Teamwork.
A teamwork approach includes team members from different disciplines working collaboratively, for a common purpose, to set goals; make decisions and share resources and responsibilities. During the team work, a group of clinicians from different disciplines, come together with the patient, perform assessment, make a diagnosis, intervene, and sett goal. The patient and their family are also involved in any discussions about the patient condition, prognosis and care plan (O’Reilly, Lee, O’Sullivan, Cullen, Kennedy, & MacFarlane, 2017).
In interdisciplinary teamwork, communication across disciplines and with the patient and their family is vital to setting the goals that most exactly reflect the patient’s health care needs.
In my experience, I had an opportunity to work with nursing assistant to give bath for a client and a physiotherapist to ambulate a patient who had general weakness during my clinical. As student nurse, I was just following both time and they were coordinating the care we gave to the clients. During both situations, the patient’s out come met because the plan was to give bath for the first patient and ambulating the second one to improve muscle strength. I personally liked both experience because it was good educational event to interact with patients.

There are a multitude of issues at both the state and federal level that need to be changed. One of the most significant changes that needs to be made is patient staffing. In particular, we need safe staff and patient ratios, and we need laws to regulate them. These ratios help keep both nurses and patients safe. Currently in place is something called the “42 Code of Federal Regulations” (Nurse Staffing: Health care advocacy: American Nurses Association, n.d.). This law contains broad language about the actual numbers of nurses needed per patient, so it doesn’t help nurses or patients. In fact, it’s difficult even to enforce it because it isn’t specific enough!
There are only 14 states that have some kind of nurse staffing law (Nurse Staffing: Health care advocacy: American Nurses Association, n.d.)! Every state should have clear, concise laws to ensure patient and nurse safety. This is important to me because it affects both my future patients and me. In order to “do no harm,” we must be able to practice nursing safely, effectively, and to the best of our ability. If we are trying to care for way more patients than is safe, we are not doing good for the patients, or ourselves. Enacting safe staffing laws allows nurses to practice safely, and in turn, give our patients the care they deserve.

A form of interdisciplinary collaborative care that I have seen is when I am at work. The team comes together as one to include lab techs, Oncologist, RN, schedulers and our radiology team, research team. Every once in a while I am fortunate enough to shadow the Oncologist which is a great opportunity for me as a nursing student to see the entire process. In this case the doctor had recommended that this patient have a CT scan done today in the clinic and also wanted the patient to get scheduled for a thoracentesis. He had asked me to put in an order for additional blood work since we already had labs done on her. I was able to put in the order and inform our lab techs that we added it on and needed to use the “just in case” tube. Once the doctor was done with this patient he had sent in his RN to go over what the chemotherapy drugs and how the remainder of the day would go for her. I walked her over to the scheduler to get the patient scheduled for a CT and the thoracentesis. Once our CT tech was available I walked the patient and her family over there before she headed over for the thoracentesis.
The patient was able to move from one area in the clinic to another with a very smooth transition and her needs were met. Our schedulers will sit with the patient and schedule the upcoming appointments and any outside appointments that the Oncologist has ordered making it easier (typically) on the patient. As a new nurse I don’t think I would have done anything differently as the patient didn’t have to travel all around and the RN even provided her with a new patient portfolio with all the resources she will need at this time. adding to my original post. As I had stated in my scenario is that the patient was able to move throughout the clinic smoothly with all of the team members she was working with and we too were able to communicate efficiently. In one of our reading assignments for the week, Patricia Yoder-Wise (p321) states that “teams are critical for patient safety because they encourage frequent and ongoing communication and create a safety net.” I read that and I think about when a patient is at our clinic or just in general and how crucial interdisciplinary collaborative care is. If one member misses something when meeting with the patient there are others involved in the care vs just one nurse who could be very distracted during that interaction.

/in Uncategorized /by developerCritical thinking and clinical judgement scenario.
You have been assigned 4 patients on an Intermediate Medical Care Unit. Two of the patients are post myocardial infarctions at various stages of their infarctions with multiple types of arrhythmias, the third patient is having drastic blood sugar fluctuations 218 down to 50 within minutes and its rebounds back up with changes in mentation and the fourth is reported to be having frequent TIA’s. One of the MI patients is having some dizziness and your TIA patient is presenting signs of impending stroke.
How would you prioritize your assessments and activities? How would you describe your critical thinking process and how do you organize and prioritize implementation of care?
List (using bullet points) IN ORDER from 1st to last how you would approach this situation. Who would you see first?
Then, in 4-5 sentences, I want you to justify your decision regarding why your chose your first patient.

/in Uncategorized /by developerHealth can indeed differ between individuals depending on some factors, personally being healthy is important to me, and I strive every day to make choices that will benefit my wellbeing. When I initially started nursing school I had a tough time managing my stress, maintaining a healthy diet due to stress eating, as well as lacking physical activity due to poor time management. Being healthy is so important to me because for one I am going to be a nurse which means I am going to be educating patients on a daily basis about improving their health and well being. Also, what motivates me is reminding myself of what it felt like to not pay attention to my health and how horrible I felt every day when I did not care. Making your health a priority, in my opinion, will help you motivate others that are struggling with their health to do better, which can be done through education or even sharing a personal experience.

/in Uncategorized /by developerThere are so many aspects of being “healthy”. It can be challenging to give attention to every area of health that needs care. For example some people might be in good physical health but be lacking in the area of mental health, which consequently can have negative impacts on your physical wellbeing. I have always been pretty healthy in terms of getting enough exercise and eating healthy. Nursing school has definitely taken a tole on my health. My body does not handle stress well at all and I can feel the negative impact the stress of school, work and being a single mom has taken on me. I was able to manage much better in the beginning, I was going to yoga a lot and eating really healthy. The last 6 months I have been going to the gym less and less and the stress has robbed me of my appetite which has caused me to lose weight and lose energy. I have been working to get a handle on my mental health so that I can push through the rest of the program in a health way and not be so stressed out. I have been doing research into burnout and how to combat it. I have started doing more meditation to try and clear my head and taking my life one day at a time in order to not get overwhelmed. It is so important to focus on every aspect of health so that we can be well enough to take care of our families and our patients.

/in Uncategorized /by developerI can’t decide if I agree or disagree with this statement by the American Nursing Association. Educating nurses is so very important and learning more information is going to help expand the nurse’s knowledge. But many nurses don’t pay attention to new education. Usually there are courses that you have to complete in your job but many times nurses just click through this to get done with it and not pay attention to any of it. Nurses need to be taught in different ways to learn. If it is the nurse who is not wanting to be properly educated then I do think it is the nurse’s fault. Yes, the nurse’s level of education matters but so does experience.
