When a new director makes a decision of reorganizing the department without consulting the nurses who have been operating there for some time develops a conflict issue. This director rules autocratically which a style which builds tension among the management as well as employees. Nurses are important in the execution of better health care in the community and their roles are undisputable (Huber, 2018). Thus, nurses are supposed to be involved in changes that an organization wants and in decision making. 

Conflict may be described as the perceived threat that can be external or internal, positive or negative and disruptive or competitive. Leaders are supposed to be attentive to negative and disruptive conflicts that impact people’s actions and feeling in an undesired way. In case a change is made without involving nurses lead to high turnover, resentment, frustration and negative patient results. Conflict resolution leads to the adoption of different strategies which include the following; defensive mode like avoiding the subject to prevent escalation as well as gain time. The other one is a compromise as well as meeting at halfway to ensure each party gains something and finally, creative problem solving which includes empathy with the involved parties, discussing in a constructive manner and developing an assertive dialogue which results in solution agreement (Huber, 2018). 

As a nurse leader, in case of conflict, I would employ creative problem solving as a method of conflict resolution. I would make sure am familiar with the changing scope initiated by the director and understand the concerns of the staff as well as how the changes may negatively affect patient care, influence staff feelings and emotion and disrupt the entire nursing process. I would talk to the management to meet with the nurse staff to address the problem. Communication is a safe way for the management to share changes to the staff. I would motivate the nurses to speak out their concerns, remain assertive and communicate their ideas. I would also encourage an external mediator who is trusted, objective and knowledgeable.

Moreover, every professional connection should be based on interactions that involve knowledge sharing and mutual professional development

  In fact, networking may play an even larger role when it comes to nursing jobs.  Nurses must develop strong relationships with peers and managers at both their current and former jobs. This means maintaining some level of contact with former coworkers to ensure relationships stay fresh and relevant and open. It also means developing relationships that are strong enough to be able to ask for and receive a glowing professional reference when needed. Given that nurse’s central role in patient care is maintaining current knowledge of trends and advancements, teamwork is vital too.

  A situation in which I have networked for the health of a population or your community was when our facility lagged really behind in wound care, our new admissions from hospital always come with very bad wound and before we could start our own policy on wound care some are worse, I made my director of nursing hire a wound doctor I met when I was in school. Health care reengineering is a discipline that helps organizations reorder priorities, provide more cost-effective care, and increase value to customers. Reengineering should be driven what is best for the organization and the needs of the organization. Reengineering is a critical “core competency” and “requisite skill”  for health care organizations if they are to succeed under managed care in the future. Managers and leaders play a vital role in the process of reengineering. The managers role is to ensure “that the mission of the organization, which focuses on providing excellent care for clients, is fulfilled through effective and efficient coordination of resources” (GCU, 2013). Managers display the skills for showing responsibility by guaranteeing that the staff is competent to provide the care of each individualized patient. Managers show their skills by holding staff accountable. Managers have been described as planners, problem solvers, delegators, and staffers (Huber, 2014). GCU, 2013, described the roles of management as planning, organizing, staffing, directing, and controlling. Leaders are the mentors and inspirations of the organizations. The leaders make the goals and outcomes known for the organization. The leaders are critical to organizations as they ensure that the operations than run from day-to-day are ran smoothly without any bumps in the roads. Managers and leaders have distinct roles with their organization but they both must integrate their roles to provide effective leadership. Nurse managers and leader’s role in reengineering health care is by being advocates for patients and staff, as well as making sure the hospital’s environment is calm and functioning smoothly. “Advocacy is a process used to facilitate growth in others. Common methods of advocacy are: 1) providing others with information that empowers them to make their own decisions, and 2) acting on behalf of others who do not have the ability or power to advocate for themselves” (Reengineering, 2011). It is important for the nurse managers and leaders to create a hospital facility environment where patients are respected, and are empowered by educating them on their health and condition to enable them to make decisions about their treatment and care. It is also important for managers/leaders to continually check on their staff’s patient care feedback and any concerns they may have. It is also important for them to listen to their staff and patient. “Leaders must listen to the sounds of change. Leaders must help people work through the grieving process and conflicts experienced during time of change. Continuous quality improvement (CQI) is taken very serious at my organization, as I am sure it is taken serious at my fellow classmates facilities. CQI is a quality management process that encourages the health care team members to continuous ask “how are we doing” and “can we get better” (Moran, 2012). CQI is for the safety and education of the patients and staff members. We report out each morning about 30-day readmissions in house or patients at high risk for a 30-day readmit. We have a team that identifies the trends of the readmission predictors in order to identified patients currently in house at high risk for 30-day readmission. After the identification of the high risk patients, a structured discharge plan is put in place. Education on the disease process and discharge instructions is immediately started with the patient and care givers. Outside resources are immediately identified, such as home health and hospice, and the patient is given the patient choice form to pick the company that they would like to use on discharge. On discharge, the patient will know their disease process and how to manage at home. The patient will state what medications they are taking and what they are taking them for.
The patient will also state what time of day the medication is due. The patient will receive a phone call within forty eight hours after discharge from the hospital to make sure the patient continues to understand all discharge instructions and medications.

