Choose one legislator on the state or federal level who is also a nurse, and discuss the importance of their role as advocate for improving health care delivery. What specific bill(s) have they sponsored or supported that has/have influenced health care?

Congressman Diana Black, representing Tennessee’s 6th Congressional district. She practiced as a nurse for 40 years.

It is important to have a nurse represented in the house, where laws and policies are passed, because an insider who knows the needs of nurses and healthcare system would be passionate to vote and advocate for the healthcare. There are millions of topics been discussed in the house on state and federal levels, but healthcare issues are very important, because health is universal, everyone use the healthcare system. It is very crucial for a nurse to be part of lawmaking process, not only to speak out for the nurses and make a positive impact on nursing profession, but also as a role model to other nurses, aspiring nurses to go for legislative positions where they can be part of change-making process in the society; which could be beneficial to the patients.

Black supported the passing of Increased   Telehealth Access to Medicare Act, this bill increases access to telehealth services for Medicare Advantage users, in this age of technology, health care access is made easier and affordable using technology. Telehealth focuses on the use of innovative technology for the convenience of patients and caregivers to enhance the quality of care while saving money for both Medicare program and the patients (Black, 2017). This service enhances access to healthcare and reduces the number of in-patient visit to healthcare providers, covered by traditional Medicare plans under the current law (Black, 2017)

Current statistics show that at present, there are 1,048 Associate Degree Nursing (ADN) colleges and 696 Baccalaureate Degree Nursing (BSN) institutions in the United States (Cherry & Jacob, 2017). Recent studies show that a correlation exists between patient safety outcomes and whether a nurse has an ADN or BSN. Linda Aiken, PhD, RN, who is associated with the Robert Wood Johnson Foundation (RWFJ), believes that hospitals who employ more BSN-prepared nurses have better patient outcomes and lower mortality rates (RWJF, 2014).

          In 2003, Aikens reported on studies done in Pennsylvania hospitals showing a 5% decrease in patient deaths at hospitals that staffed a 10% increase in BSN-prepared nurses (RWJF, 2014). Challengers of Aikens note that these studies raise further questions (Sentinel Watch, 2014). For example, what were the actual causes of death of these patients, and were they related to the action of an ADN nurse? Could there be other factors or causal relationships that affected patient care in these circumstances?

          In defense of Aikens, the Sentinel (2014) states that no one is calling ADN nurses “bad nurses”. In fact, errors, infections, and adverse events are often due to process and system problems. Aikens maintains that by promoting evidence-based practice and leadership, the BSN curriculum will better prepare nurses to correct problems and implement solutions.

          I agree that it is advantageous for nurses to expand their knowledge by obtaining a BSN degree, but I do not feel that deaths and adverse outcomes are the result if ADNs care, or lack of. Many other factors need to be considered. For example, poor staffing, long shifts, poor communication, fatigue, documentation errors, etc. affect everyone no matter what type of degree they hold. I have been at the hospital where I am currently employed for almost 20 years and have seen no evidence that the care given by ADNs is substandard to the care given by BSN-prepared nurses.

Comment Of Health Belief Model

The Health Belief Model (HBM) is a psychological model developed in the 1950’s to try to explain and predict a patient’s response to health issues (Edelman, 2013). This model was developed to explain why some people fail to adopt preventative health measures even when the treatment is free. This model suggests that a message will have the maximum effect and behavior change if targets perceived barriers, benefits, self-efficacy, and threat (Jones et al., 2016).

The following components are edited for the text book Health Care Promotion Throughout the Life Span.

  • Individual perceptions or readiness for change
  • The value of health to the individual compared with other aspects of living
  • Perceived susceptibility to a health problem, disease, or complications
  • Perceived seriousness of the disease level threatening the achievement of certain goals or aims
  • Risk factors to a disease attributed to heredity, race or culture, medical history, or other causes
  • Perceived benefits of health action
  • Perceived barriers to promotion action

Lifestyle changes can be the toughest to integrate into daily activities because they require the patient to consciously practice new behaviors. One way for the nurse to help make these changes permanent is to evaluate the readiness to change. The nurse should use questions to elicit responses that indicate the patient is ready to change behavior. Without a willing participant all other information may not lead to the desired changes.

