Case Study Response

Volume 1, Case #14: The scatter-brained mother whose daughter has ADHD, like mother, like daughter

This case is about a mother who comes to the office with difficulty with focusing and forgetfulness. This is affecting her everyday life and being a single mom of 2, she is having a hard time with caring for them. The stress of what this mother is going through can lead to sadness and feelings of despair or depression. It is becoming more common for adults to have Attention deficit hyperactivity disorder than just for children(Stahl, 2013). If the children are experiencing symptoms such as trouble  focusing or completing tasks then most likely the parents will have some trait as well. For this client, it is important to ask about family history. Did any of your family members have symptoms associated with ADHD or anxiety? Are you having any thoughts of harming yourself or others? She complains a lot about her kids and her life and seems to be frustrated at times. We want to get her the help she needs as soon as possible. What are your strengths and weaknesses and how can you overcome some of the weaknesses? 

The first step for this client is to have her take the Adult ADHD self report scale. This is a self assessment that the client can answer truthfully and assist in the diagnosis of ADHD(Clancy et al. 2011). Differential diagnosis for this client include Generalized anxiety disorder, Depressive disorder, and Bipolar disorder. Further research would need to be conducted to determine the correct diagnosis. Since the patient matches symptoms for generalized anxiety and ADHD, these would be the two diagnosis and treatment modalities can begin. Stress can contribute to increased symptoms of ADHD  and anxiety. Stress relieving measures should  be discussed and reviewed with clients. Since ADHD affects the prefrontal cortex causing irritability and inattention. A stimulant medication can be given and is often given for clients with adhd(Stahl, 2013). Often times adjunct medications would need to be good to help relieve symptoms this one cannot(Psych, 2016). . I will remember to always start with the lesser approach to treatment using therapy and relaxation techniques before using the medications if possible.

The American Heart Association

There are many national and local’s non-governmental organizations (NGOs) that promotes health and wellness to all people. The American Heart Association (AHA) is one of the oldest and more actively involved in people’s health and diseases prevention. According to the American Heart Association (n.d.), their goal is to “see a world free of cardiovascular diseases and stroke.” They fund and conduct many researches, studies, events, and education materials to help fighting and preventing heart disease among all Americans and around the world. AHA promotes many ways to decrease heart diseases, which is the number one killer among people in the world. Regarding the promotion of wellness, we can certainly see how they encourage healthy eating habits, including recipes, types of foods that are healthy for the heart, cooking skills; healthy lifestyle, as stress management, quitting smoking, stress management, healthy sleep patterns, fitness, walking, exercising, and losing weight (AHA, n.d.). The AHA, at the same time, publishes studies and documents on many of the heart disease that most affect the population. They write and publish about aortic aneurysm, arrhythmia, atrial fibrillation, cardiac arrest, cardiomyopathy, cholesterol, congenital heart defects, diabetes, heart attack, heart failure, murmurs, valve problems, HTN, strokes, smoking cessation, and many more (AHA, n.d.). An example of the American Heart Association work to treat and eradicate heart diseases are the Guidelines for the Management of Spontaneous Intracerebral Hemorrhage meant for healthcare providers published by the American Heart Association/American Stroke Association (Hemphill et al., 2015). The AHA helps all people across the country and even the globe despite their cultural or ethnic background. NGOs goal is to improve the health and well-being of individuals, communities and nations (Jacobsen, 2019).

According to Feldstein (2006), the number one job of a legislator is to get reelected. With this in mind, we can see that the driving force of how legislators vote on policies is greatly affected by the opinions of the people that are voting for them. According to Joel Teitelbaum in Laureat Education (2018), the ACA, Affordable Care Act, that was put in place by the Obama administration, barely passed legislation with no votes from Republicans in office. With the shift now from Obama, a democratic president to now President Trump, a republican president and administration, we can understand how legislators may vote differently in order to win or gain the voting support of Americans who voted a republican president into office. According to Brodie, Dean & Cho (2011), the opinions of Americans regarding the ACA was dependent on the geographical location and the political influence in that region which supports the statement that an elected legislator from an area is “paid” in votes. It is to the benefit of the legislator to vote according to the likelihood that their opinion on the issue is going to get them reelected.

The same theory stands in place for other political issues such as Medicaid and Medicare issues. Legislators will support or push through a law that they feel will gain the favor of the majority of their voting area in order to secure their career. This supports the idea that nurses have a large impact on health care policies in the United States and the obligation to voice our opinions. Many legislators though intelligent, may just not have the information or know what medical professionals know about issues that are affecting people (Laureate Education, 2018 b). There are 2.82 million nurses in the United States (Registered nurses number U.S. 2001-2016, 2018) and when we learn how to assess the needs of our communities, create a policy and learn how to present and communicate these policies to legislators, we can make a difference for our community and the health of our country.

