Asthma is a chronic inflammatory pulmonary disease that obstructs the air paths to the lungs, making it difficult or in serious attacks, almost impossible to breathe. Asthma is primarily caused by environmental factors, allergens, as well as viral infections. In addition, Healthy People 2020 seeks to increase the fraction of people with current asthma who get appropriate asthma care, according to NAEPP (Etats-Unis, Department of Health and Human Services, Centers for Disease Control and Prevention & National Center for Health Statistics, 2012). Part of this initiative includes evaluating and monitoring, patient education and awareness, and controlling factors that contribute to asthma severity and medical treatment. In line with the goals of the Healthy People 2020, CDC established the National Asthma Control Program (NACP) to support millions of individuals with Asthma in the United States to control the disease (Centers for Disease Control and Prevention, 2017). The program seeks to minimize the number of asthma-related deaths, reduce asthma hospitalizations and emergency visits, and reduce cases of school days or workdays missed.

Which factor do you feel has the most impact on family members? This is a difficult question for me to answer. Being a hospice nurse, I see on a daily basis, the impacts of chronic illness on family members. I would have to say that in my experience, depending on their situation, one topic may have a greater impact than for other families. I have also found that the impact of one often leads to impacts by others. For example, children, who often live out of state, have to take time off of work to care for their ill parents. This in turn leads to financial strain as well as their family feeling neglected. If the patient requires around the clock care, then the carer because socially isolated as well as physically exhausted.
The physical exhaustion is something I see frequently due to the medicine regimen in the active dying phase of life. Often, the patient needs to be medicated every two hours and if there is only one caregiver that person does not get enough rest. This can lead to physical and emotional strain.
On a personal note, my mother cared for her mentally challenged sister for five years after my grandmother passed. She was very socially isolated because they didn’t leave the house much. I could see my mother because more depressed and having a shorter temper. I would try to help by staying with my aunt while my mother got out of the house but she always felt guilty and never stayed gone long. Now that my aunt has passed, my mother is volunteering and sitting with patients in hospice to allow their families that much needed time away. She has been an asset to these families because she can help them on a personal level.

After reading this article, I believe that the most impact on family members is that of the emotional impact. “Family members suffer greatly from the emotional effects of living with, and caring for, a relative with a disease, with the impact of some diseases being felt by every member of the family” (Golics, Basra, Finlay, & Salek. 2013). From the stress of the family members of the patient they are also at risk of affecting their sleep, poor food choices, concerns of their medical care and limited freedom. I believe that it is important to include the patient’s family in the plan of care in efforts to maintain compliance. Family members can help keep the patient on track of their care and can also hold the patient accountable. Families are usually a good support system for the patient.

For this weeks discussion I chose to discuss Diabetes. The goal of Healthy People 2020 is to reduce the risk of burden for those with Diabetes and to improve the quality of life for those with diabetes. Diabetes occurs when the body cannot produce enough insulin and/or cannot respond appropriately to insulin. Insulin is a hormone that the body needs to absorb and use sugar as fuel for the body’s cells. Without a properly functioning insulin signaling system, blood glucose levels become elevated and other metabolic abnormalities occur, leading to the development of serious, disabling complications (Healthy People, 2018). One of the directives from the CDC was to design wellness programs at work to prevent diabetes and other workplace illnesses. This could also prevent complications associated with diabetes (CDC, 2017).
The role of the caregiver is a big part in patient success. If the patient has a caregiver who is actively involved in the disease process, the outcome for the patient will be much greater. However, the strain of taking care of a chronically ill patient, can be excruciating. Taking care of a patient who is chronically ill and sometimes non compliant can be very exhausting, both physically and mentally. When a patient will not comply with their treatment plan, it can make the caregiver question their role. Non-compliance on the part of the patient is the greatest cause of caregiver strain. In order to prevent this strain, caregivers must be able to remove themselves from the situations at times. They can only do what they can do. Pushing the patient will not necessarily make them comply with treatment.
