We need to gather data on the opioid crisis in Anytown. One way to collect data is by using surveys.

For this assignment:

1. Create a survey to assess opioid use in Anytown. You can use an online survey creator, like SurveyMonkey, or write the survey in Word.
2. Determine and explain the type of survey you would use. Include your rationale for the following:
   • The number of questions in the survey
   • Describe the questions used and reasoning for the selected content
   • Distribution and collection of the survey
   • Selection of the participants for the survey
3. Assess and describe how you will ensure validity and reliability of the survey.

For this assignment, include either the actual survey or a link to the online survey. Your response to the questions above should be 1–2 pages, excluding title and references pages. Use current APA formatting to style your response and to cite your sources. Integrate a minimum of two scholarly sources (including your text) into the paragraphs of your paper. Use internal citations pointing to evidence in the literature and supporting your ideas. You will need to include a reference page.

At least 80% of your paper must be original content/writing.

No more than 20% of your content/information may come from references.

Use at least three references from outside the course material, one reference must be from EBSCOhost. Text book, lectures, and other materials in the course may be used, but are not counted toward the three reference requirement.

Cite all reference material (data, dates, graphs, quotes, paraphrased words, values, etc.) in the paper and list on a reference page in APA style.