Nursing practice is always evolving as new evidence is brought forward in research studies.  Evidence-based practice is the use of the best evidence in making clinical decisions based on an integration of the best available evidence, most often from disciplined research, with clinical expertise and patient preferences (Polit & Beck, 2018). Nursing adopts EBP based on appraised and synthesized evidence determined to be relevant depending on clinical expertise and patient preferences.  EBP affects clinical practice and results in decisions that are clinically appropriate, cost-effective, and result in positive patient outcomes (Polit & Beck, 2018).  EBP is a process that involves asking an answerable clinical question (PICOT), searching for relevant research-based evidence, appraising and synthesizing the evidence, integrating the evidence with clinical expertise and patient preferences, and assessing the effectiveness of actions (Polit & Beck, 2018).  The effects must always be assessed for the actual outcome and if adjustments need to be made for a given population or location.  According to Polit & Beck (2018), a study by Shah and colleagues (2016) regarding the implementation of 2% chlorhexidine gluconate (CHG) bathing for intensive care unit (ICU) patients to prevent central line-associated bloodstream infections (CLABSI) EBP concluded that this implementation was effective in prevention.  Therefore, the nursing practice adopted HCG bathing for all patients with central lines as a proven EBP measure to prevent CLABSI (Polit & Beck, 2018).  However, EBP takes an average of 17 years to transfer from evidence to practice, in an effort to reduce translation time, translation and implementation science have been developed to aid in the more timely implementation of new knowledge into practice (Tucker, 2019).  The faster the translation time the more beneficial the implementation can be when applied at the bedside.

Nursing researches, evidence-based practice (EBP), and theories can be subject to reviews and readings. Nursing is an ever-changing science and profession, even though nurses based their knowledge on previous studies. Literature review is significant to the research process because it balances the strength of the knowledge obtained and review the previous information obtained. The nursing research process includes the review or creation of concepts and theories (Polit & Beck, 2018). Therefore, a deep reading and investigation of the research allows for either confirmation, or exclusion, of that research. As Bernhofer (2015) expresses, a thorough review of the literature can show that there is no need to change the practice or, in the other hand, it can show that there is a possibility to improve the outcomes desired by changing the practice protocols. A literature review may influence new researches process and practices as it proves the knowledge already obtained is based on strong and appropriate evidences that were not biased.

Complementary health modalities are the use of natural therapies and healing cultures for medicinal purposes. Examples of the alternative of conventional medicines are:

Traditional Alternative Medicine

 In ancient years, traditional medicine was used as a source of health care and also in a spiritual and cultural belief system through the use of herbal medicine to improve health standards. In the contemporary world, traditional medicine has been integrated into health systems (Aveni et al., 2017). Examples of alternative medicine are Ayurveda, homeopathy, naturopathy, and Chinese oriental medicine.

Body Therapy

Body therapy is a form of healing through touch used by the ancient man as a powerful healing force. It is believed that to achieve optimum body functionalities, one has to maintain equilibrium in the life energies. Thus, today, hand techniques are used to restore a balance in the flow of this energy in a human’s body. The healing touch theory is based on physical, mental, and emotional health aspects which results cannot be effectively measured (Isidoro, Huang, & Sheen, 2016). Some of the body touch therapies include chiropractic and osteopathic medicine, massages, tai chi, and body movement therapies.

Diet and Herbs

 Nutrition is an important factor in leading a healthy lifestyle. A good diet helps in the maintenance of a healthy weight and reduces the risks of contracting chronic diseases which promotes health. A nutrient deficiency is a major problem in today’s society; this is caused by poor feeding habits that have led to obesity and the development of chronic diseases such as heart diseases, diabetics, and cancer.  The health issues have instigated the development of dietary and herbal approaches such as dietary supplements in an attempt to achieve a balanced diet (Isidoro, Huang, & Sheen, 2016).

Nursing boards were created to establish guidelines for the scope of nursing practice. Each state has established its own board; thus regulations can vary by state. Advanced practice registered nurses must be familiar with their own state board’s regulations as it pertains to their practice, and advocate for the expansion of the nurse practitioner’s ability to practice and provide best care. 

Regulation 1

A main topic of conversation among nursing boards is the ability of advanced practice registered nurses (APRN’s) to practice independently of a collaborating physician. As discussed by Bosse et al., (2017), the ability of APRN’s to practice independently would help fill the gap in access to care associated with physician shortages and remote locations. Greater access to care has the potential to improve outcomes and reduce healthcare costs and complications. Currently, Missouri and Illinois both require APRN’s to maintain a collaborative practice agreement with a physician within a certain geographic range (Nurse Practice Act, 2018; Nursing & Collaborative Practice, 1996). The National Council of State Boards of Nursing (NCSBN) encourages APRN’s to advocate for independent practice in their states (Bosse et al., 2017). 

Regulation 2 

As technology plays an increasingly central role in the provision of healthcare, the use of technology must be addressed by regulatory boards. Telehealth is another solution to improving access to healthcare, especially in rural areas. Missouri permits APRN’s to practice virtually if the care is being provided to patients in remote areas (Utilization of telehealth, 2014). Illinois permits APRN’s to treat residents of the state via telehealth without geographical stipulations (Telehealth Act, 2018). 

Conclusion

As nurse practitioners, we can adhere to these regulations while also advocating for the advancement of the advanced practice registered nurse’s role in healthcare. Another opportunity for advocacy lies in the use of telehealth to improve access to care in rural areas. By providing exceptional care and demonstrating the knowledge and skills needed to practice independently, we can emulate the need to expand the scope of practice for nurse practitioners.