The framework of the HBM allows the nurse to choose the appropriate intervention that is achievable and permanent. Each component guides the nurse by identifying barriers and identifying ways for the patient to adhere to the new behaviors

Nurses that have gone on to become legislators “approach the job from the perspective of a nurse, nurse legislators can represent the interests of their colleagues in a way that no one else can” (Larson, 2016, para. 5).

Lois Capps, RN, served as a U.S. Representative for California’s 24th congressional district from 1998 to 2017. She has been a passionate advocate for nurses and has raised awareness about the ways nurses improve health care for all (McCutcheon, 2015). She has made many contributions on behalf of nurses and to the improvements in health care. Her retirement from Congress will surely produce a void in her absence. Through several sessions of congress, Capps was very active in and sponsored a bill that was backed by the ANA that would “improve the health, safety, and outcomes for patients by ensuring sufficient nurse staffing in hospitals” (McCutcheon, 2015, para. 3). Capps sponsored The Registered Nurse Safe Staffing Act requiring hospitals to establish a panel to develop unit-by-unit flexible nurse staffing plans based on several determining factors and this panel was to be made up with a majority of RNs. She also was instrumental in the enactment of the Nurse Reinvestment Act in 2002 to fund nursing education and development of workforce (McCutcheon, 2015).  

Above all, Lois Capps, performed her work in Congress, with a smile and much kindness, which is not a common thing seen in a divided Congress as we have now.

After discussion with your mentor, name one financial aspect, one quality aspect, and one clinical aspect that need to be taken into account in developing the evidence-based practice project. Explain how your proposal will, directly and indirectly, impact each of the aspects.

Speaking with the mentor, the financial aspect of the project would be hours spent trying to get nurses educated on the proposed guidelines for late preterm infants. Hours will be needed as well to get the basic checklist implemented into the computer in this particular area, which requires going to different committees and getting approvals needed. This takes more than one nurse or person to get this done. This requires more hours of pay, and that would be the financial aspect of the proposed project.

Now, the clinical aspect that has to be taken into account with developing the project would be how the nurses will accept the project. As lifelong learners, staff nurses continually encounter new knowledge of relevance to their patients. The conscientious nurse remains alert to clinical problems and encourages investigation into ways to improve patient outcomes(Di Lenoardi, 2014). The nurses need to be given research and data that proves this could help readmission of late preterm infants. If given this data, they would be involved in the process of implementing as well,

In 2001, the Institute of Medicine (IOM) recommended evidenced based practice to improve healthcare outcomes(Stephens, 2013). This project is to improve the quality outcome of late preterm infants and prevent their readmission to hospital.

All the aspects brought together will make a good impact on the project the author believes with the research and data given to the nurses, implementation and then follow up of results will show this is a great solution to help alleviate many readmissions nationwide as well. If this hospital shows that the guidelines are helping there, then they can show other colleagues in the nation that this helps and also will cut costs by preventing readmissions.

The healthcare organizations are moving to have more bachelor prepared nurses so; they can help make patient care decisions. The Institute of Medicine (IOM) recommends nurses to have a BSN or higher to help with the overall changing healthcare system. By 2020, they want 80% of nurses to have a BSN or higher. Why? Nurses need to have an expanded more knowledge of different areas of nursing and evidence-based practice. As hospital care has become more complex, nurses must make critical decisions associated with care for sicker, frailer patients and work with sophisticated, life-saving technology (Garner, 2011). Evidence-based practice is vital to provide the most effective patient care to improve patient outcomes because patients are becoming more complex.

