INSTRUCTIONS:

1. Go to http://www.healthypeople.gov/2020/default.aspx and review the Healthy People 2020 initiative. Pay close attention to the objectives, goals and the indicators.

2. Using your city or county public health department identify an epidemiological and surveillance data related to population based communicable illnesses described in table 5-1 page 72 of our class text book. Besides the illnesses describe you can also choose influenza or any other illnesses that are currently affecting your community.

3. Once you have chosen your topics you must send me an e-mail telling me what illnesses  you choose. 

4. Choose 3 population based communicable illness and prepare an analysis discussing how this 3-communicable illness affect your community and how they were identified and a plan of action to contained them base on healthy people 2020.

5. Presentation must be in APA format word document, Arial 12 font attach to the forum/thread in the assignment tab and turnitin with a minimum of 1000 words (excluding first and references page) with a minimum of 3 evidence based references (excluding the class textbook) no older than 5 years are required. The assignment must include the zip code of the city or county chosen. Follow the APA example paper that was sent to you at the beginning of the course for guidance. Make sure references are use according to APA guidelines and electronic references must be from reliable sources such as CDC. FAMILY HEALTH ASSESSMENT

After you have read chapter 29 and 30 choose a family in your community and conduct a family health assessment using the following questions below.

1. Family composition. 

Type of family, age, gender and racial/ethnic composition of the family.

3. Do family members have any existing physical or psychological conditions that are affecting family function?

6. Do individual family member’s developmental states create stress in the family?

7. What developmental stage is the family in? How well has the family achieve the task of this and previous developmental stages? 

8. Any family history of genetic predisposition to disease?

9. Immunization status of the family?

10. Any child or adolescent experiencing problems

11. Hospital admission of any family member and how it is handle by the other members?

12. What are the typical modes of family communication? It is affective? Why?

13. How are decision make in the family?

14. Is there evidence of violence within the family? What forms of discipline are use?

15. How well the family deals with crisis?

16. What cultural and religious factors influence the family health and social status?

17. What are the family goals?

18. Identify any external or internal sources of support that are available?

19. Is there evidence of role conflict? Role overload?

20. Does the family have an emergency plan to deal with family crisis, disasters?

Identify 3 nursing diagnosis and develop a short plan of care using the nursing process.

After reading the following scenario decide whether Mr. Howe made the right decision in regard to his ailing wife. Describe the factors you considerd in in reaching your decision?

An elderly Florida man was in jail after he said he fatally shot his ailing wife because her medications were no longer affordable and she was in pain. William Howe, 86, was being held without bond after telling authorities that he had shot his wife in the head as she slept that morning in their home according to an affidavit. Mr. Howe called the 911 emergency line several hours after the fatal shooting. Howe told authorities that after the shooting he went to the kitchen for a cup of coffee, and called his daughters to tell them what he had done.

“I want to apologize I didn’t call earlier. I wanted to tell my kids what happened first,” Howe told authorities. Howe told authorities that he had been thinking of killing his wife, 84, for several days because she was in pain, the affidavit said. Howe said she had told him that she wanted to die in the past, and she had asked him to kill her, according to the affidavit.

A dispatcher at the County Sheriff’s Office said Howe remained in jail on a charge of first-degree, premeditated murder after an initial court appearance.

A local broadcaster reported that the couple had filed for bankruptcy in 2016 and that Carolyn Howe had been suffering from severe arthritis and other medical issues for at least 15 years. The AARP, a non-profit advocacy organization for people 50 years and older, said in a report last November that increasing costs for certain specialty prescription drugs have put them out of reach for many people and that Medicare does not necessarily make those drugs affordable. Authorities did not say what specific drugs Mrs. Howe was taking nor what kind of insurance the couple may have had. A search of records at the Florida Department of Law Enforcement does not show a prior criminal history for Mr. Howe.
