Utilizing the information you have gathered over the weeks regarding the specific illness group you identified, this week, you will create a plan of care for your chronic illness group.

Create the plan in a 4- to 6-page Microsoft Word document (the 4–6 pages include the holistic care plan). Include the following in your plan:

  • Start the paper with a brief introduction describing the chronically ill group you selected and provide rationale for selecting this illness and the participants. Clearly identify the Healthy People 2020 topic chosen and why this topic was chosen.
  • You will want to compile the information gathered from Weeks 1–4 over 2 to 3 pages   (((((SEE ATTACHED DOCUMENTS)))). . This should be in APA format and paragraph form. This is not to be copied and pasted from previous assignments. It is to be a summary of each week. 
  • The paper should include the care plan for your chronic illness group organized under the following headings:
    • Nursing Diagnoses
    • Assessment Data (objective and subjective)
    • Interview Results
    • Desired Outcomes
    • Evaluation Criteria

    • Evaluation of Patient Outcomes

Healthy people 2020 has many goals. I feel nurses can greatly impact the healthy people 2020 goals by first teaching patients about prevention. Healthy people goals for global health is one of those preventions. “Improve public health and strengthen U.S. national security through global disease detection, response, prevention, and control strategies.” Disease detection starts with well check-ups with your primary care physician. Nursing strategies include educating patients on the importance of keeping check-up appointments. Any chronic illness to be assessed and any new information to be given to patient verbally or written materials given to patient.

Secondly, we as nurses can introduce health promotion. Healthy people 2020 in immunizations and infectious disease goals, “Increase immunization rates and reduce preventable infectious diseases.” Floor nurses can educate patients before discharge on influenzas, pneumonia, or shingles vaccine. We in the cancer center as nurses give handouts on neutropenia symptoms and how to prevent infection (we have many patients that are or become neutropenic). We handout thermometers and give face masks to our patients.  According to NIH.gov, “Nurses must have an evidence-based understanding of the significant effect that can be made through health promotion interventions and communicate this understanding to the public at large. As more people grow in their awareness of activities that lead to good health and become knowledgeable about their own health status and the health of their families, the overall health of the population will improve.” At my hospital where I work we have health screening fairs a few times a year. Other hospitals around our town offer that as well.

According to my current other class Caring for the Community in week 2 it discusses the first level of prevention which is primary prevention. “This is conventional prevention—preventing an undesirable thing from happening. It includes efforts at immunization, nutrition teaching, and education about modes of transmission of communicable disease. For example, encouraging people to protect themselves from the sun’s ultraviolet rays is an example of primary prevention of skin cancer. Other examples in primary prevention include well baby clinics, HIV community education efforts, and programs that discourage substance abuse. In the community, this may involve fitness events, water fluoridation, or even efforts to install sidewalks near schools.”

      As nurses, I think that we can impact the Healthy People 2020 goals by carrying them out. It’s very important that what we do it’s always thinking on the safety, improvement, healthy lifestyle, prevention of illness of every patient we come across. With this, we also as we discussed in week 4, must keep in mind the well-being of lesbian, gay, bisexual and transgender (LGBT) individuals. Research suggests that LGBT individuals face health disparities linked to societal stigma, discrimination, and denial of their civil and human rights. There is a need for more research to document, understand, and address the environmental factors that contribute to health disparities in the LGBT community (AMN Healthcare, 2015).

      Nurses can impact community health positively by committing, serving as leaders, contributing and reinforcing health equity to achieve optimal health for all populations. Realizing the vision of healthy people in healthy communities is possible only if the community, in its full cultural, social, and economic diversity, is an authentic partner in changing the conditions for health.(Institute of Medicine, 2012).

When reviewing the Healthy People 2020 objectives for Access to Health Services, all of their objectives are legitimate and important.  All of the topics are problems in our healthcare system, however, I feel that increasing the persons with a usual care provider and increase the number of practicing care providers is the most important (Healthy People, 2018).  I do not know if this is just a problem in California, or if it is nationwide, but patients in my area have an extremely difficult time finding a primary care provider, whether it is a Physician, Physicians Assistant or Nurse Practitioner.  There is such a shortage in my area, that patients have to wait months to get established with a provider.  Then when they are ill or have an acute problem their provider is unable to see them within a few days because there is no appointments for weeks.  Most end up going to a prompt care type setting or to the emergency department.  I believe that if we can increase the number of practicing care providers patients will then be able to have a usual care provider that can see them in a timely manner for more acute problems and to adequately manage the patients chronic illness, leading to fewer emergency department visits and hospitalizations.  

This is one of the reasons I would like to pursue a Master’s Degree and become a Family Nurse Practitioner after obtaining my BSN.  I see so many patients in the Emergency Department, that are there every week for problems that would be easily treated by a physician or mid-level.  Most of these patients are elderly without access to resources and are failing outpatient treatment.  When you have a practitioner that can see the patient, set up in home resources and follow up, the patients outcome is likely to much more successful.  

It appears that the shortage of primary care providers presents a challenge for the entire US healthcare system and is not only in my area.  Historically, physicians did not present with a positive attitude when the topic of including PAs and NPs, as key players in augmenting primary care.  Current medical practice has become a complex, multilayered problem, that is requiring a team approach.  Physicians alone will be unlikely to meet future demands for primary care and and also specialty care. Therefore, greater attention will likely be focused on the PA and NP profession and its ability to continue to adapt to meet society’s healthcare needs (Coplan, Smith & Cawley, 2017).
