Introduction

Whether you are a nurse, a public health professional, a health care administrator, or in another role in the health care field, you must base your decisions on a set of ethical principles and values. Your decisions must be fair, equitable, and defensible. Each discipline has established a professional code of ethics to guide ethical behavior. In this assessment, you will practice working through an ethical dilemma as described in a case study. Your practice will help you develop a method for formulating ethical decisions.

For this assessment, you will develop a solution to a specific ethical dilemma faced by a health care professional. In your assessment:

1. Access the Ethical Case Studies | Transcript media piece to review the case studies you may use for this assessment. 

o Select the case most closely related to your area of interest and use it to complete the assessment.

o Note: The case study may not supply all of the information you need. In such cases, you should consider a variety of possibilities and infer potential conclusions. However, please be sure to identify any assumptions or speculations you make.

2. Identify which case study you selected and briefly summarize the facts surrounding it. Identify the problem or issue that presents an ethical dilemma or challenge and describe that dilemma or challenge.

3. Access the Ethical Decision-Making Model | Transcript media piece and use the three components of the ethical decision-making model (moral awareness, moral judgment, and ethical behavior) to analyze the ethical issues.

4. Analyze the factors that contributed to the problem or issue. 

o Identify who is involved or affected by the problem or issue.

o Describe the factors that contributed to the problem or issue and explain how they contributed.

5. In addition to the readings provided, use the library to locate at least one academic peer-reviewed journal article relevant to the problem or issue that you can use to support your analysis of the situation. 

o Cite and apply the journal article as evidence to support your critical thinking and analysis of the case.

o Assess the credibility of the information source.

o Assess the relevance of the information source.

6. Discuss the effectiveness of the communication approaches present in the case study. 

o Describe how the health care professional communicated with others.

o Describe the communication and communication strategies that were applied, both in creating and in resolving the problems or issues presented.

o Assess instances where the professional communicated effectively or ineffectively.

7. Discuss the effectiveness of the approach used by the professional related to any problems or issues involving ethical practice in the case. 

o Describe the actions taken in response to the ethical dilemma or challenge presented in the case study.

o Summarize how well the professional managed professional responsibilities and priorities to resolve the problem or issue in the case.

o Discuss the key lessons this case provides for health care professionals.

8. Apply ethical principles to a possible solution to the proposed problem or issue from the case study. 

o Describe the proposed solution.

o Discuss how the approach makes this professional more effective or less effective in building relationships across disciplines within his or her organization.

o Discuss how likely it is the proposed solution will foster professional collaboration.

9. Write clearly and logically, with correct use of spelling, grammar, punctuation, and mechanics. 

o Determine the proper application of the rules of grammar and mechanics.

10. Write using APA style for in-text citations, quotes, and references. 

o Determine the proper application of APA formatting requirements and scholarly writing standards.

o Apply the principles of effective composition.

o Integrate information from outside sources into academic writing by appropriately quoting, paraphrasing, and summarizing, following APA style.

metastatic breast cancer, Margaret’s prognosis
is less than 6 months. Margaret’s cancer
has metastasized to her ribs and liver and
often causes intense pa
in. Although Margaret
speaks freely of her impending death, her family
has expressed their discomfort at her
decision to secure hospice care.
Over the last 10 years, Margaret, a nurs
e, has practiced homeopathy (for herself
and others) and consults frequently with a
medical intuitionist. She follows the medical

RISH CASE STUDY  
The O’Rourke family lives on a small farm in Iowa and comprises David, aged 30; his
wife, Mary, aged 29; and two children: Bridge
t, aged 7, and Michael, aged 6. Both David
and Mary are second-generation Irish. Before
purchasing their farm 5 years ago, David
sold farm equipment in Ohio. The O’R
ourkes are Catholic; Mary converted to
Catholicism when they married.
David, who works long hours outdoors, is
concerned about profitability from
his corn crop because of the unpredictable size of the harvest, and thus, his income
varies depending on the weather. Mary di
d not work outside the home because she
wanted to be with their children until they started school. However, because both
children are now school age, Mary has discus
sed with David the possibility of working
part time to supplement the family income. He
would prefer that she stay at home, but
Mary is anxious to return to the wo
rkforce and believes the timing is right.
Both David and Mary are happy with ju
st two children and do not desire more.
They use the rhythm method for family planning.
Eating a healthy breakfast is important
to the O’Rourkes. Because eggs are
readily available on the farm, they have frie
d eggs with potato bread and juice at least
four times a week. Their main meal in the
evening usually includes meat, potatoes, and
a vegetable. David enjoys a
glass of beer with dinner.
David has been a little edgy lately becau
se of his concerns about the corn crop.
He admits to having some minor chest pain, wh
ich he attributes to indigestion. His last
visit to a physician was before their marri
age. Mary knows David is concerned about
finances and believes it would help if she had a job.

