Some of the challenges of having different generations in the workplace include a difference in education, particularly vocational training, and a difference in communication style. As current practices are studied and re-evaluated, traditional practices may be replaced by those that are more recently researched and are evidence-based. Veteran nurses may believe it is a waste of resources to change a practice that has been in place for so long and, in their view, has yielded adequate results.  Similarly, younger generations may readily dismiss suggestions and viewpoints from veteran nurses in favor of the new practices learned in school without considering that years of clinical experience can be just as valuable. This disconnect between nurses of differing generations may be rooted in a difference of communication style. For example, Baby Boomer nurses were trained to only voice their opinions when asked, whereas Millennial nurses’ were taught to speak up any time they have a problem with, or idea for, new practices; this difference in communication may lead the Baby Boomer nurse to regard the Millennial nurse as impertinent and disrespectful of authority instead of realizing that new nurse is actually trying to improve the patient experience (Phillips, 2016, p.198).

In my experience, some veteran workers can be set in their ways to a fault, even if there is clear evidence that practices they are accustomed to are simply not the best practices anymore. This is not to say that middle-aged or younger workers cannot share this attribute, however the veteran workers sometimes use their field experience as a rationale to ignore current research findings and continue using practices they deem appropriate. Also, as an employee that is frequently on the team that rolls out new technology and equipment, I feel that veteran workers take more time to warm up to change and can be somewhat hostile to a younger person, such as myself, training them.

As far as accommodations, we have certain older employees that will not perform job-necessary duties because of back issues, knee issues, or other medical issues in the same vein. As a result, these employees rarely get assigned total care patients, making the younger staff resentful because they feel they, literally, do all the heavy lifting while these older employees are routinely assigned the less acute patients. To a certain extent, allowances should be made for older workers, including nurses. After a lifetime of caring for patients, it is reasonable to deduce that the joints, particularly knees and back, can be irreparably damaged from repeated use. However, in my opinion if an employee is unable to perform the duties required of their job title, they should be doing a different job. It is not fair that one group of nurses consistently gets less acute patients, thereby doing half the work, but still receiving the same benefits as those nurses that pick up the slack. A potential solution would be for the nurses with physical limitations to shift to a position that utilizes their experience but is less physically demanding; such positions include resource nurse, case manager, or patient care coordinator. If this were fiscally feasible, the unit would be able to retain the wealth of knowledge provided by these seasoned nurses and still have room to hire new nurses that can help distribute the patient loads more equitably.

            Life expectancy in the United States has increased over the past few decades and older people are generally healthier than their predecessors, however the incidence of chronic conditions has also increased in this time frame (Tabloski, 2014, p.8). This indicates that there are more people to care for and these patients are more likely to have multiple comorbidities. This trend has the potential to put an enormous strain, economically and physically, on an already overburdened health care system and nurses, particularly bedside nurses, will bear the brunt of this (Tabloski, 2014, p.13). It is important that nurses engage in primary and secondary prevention with this population, including education related to diet, exercise, and proper medication utilization (Tabloski, 2014, p.13). By encouraging older patients to take an active role in preservation of function and overall health, nurses empower these patients to help decrease their risk for chronic conditions, like hypertension, and injury (Tabloski, 2014, p.13). Also, nurses must be educated on specific attributes of the older population, such as increased sensitivity to certain medications, decreased sensory perception, and increased susceptibility to mental health issues, such as depression, that may affect their physical health or motivation to maintain a healthy lifestyle.le

The nurse caring for Mr. Jones should continue to educate him on the risks of non-compliance such as falls and hip fracture. Talking to Mr. Jones about alternatives to pain medication (such as lifestyle changes like exercise, weight loss and healthy diet) may prove to be helpful in this situation. Mr. Jones believes that if he uses a cane to get around, he will be seen as undesirable to women and that is affecting his willingness to comply with his treatment. Reassigning the patient to (or collaborating with) a younger female nurse who is empathetic to how he feels may be helpful and he may decide to listen to her advice. The nurse could also collaborate with the physical therapist to teach Mr. Jones some exercises that he can do in the privacy of his home since he is concerned with his image (Kotter-Grühn, 2015).