Two ways that the author will continue to integrate EBP into their practice is continued education on the subject at hand to keep up to date on changes in patient care to help with improvement in patient care. Also, another way is to Engage colleagues and educate them on new EBP practices, studies, or interventions that may benefit their work environment or patients (Chrisman, Jordan, Davis, & Williams, 2014)

Obstacles to the plan for the author would be nurses resisting change and management not helping with the implementation of the plan. Education to the nurses will be the first of why this is helpful for our patient’s outcomes versus the old way of doing things. Showing articles and statistics will help bring the point across. Getting management on board to help with implementation as well. Management will be made aware that providing the evidence-based care will improve patient outcomes and decrease healthcare costs.

Why is understanding the health care system at the local level important to consider when planning an EBP implementation? Conduct research and solicit anecdotal evidence from your course mentor that you will take into consideration for your own change project.

 

It is very important to understand the health care system at the local level because you need to know the surroundings and the area. The hospital I go to with my mentor is in the country and the nurse to patient ratio is sometimes higher than the bigger hospitals and the staffing isn’t always great because of the location. So, when you are trying to help cut back on the number of falls it is important to understand the area because you will have to know that staffing will always be an issue because of the area of the hospital and the size of the hospital. Falls are a huge problem in every hospital and when someone tries to make a change it will be incredibly hard for them. But when the hospital is a small hospital it is going to be really hard because of staffing issues and lack of nurses and funds. I read an article that stated the amount of patients to falls is slightly higher in smaller more rural areas 6-1,000 at the big hospitals and 6.3-1,000 at the smaller hospital. (Linda,2017) So, knowing the local health care is very important to understand so one can make sure they put that in the plan.

Stakeholder support is necessary for a successful change proposal project implementation. Consider your internal stakeholders, such as the facility, unit or health care setting where the change process is situated, and your external stakeholders, like an individual or group outside the health care setting. Why is their support necessary to the success of your project, and how you will go about securing that support?

 

            Getting support is a very important part of making any changes but when it comes to stakeholders their support is so important. Without their support the necessary funding and the OK to get it done won’t happen. If we get their support we will have the funds needed and if we get can them on our side the project is a lot more likely to succeed. Like any business the stakeholders want to make money. So, the key to getting them on your side is telling them how they can profit from this change. Let’s take falls for an example. If you show them that the change can cut down on falls wouldn’t be enough. The person purposing the change would have to explain how cutting down on falls would save the hospital a lot of money which in turn would make them more money. If you can show them the statistics and show them how much money the hospital could save from cutting down on falls. Once they see the profits they can make then they will be more likely to except the change and vote for it to happen.

Compare and contrast two change theories, and determine which theory makes the most sense for implementing your specific EBP project. Why? Has your mentor used either theory, and to what result?

 

The first change theory is Lewin’s Change Theory. This theory is very widely used in nursing. This theory has three stages the unfreezing stage, moving stage, and refreezing stage. The theory has driving and resistant forces and for the theory to be successful the driving forces have to overcome the resistant forces. The other change theory is Rogers’ Change Theory. This theory has 5 stages and they are awareness, interest, evaluation, implementation and adoption.(Oguejiofo,2017) It is successful when nurses who ignored the proposed change earlier adopt it because of what they hear from nurses who adopted it initially. Both of these theory’s are widely used in nursing and both require nurses that want the change or who are willing to make the change. My mentor has used the Lewin’s change theory recently. The hospital already has hourly rounding but she just introduced new paperwork that has to be signed every hour. The unfreezing period she just explained how the new way will be better and she showed the nurses how it will be easier because the techs can also sign the sheet. The moving stage she let the nurses tell her how they feel about the whole situation and letting them express what they think will work. The final stage is refreezing and during this stage she went around for the first week making sure the nurses get this in their daily habit. I believe this theory makes more sense to my EBP because there are a lot of nurses that will be the driving force to make this happen and less people being the resistant force. So, it will be more likely to succeed.
