Please respond to the two Scenarios in this paper (See the Answer format attached). Scenario 1: You are a new nursing student who has been hospitalized for injuries following a major car accident. Earlier today you were told the fracture in your right leg will require complicated surgery to repair and extensive rehab to recover. There is a question as to whether you will ever regain full use of your leg. The surgery is scheduled for tomorrow morning. Tonight you are alone in your hospital room and the night nurse has just entered. What are your concerns when you reflect on this personal patient experience? What will your experience be like? Scenario 2: You realize that you share a different values system from the nurse. You reflect that this might be challenging for you as a nurse someday. These values might be due to differences in lifestyle, cultural background, belief system, or attitudes about health care. Reflect on your own values and identify a personal value/belief that may become a conflict for you in delivering quality patient care.

Caring for the ethnically diverse older adult can prove to be challenging in efforts to maintain function and provide comfort. I will use an older African American male with hypertension. For this patient, the two strategies that I would implement would be diet and exercise. According to healthy people 2020, heart disease is the number 1 cause of death, and hypertension is a modifiable risk factor.
African Americans use a lot of seasonings, in particular salt, and this greatly impacts their chronic illness management. I would implement a dietary education strategy that includes salt substitution, and seasoning alternatives that keep food appetizing yet healthy. Also, we must integrate healthier meal choices, and refrain from fast food, frozen meals, and In order to maintain compliance with dietary changes and substitutions, food must taste good. Education is needed on reading food package labels and choosing lower sodium options.
The second strategy will be to educate and help implement an exercise regimen. Many older adults are hesitant to exercise because they feel that it may be time consuming, but there can be ways to incorporate it in small steps through out the day. The Mayo Clinic staff states that regular physical activity makes your heart stronger to pump more blood with less effort, and the end result is decreased blood pressure. Discuss adding walking during breaks and before and after work, then increase or add an activity at intervals to create a program catered to them.

The self-assessment is completed using the TIGER-based Assessment of Nursing Informatics Competencies (TANIC) tool. TIGER refers to the Technology Informatics Guiding Education Reform Initiative which identified a list of the minimum informatics competencies for all nurses and students graduating from pre- and post-licensure programs. When dealing with ethnically diverse older patients, it is important to understand and respect their cultural beliefs on health and wellness. Understanding the health belief paradigms that the person may relate to can assist in designing a health and wellness plan specific to that individual. Each culture has their own health belief system; Latino, African, Caribbean, African American, and Middle Eastern groups often believe in the Magico-Religious paradigm, Asian, North American, and Indian groups may follow the Holistic paradigm and the Scientific paradigm is common among Western cultures including the United States and Canada (Murray, 2019). Knowing their belief system can help the caregiver make informed decisions on their care. The Magico-Religious paradigm believes the health of the community comes before the health of the individual where the Holistic paradigm views the whole person in relation to the environment.
This knowledge can help decide what type of exercise regimen to begin a patient with a chronic condition on. Patients may benefit more from an individual exercise routine such as walking, cycling or swimming if they are more focused on holistic views. Patients with Magico-Religious views could benefit more from group exercise sessions. With group exercise the individual finds that the social connections improve the importance and effectiveness of physical activity (Beauchamp, Bennett, Dunlop, Estabrooks, Harden, Hurd Clark, Liu, Rhodes, Wolf, 2018). Knowing their preferences can assist the elderly in programs that are comfortable to their personal tastes.
A second strategy would focus on nutrition for the elderly person. While many dietary restrictions may apply to the individual based upon their chronic condition such as low salt, low cholesterol, low potassium or many other various restrictions, the individual needs to enjoy the food they consume or the risk of malnutrition from not eating may develop. It is important to include the older individual in decisions about their food to increase their appetite and quality of life (Dorner, Friedrich, 2018). Sometimes the rules of the diet have to be compromised with the individual to ensure proper nutritional intake.