 The relationship between national and global health and nursing leadership is extremely important.        For instance, on a national level, one example is the undocumented population. Health disparities often occur due to lack of health insurance and access to healthcare (Huber, 2018). In the United States alone there are more than 12 million individuals without access to healthcare, many with chronic or long-term ailments (Sorrell, 2017). Lack of decent health coverage is an ethical issue because adequate healthcare is a basic human right and need (Sorrell, 2017). Healthcare should be fair, safe and impartial, and should improve the health of the population (Huber, 2018). One of the drawbacks is that healthcare and nursing leaders have lack of guidance concerning the authorization and delivery of healthcare to this population (Sorrell, 2017).

       How should the nursing leader advocate for these immigrants?  Nurse leaders and other healthcare providers need to publicly recommend and support policies that provide lasting and impartial health provisions for these individuals (Sorrell, 2017).  Nurse leaders need to join with lawmakers and organizations such as the Center on Health Equity Action for System Transformation or the Health Equity Task Force to help develop a health equity centered system (Families USA, 2019).

      The healthcare profession has numerous regulatory bodies ensuring patient safety is maintained at a high standard.  One such organization is The Joint Commission (TJC), an organization established in 1951 that provides voluntary accreditation through a very in-depth evaluation of compliance with standards of care of an organization (Wrzesniewski, 2017).  Continued accreditation establishes an organization’s determination to provide evidence-based, safe, quality care.  In 2001 TJC introduced a new requirement of a formal patient safety program (Huber, 2018).  The continuously elevated level of standards and safety measures required by TJC ensures safety is always at the forefront of health care.  Although medication administration error goals can be to keep errors to a minimum, it is impossible to avoid errors where the human element exists (Wang et al., 2015).  Hospitals can establish the building blocks needed to provide care of the highest standards through the safety framework required by TJC accreditation.  However, to assure continued safety and compliance it is important as a hospital culture to maintain standards and not push staff compliance before inspections (Wrzesniewski, 2017).  The standard of care and safety measures should be instilled in healthcare workers.  Policies and procedures should be adhered to and followed to the letter to avoid staff confusion and straying from the goal of safety.  Safety in an organization is a culture and must be preserved at its highest form. 

As nurses, we follow organizational policies and procedures, question when necessary, and advocate for change when applicable. Nurses are the last line of defense in medical errors.  This responsibility should not be taken lightly and a conscious continuous effort should be made to apply evidence-based practice when providing care to patients.  Patient safety should be the number one priority in any health care and to any given healthcare provider as we have all taken an oath to above all not do any harm to our patients.

            The organization that I had the pleasure of doing my Capstone practicum at was the Broward College Health Sciences Simulation Center (HSSC). The mission of this organization is to provide quality, interprofessional, experiential learning, using the best high-fidelity healthcare simulation practices. The HSSC focuses in preparing students and other healthcare professionals to be confident, compassionate, and competent in their field. This organization provides a safe and controlled environment where the faculty can offer guidance. They practice using tools, such as SBAR, that will be used in the real-world so there is a feel of better preparation.

            Healthcare system is an organization of people and resources to deliver services to meet the health needs of a targeted populations (Pina et al., 2015). This simulation center helps prepare the future healthcare professionals by guiding them through scenarios that they may come across in the field. It is such a benefit for students to come to this organization to utilize the equipment and gain knowledge and skills. Since the mission is to prepare healthcare students, while I was completing my Capstone I was able to see the relationship between the faculty and students which showed professionalism and direction. They were honoring their mission and philosophy which is to prepare these students to be safe health professionals.

It took an hour to get here but at least he could see his girl almost every day. Sure he had moved back in with his mom but it was easier to have someone to help him get around and cook for him. It was a bummer that he couldn’t work right now. He guessed it didn’t matter much since he really didn’t have a job to go back to. He had been replaced at the plant. They said they would find something for him to do once he could get around more easily. Eddie sure hoped the doc would take the pins out of his leg today and give him a clearance to work.