There is no delaying that hospitals need to deal a brand in today’s medical world. I can speak for the organization where our brand is high-quality care at low cost. A part of growing a brand that delivers high-quality care consistently it is vital to continue to maintain an emphasis/focus on identified quality improvement initiatives. To create a hospital culture supportive of quality improvement, respondents stressed the importance of hospital leadership being in the vanguard to engage nurses and other staff. As a representative of an accrediting organization said, “For any quality improvement project to be successful, the literature shows that support has to trickle down from the top (Draper, Felland, Liebhaber, & Melichar, 2008).

The chief nursing officer, CNO, is passionate about the delivery of high-quality care, she talks about it each month at nursing leadership, and it is a constant in my staff meeting minutes. It is vital that we empower the nurses who are delivering the care at the front line to buy into initiatives. We have encouraged staff to participate in hospital-wide performance improvement and report out our progress as a healthcare system in unit councils.

One example of how we are applying CQI in our current practice is through our CAUTI initiative. We have developed an education package for the staff which was accompanied by a nurse-driven protocol for patients that have a foley. If we have a patient who comes through the emergency department, they need to meet specific criteria before a foley can be inserted. We have seen great successes in this project, and the results have been extremely positive.

Regardless of method, participants are purposefully enrolled based on their familiarity with the phenomenon. Importantly, the objective of qualitative research is not the accumulation of information, but the growth of understanding about phenomena of concern to nursing.

Difference Between Grounded Theory and Ethnography

Definition

Grounded theory and ethnography are two qualitative research  methodologies. Grounded theory, developed by Barney Glaser and Anselm Strauss, is a methodology that involves developing theory through the analysis of data. Ethnography is the systematic study of a culture or community. The main difference between grounded theory and ethnography is their purpose; grounded theory aims at developing theories whereas ethnography aims at exploring and understanding a particular culture or community (Hasa,2017).  

Grounded Theory: Grounded theory is a methodology that involves developing theory through the analysis of data.

Ethnography: Ethnography is the detailed and systematic study of people and cultures.

Purpose

Grounded Theory: Grounded theory aims to develop theories in relation to the collected data.

Review of Literature

Grounded Theory: Researchers do not consult literature before analyzing data since it may influence their findings. 

Ethnography: Ethnographers can consult literature before starting the field work.

Sampling

Grounded Theory:Theoretical sampling technique is used since it assists in developing theory.

Ethnography:Purposive sampling method is used since the study emphasizes on a particular aspect of culture.

Similarities between Grounded Theory and Ethnography

Firstly, in both methodologies, the researcher studies the phenomenon in its natural context and uses a holistic approach to study the phenomenon .

Secondly, in both the methodologies more than one data collected approach is adopted as it helps the researcher in providing multiple interpretations and also enhances the accuracy and credibility of the research study(Datt,2014).

The third similarity between the two methodologies is that; the researcher presents the reports from the perspective of participant who have experienced the phenomenon in the natural setting. Excerpts from the interviews, stories of participants are added to the report which enhances the overall richness of the research findings.

Phenomenological research attempts to examine how people perceive and comprehend a particular situation. The information can only be collected from individuals who have lived through the experience. Grounded theory research is based on symbolic reaction theory where it is used to discover problems that exist in a social scene and how the person views or deals with them. It involves formulation, testing, and redevelopment until a theory is developed. In spite of the differences, all of these approaches share the focus of phenomenology, which is exploring how human beings make sense of experience and the meaning they give to these experiences. Phenomenology is being used in the social and human sciences including sociology, education, psychology, nursing and health sciences (Creswell, 2007).

Similarities                                                                                                                                                           Phenomenology and grounded theory studies have many similarities. The researchers explore real life situations and require a high degree of interaction between the researcher and the individual, groups or situations being examined. Both grounded theorists and phenomenologists seek to collect and analyses data from participants’ perspectives and try to ensure their findings are not influenced by preconceived ideas. To achieve this, they often involve participants in data analysis to increase the trustworthiness of the findings. In brief, grounded theorists and phenomenologists both seek to explore individuals’ experiences in the context of the worlds in which they live.

