“S”- Specific– State exactly what you want to accomplish (Who, What, Where and Why)

“M” Measurable– How will you demonstrate and evaluate the extent to which the 

goal has been met.

“A” Achievable– What is your action-oriented verb? (Please see last page of syllabus)

“R” Relevant– How does the goal tie into your key responsibilities? How is it aligned to objectives?

“T” Time-bound– Set a target date- “by when” to guide your goal. Include deadlines, dates, frequency.

· Action Plan/Evaluation tool methods ( HOW ARE WE EVALUATING AND WHAT ARE OUR ACTION PLAN USE YOUR IMAGINATION AND RESEARCH)

· 4-6 Evidence Based References ( IMPORTANT!!!!! )

Goals must be related to the course objectives and the area of your practicum experience. The objectives should be specific and measurable. The plan of action should include step-by-step process to meet the objectives. The implications for nursing practice should be evidence-based.
