Topic 1: Vulnerable Populations

As you discovered in this week’s lectures and textbook readings, numerous vulnerable populations exist in the community. Identify three local organizations in your community that deliver healthcare services to vulnerable populations. Based on your research findings, answer the following:

  • Which population does each organization serve?
  • What services does it provide?
  • Where does it get its funding (local, state, or federal sources)?
  • Evaluate the relative value of each organization’s services to individuals and families in the vulnerable population it serves.

Topic 2: Disaster Management

Contact your local public health department to learn about its role in a local disaster, including the role of the nurses who work there.
I called the ……… county health department and spoke with a Mr. ………… He explained to me that he has a list of local nurses that he sends out an email to in order to ask for assistance when there is a need. Currently he is working out at the panhandle and stated that he does have a few volunteers from here who are helping to wrap things up at some of the shelters from the last hurricane. It’s nice that all the countries that have help available come in and help when there is a need. He said that sometimes they need nurses to help with flu shots and such and so he sends out an email and if you are available you just show up. He asked me if I knew anything about the Medical Reserve Corps (MRC). I had only just read about it in our material this week. As you guys may already know the MRC is a government organization comprised of a large network of volunteers that come together to improve the welfare and safety of a community (HHS, 2016). Mr. ………asked if I was interested in being a part of it.

Would you be a good candidate to serve on a disaster team?
I do believe that I have something to offer the community, we all do. According to the information that Mr. …….. gave me, I have the opportunity to decline when I am needed if I have other obligations. So of course my job and my family come 1st. I did sign up to be part of the e-mailing group and Mr. ……. will be forwarding my information to another colleague in order to set up membership to the MRC.
Would your current employer be flexible in allowing you to participate in a disaster situation?
I am already part of the disaster team for work, so they would take priority. If I were PRN, I am sure they would be accommodating, but I am a full time staff member, and since being part of the MRC is voluntary, they would not be a priority for me at this time.
What is your personal preparedness to face a disaster?
We have a hurricane/disaster box prepared that has everything needed to help sustain us for 1 week. I have lived in FL for so long that I grew up with a family plan and so I carry that with me now that I have my own family. I have only ever evacuated once and having our preparedness box made that process a lot less stressful.

Topic 1: Child and Adolescent Health Risks

Ze

Obesity is not exclusive to adults. Each day, more and more children are finding themselves at risk for overweight and obesity. Childhood obesity often accompanies many of the obesity-related conditions adults affected by obesity often experience, such as type 2 diabetes, hypertension, sleep apnea and more. Recent data shows that up to 80 percent of children affected by obesity will continue to be affected by obesity into adulthood (Lobstein, et all, 2015). Obesity impacts children in a variety of ways. First and foremost, a child’s health is impacted as they have now opened themselves up to a wide variety of health issues – issues that most of us didn’t experience until middle-age. In addition to health implications, there’s also one other area that children face which can be very serious – weight bias and bullying. Kids impacted by obesity often find themselves the target of bullying. This bullying can take place in the classroom, in your neighborhood and even in your own home. It is very important to recognize this type of behavior and address it quickly (Lobstein, et all, 2015). The Obesity Action Coalition (OAC), a nonprofit dedicated to educating and advocating for those affected by obesity, provides valuable resources on weight bullying.

Over the last decades, food has become more affordable to larger numbers of people as the price of food has decreased substantially relative to income and the concept of ‘food’ has changed from a means of nourishment to a marker of lifestyle and a source of pleasure. Clearly, increases in physical activity are not likely to offset an energy rich, poor nutritive diet. It takes between 1–2 hours of extremely vigorous activity to counteract a single large-sized (i.e., >=785 kcal) children’s meal at a fast food restaurant (Lobstein, et all, 2015). Frequent consumption of such a diet can hardly be counteracted by the average child or adult.

It has been hypothesized that a steady decline in physical activity among all age groups has heavily contributed to rising rates of obesity all around the world. Physical activity strongly influenced weight gain in a study of monozygotic twins (Rivera, et all, 2014). Numerous studies have shown that sedentary behaviors like watching television and playing computer games are associated with increased prevalence of obesity. Furthermore, parents report that they prefer having their children watch television at home rather than play outside unattended because parents are then able to complete their chores while keeping an eye on their children (Rivera, et all, 2014). In addition, increased proportions of children who are being driven to school and low participation rates in sports and physical education, particularly among adolescent girls, are also associated with increased obesity prevalence.

