I too like Nightingale’s Environmental Theory.  I tell my patient’s in home health to open their curtains/blinds because the sun is good for the soul.  Sitting in dark rooms/homes does nothing for the mind, especially after or during illness.

I do think that in some areas of the world or even our country still have the same problems like they did back in Nightingale’s time. My sister in law just participated in building a home for a family in Texas. In their previous home (similar to a shack), they had no proper drainage. Sewage was leaching into the home causing all kinds of problems for the family.  Thankfully they had a great church in their area that is providing new homes for families.

A nursing conceptual model is used to set a guideline and is based on a concept.  Nursing theory

describes the knowledge used to support nursing practice and is based on evidence.

In Florence Nightingales Environmental Theory, she states that the nurse can utilize the patient’s

environment to help optimize their healing. This can be done by making sure certain environmental factors

are present such as:

– clean water

-fresh air,

-sunlight

-cleanliness of the environment

-and appropriate drainage

This practice is still used today to aid is patients healing. Although we no longer need to worry about

things such as appropriate drainage, we still maintain a cleanly environment, make sure the patient has fresh

clean water to drink if allowed. In the hospital setting we turn on lights and open blinds during the day and

turn off lights and close the blinds at night.  We fill out the “white boards” to help the patient stay oriented to

date and day as well as who their providers are and the goal for the day. In the hospital where I work, we

have what is called “Quiet Time” It is a 2-hour block during the day from 1430-1630 where the hallway lights

are turned down, the staff is expected to be quieter in their work and no over head paging is allowed. This is

used to decrease stimulation and promote healing in the hospital patients. Signs are posted all around the

hospital notifying staff and visitors of quiet time hours.

A Nursing theory is a set of concepts, definitions, relationships, and propositions derived from nursing models (Smith & Liehr, 2008). A conceptual model is a visual method of representing a set of causal relationships between factors that were believed to impact one or more biodiversity targets (Morgan, 2005). One of the main differences between the two is that a theory is based off research questions and study variables, while a conceptual model is used to guide research and can be tested and changed. Nursing theories are made up of conceptual models. Nurses like Virginia Henderson, Martha Rogers, Dorothea E. Orem, Betty Neuman, Hildegard Peplau, Madeleine Leininger, Patricia Benner, have created theories to help better understand the nursing profession. From different point of views, we can build our own concepts of what this career is about. Understanding this and extracting the best of each theory, we, as nurses, can take the level of healthcare to its maximum.

Betty Neuman developed the System Model, which is a nursing theory that focuses on the way that a patient responds to environmental stressors. These stressors include the intrapersonal, interpersonal, and extrapersonal types. It also involves the use of different levels of nursing prevention intervention (primary, secondary and tertiary levels) in order to maintain the highest level of wellness in the patient.  Nursing involves being concerned with the whole person, including the patient’s state of mind, which involves stress levels.

By applying nursing theories, effective nursing care can be provided for patients. An example in nursing practice in which Neuman’s theory would be effective is in treating patients that suffer from diseases with no cure, such as Multiple sclerosis (MS). This is an autoimmune disease of the central nervous system. It can take a toll on the patient’s emotions. The goal of treatment is to control the symptoms to be able to provide a better quality of life to the patient. For the patient, being able to speak about their situation to someone that would understand, such as a nurse, can help relieve stress. Nurses can ask and teach the patients to rest more, avoid certain foods, protect themselves from the sun, and other methods to help reduce fatigue and stress (Ahmadi & Sadeghi, 2017). By putting an importance on stress levels in a patient, the patient’s standard of life will improve. Reducing and preventing stress will help improve sleep patterns and reduce anxiety and depression. Also, being stressed, the body uses energy that is needed in order to maintain the immune system, and protect the body from illness, so through preventing it, immune systems will be stronger.