Differences                                                                                                                                                       Phenomenology and grounded theory studies have many differences Phenomenology emerged from philosophy, primarily influenced by Edmund Husserl and Martin Heidegger; it aims to describe and explore experiences, which can only be done by collecting data from individuals who have lived through those experiences. Hence phenomenologists often refer to the lived experience and data is often limited to interviews, while findings are reported as a rich description of the experience drawing on characteristics identified during data analysis (Ploeg, 2004). Grounded theory was developed in sociology, and was first described by Barney Glaser and Anselm Strauss as a qualitative methodological approach in which the aim was to generate a grounded theory, to describe and explain the phenomenon under study. Knowing scientific management theory as defined as supervising by way of focusing on task accomplishments rather than interpersonal relationships (Huber 2014). Some routines in health care that seem to be inefficient in regards to scientific management are the overwhelming focus on tasks, the evaluations based on tasks accomplished, and the pressure to join committees to add more tasks to nurses’ duties. A lot of times nurses have trouble tending to the needs of their patients because they are so focused on getting the tasks done so as to not get in trouble. Many times tasks are added to the nurses’ workload making it hard to complete everything on time. This causes incidental overtime which is also greatly frowned upon. Finally, the committees are supposed to be a good thing, allowing nurses to participate in shared governance. The issue arises when the requirements for the committees capitalize on the time the nurse should be working or decompressing from the stress of daily work. Some examples of participative decision-making at the hospital that I work at are unit forums and shared governance council. The unit forums allow for nurses and aides in the unit to meet and discuss issues to resolve amongst themselves. Shared governance councils allow for nurses to meet with administration to discuss issues at the bedside that can only be fixed by administration.

The scientific management theory states that humans are motivated by money, and there should be a separation between managers and workers. The expected result is a dramatic increase in productivity of the workers and profits for the organization. Some of the routines in the health care setting that are inefficient is nurse-to-patient ratio and staff retention. I feel the nurse-to-patient ratios are unsafe as it overloads the nurse and nursing assistants in carrying for high acuity patients with a higher prevalence of medical errors and oversight. When staffing is is done according to census instead of census it creates an unsafe environment for the patients and the staff. Without adequate staffing, patient care is inadequate and the patients suffer. Which in turns causes patient surveys to report negative feedback giving the hospital low numbers.Staff retention is important to ensure there is adequate staff to meet the demands of an aging population with numerous chronic illnesses. When facilities ignore the concerns of staff or do not provide attempt to implement change that benefits the staff and the patients, morale falls and staff leave. When nurses feel their safety and license are in jeopardy, they will seek out a better work environment they feel safe in. The facility I work for currently does annual retention surveys that allow its employees to voice any concerns and share what they feel the facility is doing well or could improve on. The nurses are asked various questions in regards to the leaders and managers they work directly under and whether their needs are being addressed. The facility offers committees that focus on allowing staff to have voice. Some include unit base council for each unit of the facility, committee for staffing concerns and safety committees that focus on sentinel events.

The first step to gaining the support and funding with your idea to improve patient care is to perform research and be prepared. One should be able to support their ideas with literature and evidence based practice. I would input my idea along with the evidence based practice into a neatly design PowerPoint. The presentation would last approximately 10 minutes. The presentation will clear and simple for the upper management team to understand. The presentation would state the purpose of the idea, benefits of the idea, cost of the idea, and the population that the idea is targeting. I would show compassion for my idea while connecting with the upper management. At the end of the presentation there will be allotted time for any questions that upper management may have.

Top tips for presenting an effective presentation: Show compassion and connect with the audience, focus on the audience needs, keep it simple, smile and make eye contact, start strong, keep slides to the minimum of 10-30, tell stories, use your voice effectively, body language, and relax, breath, enjoy (Anderson, 2013).

My idea to improve patient care, I would use visual presentation.  The visual presentation would be a PowerPoint presentation with charts and possibly graphs.  This type of communication tool would be great because (a picture is worth a thousand words). The upper management individuals are usually busy.  It is the first impression that counts.  The presentation would be kept short, simple, and to the point. It is not about shoving the idea and its numerous merits down your audience’s throat.  Think about how you can carefully usher your idea through the approval process. I would want them to remember what I presented. They would not remember through words but through charts or graphs.  “Graphs or charts help people understand data quickly (mindtools.com). Whether you want to make a comparison, show a relationship, or highlight a trend, they help your audience see what you are talking about.

Good communicators use the KISS (“Keep It Simple and Straightforward”) principle. They know that less is often more, and that good communication should be efficient as well as effective (mindtools.com)