Almost all public health researchers and clinicians agree that prevention could be the key strategy for controlling the current epidemic of obesity. Prevention may include primary prevention of overweight or obesity itself, secondary prevention or avoidance of weight regains following weight loss, and prevention of further weight increases in obese individuals unable to lose weight (Rivera, et all, 2014). Until now, most approaches have focused on changing the behavior of individuals on diet and exercise and it seems that these strategies have had little impact on the growing increase of the obesity epidemic.

Topic 1: Child and Adolescent Health Risks

As you discovered in this week’s lectures and readings, several populations face multiple health risks across their lifespan. Children and adolescents are a population that is at a higher health risk for obesity. A national movement is underway to reduce risk factors for developing obesity in children. Part of this movement is the “Let’s Move!” campaign, which is a comprehensive and coordinated initiative to prevent childhood obesity. The initiative emphasizes four primary components: healthy schools, access to affordable and healthy food, raising children’s physical activity levels, and empowering families to make healthy choices.

  • Review Healthy People 2020. Identify objectives that will combat childhood and adolescent obesity.
  • Provide suggestions on how community health nurses can contribute to these national health objectives and accomplish the goal of decreasing obesity among this population.

Epidemiology’s Role in Infectious and Communicable Diseases

As you discovered in this week’s lecture and readings, epidemiology studies various occurrences and health-related states or events that effect the general population. Epidemiologists examine the dispersion of health events in populations and use this knowledge to control and prevent future health problems.

Topic 1: Disease Outbreak

Select an infectious disease and research the CDC website for information about the disease, its natural history, presenting symptoms, and outbreak characteristics. Identify an occurrence of the disease by searching the Internet for recent reports of this disease and compare that episode or occurrence with information from the CDC website.

  • How closely did that outbreak resemble the case definition?

The ACA or the AHCA: Imperfect Solutions to Increasing Access to Health Care?

.

To support your work, use your course and text readings and also use the —————–University Online Library. As in all assignments, cite your sources in your work and provide references for the citations in APA format.

This week’s discussion focuses on the federal government’s role in protecting the public’s health using the ACA and current efforts to “repeal and replace” the ACA. Please note that the government’s role in protecting the public’s health did not begin with the ACA. In the late 1800s, Lilian Wald brought health care to the community. And in 1965, Congress passed bills authorizing Medicare and Medicaid. Since 1965, Congress has expanded government-sponsored health care to pregnant women and children.

Answer the following question:

· Compare the current provisions in the ACA with those in the failed first version of the AHCA. Share your opinions about why the AHCA failed in the first attempt at passage.

Continue to read daily from one of the policy briefs that are included in your weekly readings list. POLITICO Pulse is a daily briefing from POLITICO, and the Kaiser Health News Morning Briefing is a daily briefing from the Kaiser Family Foundation. Both focus on health care. Either one will keep you abreast of what is happening in health care policies and politics.

In addition, subscribe to the “HealthCetera” podcast and listen to the podcast weekly. This is a production of WBAI, free speech radio of Pacifica Foundation Radio in New York City. Diana Mason and Barbara Glickstein are founders and codirectors of the Center for Health Media Policy (CHMP). Both are recognized as experts in media, nursing, health, and health policy. They have a longstanding relationship as producers and moderators of HealthCetera (https://www.wbai.org/index.php?option=content&task=view&id=384&Itemid=42), a live, award-winning radio program on public radio and iTunes. They are bloggers for Disruptive Women in Health Care( http://www.disruptivewomen.net/)and for the American Journal of Nursing (AJN) (https://journals.lww.com/ajnonline/pages/default.aspx). The CHMP is an interdisciplinary initiative for advancing the health of the public and healthy public policies through media, research, education, and public forums. Listen to the podcast throughout the course and think on the topics that are discussed, especially as they relate to how we advocate for issues of importance in this contemporary political environment.