The dictionary defines accountability as : the quality or state of being accountableespecially: an obligation or willingness to accept responsibility or to account for one’s actions (Merriam-webster.com, 2019)

Nurses have a responsibility to their patients to be accountable. One of the tools to help and guide nurses are the Nurses Code of Professional Conduct. The nurses Code of Professional conduct or Code of ethics,” is a compilation of ethical rules designed to guide the nurse in making the correct decision in difficult patient-care situations. The Code contains nine provisions, each dealing with a different area of patient care. Nurses must adhere to the Code throughout all interactions with patients, colleagues and other health-care providers.” (Work.chron.com, 2019)

Under Provision 4 int the Code of Ethics it states, “The nurse has authority, accountability, and responsibility for nursing practice; makes decisions; and takes actions consistent with the obligation to promote health and to provide optimal care.” (ANA, 2019)

In caring for a patient, a nurse can demonstrate professional accountability by only rendering care he/she is competent to do. An example of this would be a nurse with no tele experience or training hired to work on a med surge floor being floated or pulled to work on telemetry unit. If that nurse accepted the assignment without having passed a tele competency he/she would be working outside their scope of practice and would be breaking the code of conduct. they would not be showing accountability in their actions to the patients or their practice and could put the patient and nursing license in jeopardy. They could also be charged with misconduct. A Nurse who refused said assignment and bumped up the issue to a supervisor would be holding themselves accountable for the safety of the patients and the integrity of their license.

The reason we use clinical expertise along with EBP to make decisions is because we as health care providers cannot know everything. Even if we have years of clinical experience and expertise. In that sense it is important to bring clinical expertise together with EBP to bring forward the best care for our patients and by incorporating both we are being fully accountable to our practice and for our patients.

Outline the concept of professional accountability as it pertains to nursing. Provide examples of how a nurse demonstrates professional accountability in clinical expertise, the nursing process, and evidence-based practice.

Professional nursing accountability is when a nurse takes responsibility for their   actions, judgments, and omissions when caring for patients .      Professional accountability  is also having the responsibility of remaining competent and up to date in all nursing and health care practices   (Green, S 2018).   Nurses should also have  integrity  that is always maintained ,  through their  actions, practices, and attitudes which  they  display to their patients  and interdisciplinary team.

A nurse can demonstrate professional accountability in clinical expertise  for example by  ensuring that  they are always  properly giving blood products to a patient.  Using their clinical expertise, they know they need to have an order, consent, two licensed professionals to verify the blood products and the patient before administration, and to stay with the patient for the first fifteen minutes of the transfusion. Doing this is  advocating for  the patient to get the correct products safely and  requires a  certain professional attendanc e.

In the nursing process, a nurse can demonstrate professional accountability  by using safe practices.  S uch  as  educating the patient of signs and symptoms of a transfusion reaction, to notify staff if they have any questions and concerns during the transfusion  immediately and  reporting  and intervening if there is a reaction.

P rofessional accountability can be shown in the evidence-based practices (EBP).  A nurse being educated and up to date on educations such as EBP and using it to help improve action and promote better patient outcomes.

Accountability is defined as the concept of being responsible for one’s actions. This means being accountable for our work practice, environment and patient safety. The American Nursing Association’s (ANA) code of Ethics defines professional accountability as being “answerable to oneself and others for one’s actions.” In 2016 the Gallup poll rated Nursing as one of the most trusted profession in the United States for 15 consecutive years. It is of vital importance that we maintain this level of trust with each patient encounter. As nurses our patient’s healthcare depend on the decisions and actions we make. Sometimes our actions make a difference between life and death. As nurses we face daily time challenges as well as work demands. We often have to deal with staff shortages and the acuity of our patients. Even with all these road blocks, it is our responsibility to maintain certain ethical standards in order to obtain better outcomes. The ANA code of Ethics for Nurses was developed as a guide for carrying out nursing responsibilities in a manner consistent with quality in nursing care and the ethical obligations of the profession. It was created to help nurses “navigate ethical and value conflicts, and life and death decisions, many of which are common to everyday practice” (Ethics and Human Rights, 2018). It states that, “the nurse is responsible and accountable for nursing practice and determines the appropriate delegation tasks consistent with the nurse’s obligation to provide optimum patent care” (Code of Ethics for Nurses with Interpretive Statements, 2010)

Discuss the importance of advocacy as it pertains to patient care. What is the nurse’s role in patient advocacy? Describe a situation in which you were involved with patient advocacy. Explain what the advocacy accomplished for the patient, and what the repercussions would have been if the patient would not have had an advocate.

