Changes in Eyes Due to Certain Medical Conditions and Ear Changes Caused by Aging Process
The eyes are our windows to the word. Different medical conditions can cause changes to the eyes. In this discussion I will write about Cataracts, Glaucoma, Diabetes Mellitus, and Chronic Hypertension.
Cataracts is when protein clumps up and clouds the lens, this reduces the light that reaches the retina. The easiest symptom to see objectively is clouded eyes. The patient complains of sensitivity to light and glares. Difficulty with night vision and halos around lights is also a complaint. Blurred vision is reported due to seeing thru clouded eyes.
Glaucoma causes damage to the optic nerve and gets worse over time. It can cause vision loss and blindness overtime. Early treatment can protect from vision loss. Symptoms can be hazy or blurred vision, rainbow colored circles around bright lights, severe eye and head pain, nausea and vomiting with eye pain and sudden eye loss.
Diabetes Mellitus can lead to Diabetic Retinopathy which affects vessels in the light sensitive tissue called the retina. This is the leading cause of blindness among the working age. Symptoms can be wavy vision, color changes, spots or floaters, trouble with night vision. Increase in sugar level causes the lens of the eye to swell which affects vision. Decrease in sugar level causes blurriness and double vision.
Chronic Hypertension can cause damage to the blood vessels in the retina, the area at the back of the eye where images focus, this is known as hypertensive retinopathy. This can lead to blindness. Symptoms can be fluid oozing from blood vessels, double vision, blurred vision, swelling of the optic nerve or macula, and spots on retina known as cotton wool spots.
These conditions can be managed and prevented in some cases if not most with proper attention and education.
There are changes to the ears due to the aging process. Older adults should be asked about hearing loss, vertigo, ringing in the ears ( tinnitus), and even dizziness.
Otoslerosis is a possibility especially in older adults. Otoslerosis happens due to slow fusion of ossicles in the middle ear, this obstructs sound waves. This can cause difficulty with distinguishing sounds when it is noisy. Mumbled speech can also be a symptom.
Presbycusis happens over time and it is a natural sensor neural loss. The hair cells in the cochlea become less flexible. Hearing deficiency is a symptom.
During Review of Systems these conditions should be asked about. Sometimes patients are not even aware of these changes and need to be educated so they can seek help.

Another practice of nurses should be bridging a gap between the patient’s care and education. It is important to educate the patient to their health or better understanding and improve compliance to treatments. This is an important practice for nurses to employ with patients, especially of vulnerable populations because patients are more likely to manage their conditions and use preventative services. Autonomy is something that should be respected. When a person is going to get any invasive medical procedure or treatment done it is the responsibility of the healthcare provider to explain and make sure the patient is aware of the risks of the procedure. The healthcare provider should answer any questions that patient may have. Once there has been understanding of the procedure including the pros and cons, the patient is to sign an informed consent. When the patient signs the consent they are giving authority to accept medical treatment (Rock & Hoebeke, 2014).
There may be barriers that can interfere with the rights of a patient when it comes to informed consents. If a patient does not speak English it can interfere with understanding of the procedures and they may not fully comprehend why they are signing a consent (Chesnay & Anderson, 2016). When situations like these arise, hospitals can provide a professional translator to assist. There can also be cultural barriers where patient’s culture or beliefs does not allow them to go forth with a procedure that may improve their quality of life. However, the healthcare provide should respect that patient’s culture and beliefs. It is crucial that consents are understood and signed by the patient before any treatment is given or else the healthcare provider can be charged with assault. Physicians need to explain the treatments and nurses should be able to confirm the patient understands. It is very important to protect the right of the patient which is the right of self-determination. That is why the patient should have full understanding of what treatments consists of, the pros and cons, and the risks of not doing the treatment. Once that is done they need to sign a consent to confirm that they understood and made a conscious decision to accept treatment.

