Identify an experience where you had to assess the needs, interests, and goals of a patient with an ethnic background, different from yours, that resulted in a positive learning experience. How did you turn the situation into a positive learning experience? Did you personally have to deal with any stereotypical beliefs?

When I was working in SICU (Surgical Intensive Care Unit), I was assigned to a patient who was admitted to ICU after an episode of bradycardia and loss of consciousness on the medical surgical floor. A rapid response was initiated and she was intubated and brought to our unit. She was Haitian and came from a very different ethnic background. At the time I was not familiar with their culture or beliefs nor did I know her health history and what originally brought her to the hospital or the reason for her admission.

After a couple days of being assigned to her and taking care of her and her family, I began to understand a little more. She had been diagnosed with breast cancer the previous year and had undergone chemotherapy which unfortunately was unsuccessful. The cancer had spread throughout her body including her esophagus which was later found out to be the reason for the intubation. A tumor had attached itself to her airway and she was unable to breath on her own. 

I instantly became attached to the patient and family and enjoyed learning about their culture and belief system. Many people including doctors, nurses and other medical professionals couldn’t understand why a patient who was terminal would want everything done and continue aggressive treatment. Many gave up on her and the family which is why I believe they requested that I take care of her every shift I worked. Special arrangements were made between the family and my director to accommodate their wishes. 

Each shift I worked, I took care of her. Her daughter and her husband never left her side other than for bathing or other emergent matters that couldn’t be handled without them. Eventually the primary physicians would stop rounding on her daily and healthcare professional started discussing her prognosis, treatment and plan of care. No one understood why we were continuing to treat this individual as they called the treatment “futile”. 

As time went on, the hospital decided to call in Risk Management as they had a different belief and understanding regarding by patient’s wishes and medical management. This is when I realized I did, I understood her and her family and knew I needed to be her patient advocate and her voice. I fought for her treatment and plan of care which was aggressive not palliative. She expressed to me she wanted to live, even if it meant her last days were spent in the hospital. She would lay there and listen to her children laugh and tell stories about their day and her husband’s voice telling her how much he loved her. She was a mother of 10 and had the most beautiful family I had ever met. 

Before I met her, I may have had the same thoughts and comments as the other healthcare professionals. However, once I got to know her and her family as well as their cultural beliefs and background, I understood her. This was more than a positive learning experience it changed my life, my outlook and opened my eyes to cultural diversity and understanding.

The testing and assessment process is often a struggle for educators. For many, finding the balance between a seemingly “hard” or “easy” question takes much practice. Furthermore, the development of question formats and stems can be a difficult concept to master. To aid in quiz development, nurse educators can use models such as The 3Cs (illustrated below) to focus more on the content of their questions and less on the actual construction. Nurse educators can also use prompts and templates provided at quiz development websites to assist quiz design. These websites can also help instructors easily administer quizzes in the online environment.

The 3Cs Model of Test Item Writing:

  • Content, concepts, and criterion. Use the content of the lesson as well as overarching concepts and nursing criterion (like NCLEX and other formal assessments) as a blueprint for your questions. In addition, consider the ideas and teachable moments that have occurred in the classroom or during clinical rotations.
  • Clarity and complexity. Be sure that your question stems are clear, straightforward, and use simple language. In addition, write your questions at the application or analysis level of Bloom’s Taxonomy. This will prompt students to move beyond basic comprehension and memorization.
  • Critique. Always go back and examine the questions that you have created. This process should be completed both before and after examinations. As with all teaching strategies, evaluation and revision is imperative to ensure the effectiveness of instruction.

  • Review this week’s Learning Resources on developing multiple-choice quiz questions. How might you use the 3Cs and other best practices to craft your own quiz questions?
  • Reflect on a Lesson Plan that you have created in this course. Which lesson might lend itself towards a pre- or post-quiz?
  • Select one Lesson Plan you will use as the focus of your online quiz. Then, craft three quiz questions that are closely linked to the learning objectives and learning need of the lesson.
  • Consider the challenges that you experienced in developing your quiz questions.
  • Browse the available websites that educators can use to create online quizzes. After reviewing the format and capabilities of various sites, select one to upload your three questions.
  • Be sure to copy the web address (URL link) that your colleagues will need to view your quiz.
  • Update your Lesson Plan with your three quiz questions and resave with the word “updated” at the end of the file name.
  • Consider any challenges that you may have experienced in using the platform that you chose and how you dealt with those challenges.

Setting: Hospital – Director’s Office

The interview was conducted as a one-on-one informal discussion in the Director’s office at the hospital during day shift. There were a few interruptions including a code blue however, we eventually got through it and it was very interesting and informative.

Interview Q&A

Q. Please share with me an example of how you helped coach or mentor someone. What improvements did you see in the person’s knowledge or skills?

A. Early in my nursing career, I worked on the Med-Surg unit and was orienting a new nurse to the unit. She had a patient that was very ill and had multi-organ failure. She was waiting for a hospice consultation when the patient coded. At that time, the patient was a DNR therefore she was not resuscitated. The nurse felt helpless and did not’t know what to do. She literally broke down in tears. I had a one-on-one conversation with her and explained the of end of life process and shared my first experience when I lost a patient. She later came back to me and thanked me for sharing with her as it was very helpful to process hers.

Q. Tell me how you organize, plan, and prioritize your work.

A. I begin each day with staffing which sets the stage for the rest of the team and the day. After staffing I meet with the day shift nurses, prior to them getting report to review and discuss any concerns or new implementations. I then meet with other directors and bed management to evaluate our bed availability and what needs to be done with any transfers or discharges. This information is communicated with the charge nurse and he or she will use this information to plan their day. Obviously, there are many obstacles that may arise or issues that may need to be resolved throughout the day which can’t be planned for. At the end of the day there is another staffing meeting to prepare for the next day. I guess you can say each day begins with and ends with staffing, the backbone to a successful unit and patient experience.

Q. Please share an experience in which you presented to a group. What was the situation and how did it go?

A. I have presented to groups on multiple occasions however, the one that comes to mind is my first. At the time, I was promoted to Clinical Coordinator. I had never been in a management position before and was extremely intimidated and anxious when I had to conduct my first staff meeting. I prepared several days in advance and even rehearsed how I would run the meeting. I did not’t sleep the night before and was worried I would fail my director. Everyone gathered in the break room and I handed out the agenda. At that time, I realized I had nothing to worry about and this was my team. I conducted the meeting flawlessly and took a deep breath when it was over and smiled. 

Q. Share an example of when you went above and beyond the “call of duty”.

A. Wow, that is a tough one. I don’t know that I can remember one in particular. I think it is something I do on a daily basis or whenever the situation arises. I believe that being a nurse you often do more than you are “asked” to do. That is what makes our field and profession different from others. We take care of people. It’s an amazing career.

Q. In your experience, what is the key to developing a good team? 

A. I believe the key to developing a good team starts with good communication. Which includes listening to staff complaints and concerns. If something isn’t working you can try to figure out a solution or how to resolve it. If you don’t know what is wrong, you can’t fix it. Treating others with respect is also key as it promotes professionalism and a positive attitude. All of which are important elements in a good team as well as many others.