When nurses advocate for patients, stand up for patients’ rights, or work to meet their need for care, that nurse is actively assuming the role of patient advocate. The nurse is using knowledge to care for the patient and to coordinate care for the patient (Dynamics in Nursing 2018). This is important because the nurse communicates what he/she believes to be the best methods of care for the patient based off the data in all forms.  This is then communicated by the nurse across all levels of care of the patient to ensure the best possible outcomes for the patient.

A situation I recall in my time of patient care in the Emergency Room to where I advocated for a patient was realizing that based off the data I collected when caring for a patient I suggested to the doctor to perform a CT scan that wasn’t previously done and hadn’t been ordered yet.  The patient was feeling a bit short of breath and was on oxygen and on the cardiac monitor with a slightly elevated heart rate.  We had already performed a chest x-ray and labs right away and cardiac monitoring.  Because I was the nurse caring for the patient and monitoring the results in all forms, my concern was that the patient had a Pulmonary Embolism and because the doctor hadn’t actually made it bedside to see the patient yet, my advance knowledge and awareness prompted me to relay to the doctor what I believed we needed to do.  Sure enough the patient did in fact have a Pulmonary Embolism and we were able to proceed faster with care and get the patient admitted and to the floor faster as well.  Had I not advocated for the patient the outcomes could have went in many directions but mostly would have delayed prompt patient care and potential adverse outcomes.

Advocacy is as essential to the role of a nurse as any other aspect of nursing care. Interacting with patients more than any other healthcare provider, nurses are in the perfect position to serve as patient advocates (“Importance of Patient Advocacy in Nursing | UTA Online,” 2016) We as nurses are the ones who know the patient the best. We are at the bedside the majority of the time with these patients and families. We get to know them the best, so we as nurses have the responsibility to advocate for them.
I had a patient once who was pod #1 s/p laparoscopic gastrectomy sleeve. The patient seemed to be doing very well until he became very SOB, was complaining of chest pain, shoulder pain, he was tachycardic. This particular surgeon is very particluar with his patients and does not like to get anyone else involved. We strictly call him to his cell phone with any questions or concerns. However, in this case he did not reply promply to my call, so i had to call a RRT. Patient had to go to the ICU, and later found out that a patient had a leak. Had I waited for the surgeon to call me back or not called an RRT, the patient wouldnt have been treated promptly.
Advocating for our patients is crutial for them. We care for them in one of there most vulnerables times in their lives. They trust the heathcare professionals to do what is best for them.

Advocacy for patients is important to ensure that they are receiving the proper care (Helbig, 2018), and when they can’t voice for themselves, we, as nurses, should stand up for them and make sure the proper care and needs of our patients are given if at any time it is denied or is in need of investigation or proper resources. Communication and collaboration are key in the work environment in order to provide patient-centered care (Helbig, 2018).

I had an experience where the patient was seen and the doctor determined she had not been taking her medication as prescribed and had not attended PT.  The doctor came out of the room and instructed me to document non-compliance and to discontinue her treatment at the office.  I asked him what the problem was and asked if I could talk to the patient and see what may be the cause.  He gave me permission and I was able to determine the issues within twenty minutes.  As he was seeing another patient, I waited until he was done and asked to speak to him.  I explained to him the issue was the patient was in the hole with medication through Medicare and could not afford it, and was also having problems with transportation.  He gave instruction to document the result and find the resources and samples of medications the patient needed and we would supply them until she was out of the hole. Had I not intervened or advocated for the patient, she would have been labeled non-compliant as well as had to go through the process of looking for another doctor.