Companies’ goals are to try to make money to provide more for the customers and pay employees. In the healthcare system hospitals, doctors offices, and pharmacies all have to make money to pay employees and keep providing customers/ patient with excellent service. Some of these institutions are for-profit and some are nonprofit. These institutions still need to have some sort of cash flow in order to work. The biggest economic impact on these healthcare institutions are insurance companies. Insurance companies are the driving force of how these institutions get paid for the most part. However, with insurances there is a huge disparity on who has insurance and who can afford insurance. That is why universal healthcare is at the forefront of all economic conversations specifically here in the United States of America. Universal healthcare can allow all citizens of the US to be seen by a doctor and thus maintaining or bettering their health. Nevertheless, universal healthcare comes at a great economic cost. If universal healthcare were to be provided then the United States economy would be affected. That is why it is such hot and heavy economic and social issue. Most healthcare workers take oath to providing some sort of betterment two societies health overall. It is important that healthcare be delivered to everyone no matter the circumstances so that the oath can be upheld. With this in mind economics cannot be separated. Currently almost half of healthcare spending is used to just treat 5% of the population in the United States (Boudreau, 2017). In addition, healthcare costs are rising almost 2% every year (Boudreau, 2017). All things considered, it is important to start planned out the future of healthcare and healthcare economics. Universal Health Care is definitely an option on the table that can allow the US population and its citizens to receive healthcare no matter what. A plan however needs to be in place so that economic disaster does not overwhelmed the country. Additionally to the economics of universal healthcare, the delivery of healthcare to all the newfound people is another hurdle (Blais & Hayes, 2015). Hospitals and other facilities need to be prepared to deal with the vast influx of people seeking healthcare treatment once universal healthcare is adapted. By hundreds of thousands of people gaining healthcare and them coming into the system it can bombard and potentially crippled the healthcare system. Nursing Theory Necessity
In the nursing field, nursing theories have proven to help improve the nurse and patient’s environment. They have helped in improving our patients lives by promoting infection control and advocating for their health and safety. It is important for nurses to be aware that theory and practice are not to be separated, but in fact combined. The purpose of the theory is to be put in practice. Just like our textbook demonstrates via the circular graph, stating theory, research and practice individually help to validate, modify or invalidate the theory in question (George, 2011). For a theory to be acknowledged, there must be research that proves the theory correct. Lastly, the theory must be placed into practice. Theories are important in the field of nursing, many institutions base their care models on specific theorists. According to Power (2016), the theory is important in the nursing field, but one must not forget about compassion. Compassion meaning not forgetting that in room 203 there is a human being with feelings and needs and their care should involve them. With the shift now from Obama, a democratic president to now President Trump, a republican president and administration, we can understand how legislators may vote differently in order to win or gain the voting support of Americans who voted a republican president into office. According to Brodie, Dean & Cho (2011), the opinions of Americans regarding the ACA was dependent on the geographical location and the political influence in that region which supports the statement that an elected legislator from an area is “paid” in votes. It is to the benefit of the legislator to vote according to the likelihood that their opinion on the issue is going to get them reelected.
The same theory stands in place for other political issues such as Medicaid and Medicare issues. Legislators will support or push through a law that they feel will gain the favor of the majority of their voting area in order to secure their career. This supports the idea that nurses have a large impact on health care policies in the United States and the obligation to voice our opinions. Many legislators though intelligent, may just not have the information or know what medical professionals know about issues that are affecting people (Laureate Education, 2018 b). There are 2.82 million nurses in the United States (Registered nurses number U.S. 2001-2016, 2018) and when we learn how to assess the needs of our communities, create a policy and learn how to present and communicate these policies to legislators, we can make a difference for our community and the health of our country.

There are many national and local’s non-governmental organizations (NGOs) that promotes health and wellness to all people. The American Heart Association (AHA) is one of the oldest and more actively involved in people’s health and diseases prevention. According to the American Heart Association (n.d.), their goal is to “see a world free of cardiovascular diseases and stroke.” They fund and conduct many researches, studies, events, and education materials to help fighting and preventing heart disease among all Americans and around the world. AHA promotes many ways to decrease heart diseases, which is the number one killer among people in the world. Regarding the promotion of wellness, we can certainly see how they encourage healthy eating habits, including recipes, types of foods that are healthy for the heart, cooking skills; healthy lifestyle, as stress management, quitting smoking, stress management, healthy sleep patterns, fitness, walking, exercising, and losing weight (AHA, n.d.). The AHA, at the same time, publishes studies and documents on many of the heart disease that most affect the population. They write and publish about aortic aneurysm, arrhythmia, atrial fibrillation, cardiac arrest, cardiomyopathy, cholesterol, congenital heart defects, diabetes, heart attack, heart failure, murmurs, valve problems, HTN, strokes, smoking cessation, and many more (AHA, n.d.). When selecting human subjects for research, it is important to consider cultural, ethical and even social implications of the subjects. Researchers must evaluate the purpose for the research of specific individuals and determine if they are underserved population or a vulnerable population. Things like these are important to establish because implications can be developed from such. A major ethical issue that should be observed is informed consent. As described by Polit; informed consent is a process in the ethical conduct of a study that involves obtaining people’s voluntary participation in a study, after informing them of possible risks and benefits. Human subjects that are of vulnerable populations and are not able to comprehend the information of the research are implicated. If the human subject is unsure of the potential risks and benefits other measures must and should be used to create understanding for the subjects. It is ethically wrong and illegal not obtain proper informed consent. It is not safe for the subject and unfair to them as well.
Cultural barriers can also come into play and should also be observed when selecting human subjects. Providing accommodations for the subjects should be made and applied in order for complete transparency of all parties.