Bridget and Michael spend
a lot of time outside playing and doing some minor
chores for their parents. Both children
enjoy school and are looking forward to
returning in the fall. Bridget is starting to
show concern over her appearance. She does
not like her red hair and all the freckles on he
r face. Her teacher has noted that Bridget
has trouble reading and may need glasses.
Michael wants to be a farmer like his Dad
but worries about his Da

thony, aged 28, who were born in this

children speak fluent Italian. Anthony is
an attorney and does not visit with his si

teacher, married to an Italian man, Guido
Venetto, who recently immigrated from
southern Italy and is 10 years older than Lu

Case Study 1

Mrs. Jones, who is 80 years old, presents to ABC Medical Associates today for her annual wellness visit. She was last seen 6 months ago by a doctor in the practice for a bout with influenza.

In today’s visit, the patient will receive her annual gynecological evaluation in addition to a general maintenance examination. She will also have blood work done to evaluate her hypertension, hypothyroid, and type II diabetes.

Mrs. Jones is covered by Medicare and AARP Medigap insurance.

Based on the information in this case study, determine if this is a new or established patient. Then, describe what information you will need to collect from the patient and what steps you must take to ensure the services being rendered will be covered by insurance. Determine which insurance policy is primary.

Case Study 2

Mr. Jones is seen in the ER at Grand Valley Hospital for chest pain. He is from another state and is visiting family. He has never been to Grand Valley Hospital previously.

While in the ER, it is determined that the patient needs a cardiac catheterization done before discharge. This procedure is performed by Dr. Lindsay.

Mr. Jones serves in the National Guard and has been covered by TRICARE since 1989. He is also covered by his employer’s health plan, Blue Cross Blue Shield, and has been covered by this health plan for the past 2 years.

Based on the information in this case study, determine if this is a new or established patient. Then, describe what information you will need to collect from the patient and what steps you must take to ensure the services being rendered will be covered by insurance. Determine which insurance policy is primary.

  

Case Study 3

Jack Minnow, a 4-year-old boy, presents to Valley Allergy practice because of a recent allergic reaction to a bee sting. He has never had a reaction previously. Jack’s primary care physician Dr. Zane is referring him to Dr. Shelby, an allergist, for a consultation. Jack is accompanied by his mother who is the custodial parent.

During the visit, Dr. Shelby performs a series of allergy tests that identify not only an allergy to bees, but also to animal dander and grass.

Jack is covered by both his mother’s and his father’s employers’ health plans. His parents are divorced. His mother went back to work after the divorce 1 year ago and enrolled Jack under her health plan due to some concerns about whether Jack’s father would keep his job. However, his father is still employed with the same company and has worked there for the past 3 years.

Based upon the information in this case study, determine if this is a new or established patient. Then, describe what information you will need to collect from the patient/guarantor and what steps you must take to ensure the services being rendered will be covered by insurance. A 70-year-old female is admitted to your unit for changes in bowel/bladder function. Her initial lab work was WNL. Her baseline vital signs were BP 124/62, HR 84, RR 20, Temp 98.4.  The MD performed a bowel resection after it was determined that the pt has colon cancer. Post-operatively the pt is taken to a medical/surgical unit. She has an IV of NS at 100ml/hr., and an NGT to intermittent suction, a foley catheter, and a midline abdominal dressing that is dry and intact.  

You are assigned the patient post-op day one. Her VS @ 0700, BP 100/60, HR 104, RR 24, Temp. 99.2 orally. the pt is alert and oriented; her color is pale. the NGT is draining brownish-green mucous, the abdominal dressing is D/I, and the IV is at 100 ml/hr. The pt says, “I’m so tired and I feel so nauseous, and it hurts to move.”

The physician ordered the pt to be out of bed. When sitting at the bedside at 1000, the pt states, “I don’t feel very well.” after waiting 30 seconds, the nurse assists the pt to the chair. The pt becomes dizzy and her bp is 90/64, HR 114, and the urine output has been 70 ml since 0600. 

1) What would you do first with this pt? List 3 priority interventions you would implement at this time?

Consider the below case study:

Margret Spinner-Ramirez is a 66 y/o female Hispanic-American who speaks both Spanish and English fluently. She is retired, lives alone, and has Medicaid as her primary insurance.  Ms. Spinner-Ramirez was scratched by a stray cat that she feeds on her back porch daily. She has been cleaning the wound daily; however, after 2 days she went to her local ER for increased pain, redness, and swelling in her left lower leg wound where she was scratched by the stray cat.  Ms. Spinner-Ramirez explained she recently had her left knee replaced about 6 months ago and verbalized new difficulty with baring weight on that extremity to the point that she was having to use her cane again. Her vital signs at the ER visit were stable.  Her left leg wound was cleaned and redressed.  After 5 hours in the ER, Ms. Spinner-Ramirez was discharged to home on oral Keflex for her left lower leg infection and was instructed to call her primary care physician for a follow-up appointment. Five days later Ms. Spinner-Ramirez was taken back to the same ER via ambulance.  Her neighbor found her lethargic, short of breath, and was experiencing difficulty being able to move. A CT scan and blood work revealed that Ms. Spinner-Ramirez’s knee replacement in her left leg was infected secondary to the cat scratch. She was admitted as an inpatient for sepsis. She needed to have a second left knee replacement surgery with wound irrigation and debridement, which extended her inpatient stay to 3 weeks.  Once she was discharged to home she required six weeks of IV antibiotics, extensive rehab, and home health.