PART 1

The Affordable Care Act and the American Health Care Act Compared

The provisions of the Affordable Care Act (ACA) directs all employers to provide minimum value and affordable medical cover to all full-time employees or else be penalized whereas the American Health Care Act (AHCA) removed the penalty on defaulting employers (Conley, Dygert & Sellstrom, 2016). Under the ACA, it is the people’s mandate to buy healthcare coverage whereas the AHCA abolished the penalty requiring individuals to purchase health cover. However, the AHCA issues penalties to individuals who fail to pay their healthcare coverage for more than 63 days in the last 12 months, and youth who are past the age limit under parental cover and did not enroll for health cover of that year (Conley, Dygert & Sellstrom, 2016).

The penalty is a 30% surcharge in premiums, which can last for a period of 12 months (Conley, Dygert & Sellstrom, 2016). On the aspect of coverage affordability, ACA provides for cost-sharing subsidies in low-income families or individuals to allow for exchange plans for up to an average of 70%. The premiums to be paid by such individuals and families have tax reductions to enable low-income earners afford healthcare insurance (Mach, 2017). On the other hand, the AHCA offers credits on the basis of age with fixed amounts such as below 30 years-$2000, 30-40 years-$2500, $3,500-50-60 years, and it faced out credits for people earning incomes that exceed $75,000 (Conley, Dygert & Sellstrom, 2016). The ACA removed additional charges for health status whereas AHCA charges 30% standard rate for people with preexisting health complication for the past 12 months (Mach, 2017).

Opinion on Why AHCA Failed in the first Passage Attempt

The most likely reasons for the failure of AHCA during its first passage attempt were due to its provisions that would impact negatively in low income families and the less-privileged individuals with regard to their preexisting health status. For instance, the AHCA lifted penalties that employers were to be charged if they defaulted paying the compulsory health cover for all their permanent employees. This implies that employees could pay their employees’ cover only when they wished, which could push the burden back to employees. The other reasons are that people with preexisting medical conditions would be required to pay more premiums, and abolished sharing of insurance cover, and charging premiums based on age instead of economic capabilities. This means that you can be charged little premiums even if you earn high, and you can be charged high premiums even if you poor depending on your age.

PART2

Your comparisons of the ACA and AHCA were informative.  How did the mandate for employers to pay for employee’s healthcare insurance affect the employers?

Nurse’s Role in Influencing the Legislative Process

For this week’s project, you’ll answer three questions related to your opportunity as a nurse and an advocate. The first two questions relate to influencing a bill is traveling through the legislative process. The third question refers you to the social determinants of health as described in your course textbook reading and asks you to think specifically on your role as a nurse in influencing social determinants of health.( SEE ATTACHED)

In a brief paper (no more than 2 typed pages, 1 inch margins, 12 font Times New Roman or Courier New), answer the questions below. Write each question as a new topic area and then follow with a paragraph or two to answer the question. You may find it necessary to search for answers to the questions outside of the assigned reading. Be sure to use APA guidelines for writing style, spelling and grammar, and citation of sources.

Answer the following questions:

· What parts of the lawmaking process does a nurse have an opportunity to influence the final bill passage?

· List at least two ways that this influence can take place. In your opinion, would one way be preferable over the other?

· Reviewing the social determinants of health, select one to respond to this question:

o What role can nurses play in promoting health status through policy changes directly affecting this social determinant?

Submission Details:

· Submit your response in a 2-page Microsoft Word document (500 words).

Week 2 Topic /Mar

Looking further at the “what” in Figure 1-6 and answer any one of the following questions:

o Discuss at least three competencies/strategies that you have used effectively.

Become knowledgeable about issues- I try to stay abreast on current issues through reading nursing journals and following politically based nursing articles online. Both sources have offered me valuable information about current nursing policies and trends.  Join Influential groups- I am part of the ANA (American Nurses Association).“The “ANA advances the nursing profession by fostering high standards of nursing practice, promoting the economic and general welfare of nurses in the workplace, projecting a positive and realistic view of nursing, and lobbying Congress and the regulatory agencies on health care issues affecting nurses and the public” (American Journal of Nursing, 2006). Through groups such as the ANA, nurses can make an impact on political processes. Through this groups meetings, journals, and website nurses can stay up to date on current events in nursing.  Make campaign donations- Through the ANA and my own research I have and will continue to make campaign donations to campaigns that support my position in nursing. Making donations to campaigns allows me a way to support individuals or groups who are reaching the public and public officials on a large scale.
