melanie know that she should use plastic bags or booties over the bandages before taking Zoey outside if it is raining.

Two days later, Melanie returns with Zoey, whose bandages are wet, dirty, and falling off. Melanie is quite upset about the condition of the bandages. Clearly, Melanie has not followed the instructions that were given to her. She is very upset and eventually begins yelling that you clearly have no idea what you are doing.

Write a one- to two-page response that answers the following questions.

1. How should you respond to the situation? Explain why your suggestion is appropriate.

2. Describe how the original situation could have been better handled. Explain why.

In the presentation, address the following:

1. State the objective of the presentation.
2. State the Healthy People 2020 focus area your group has selected and the rationale for selection of the specific focus area.
3. Explain how the focus area relates to the individual, the family, and the community, as well as to all age groups throughout the life span.
4. Identify ways to enhance or optimize health in the selected focus area using evidence-based research. A minimum of three peer-reviewed articles must be utilized.
5. Address the health disparity among different segments of the population for the selected focus area.
6. Provide a brief description of at least two community resources and at least two website resources for professionals and clients.
7. Provide a brief profile of at least one health-related organization for the selected focus area.
