Palin’s quote was not only inaccurate, it could be said it was a scare tactic.  For those relying on politicians to define changes, especially as they relate to a lengthy new health care act, I view it as a scare tactic to sway voters against the ACA.  Nowhere in the ACA or AHCA does it enforce a “death panel” (Mason et. al., 2016). This quote from a politician is why it is so important for nurses to understand politics in health care and provide accurate, unbiased, and honest information.

  • Reflect on what informs your judgment: commitment to advance care planning, analysis of facts, and/or political party loyalties?

When it comes to my judgement, research and understanding of the facts are key.  Had I simply listened to Palin, I would not have understood the changes in health care the ACA implemented.  Although far from perfect, this act did increase the number of Americans with health coverage to 20 million people and seeks to provide better health promotion, allowing nurses to practice to the full extent of their education and licensure (Collins, 2018).

  • Is it right for nurses to endorse health reform legislation even if the legislation is not perfect? Does this apply to the recently failed American Health Care Act?

Few things in life are truly perfect.  As a nurse, I have learned you simply cannot make everyone happy and that applies to healthcare.  Nurses should seek to endorse healthcare policies that allow them to practice as nurses; teaching patients to be as independently healthy as possible.  This certainly applies to the failed AHCA.  Again, the debate is not over universal health coverage, but as to who should pay for it.  Nurses have identified the need for access to health care since the beginning of time.  Advocating and endorsing legislative that is supportive of this idea should be the nurses priority.

       I think her strategy was both unethical and excessive, but it was effective.

 She used false, unsubstantiated statements to affect a key health issue that

would have benefited a lot of patients.  The bill would have allowed Medicare to pay for optional doctor’s visits to discuss living wills and end of life issues with their physicians (Mason, 2016).  As a member of Congress, she and her family are afforded a higher level of health insurance that is effective for life after serving 6 years.  Due to her statements all references to end of life planning were deleted from the bill.

Reflect on what informs your judgment: commitment to advance care planning, analysis of facts, and/or political party loyalties?

     I tend to be loyal to my political party but only if the facts support what I think is right.  I believe we should all be afforded the right to discuss end of life issues and living wills and have it covered by our insurance and not have our families burdened with these decisions on their own.

Is it right for nurses to endorse health reform legislation even if the legislation is not perfect? Does this apply to the recently failed American Health Care Act?

     There is no perfect legislation so there must be some compromise.  I think nurses should endorse legislation if it is for the improvement of health care and in the best interest of the patients and nursing practice.

Democrats’ Plans for Healthcare

I totally agree with Sarah Palin’s sentiments on the Democrats plan for healthcare. I think what the Democrats are pushing for claiming to reduce healthcare is downright discriminatory and it will do more harm than good in the long run. The truth of the matter is, some people need more care than others. For a fact, such a system will not favor vulnerable populations like those with chronic illness, the elderly and the disabled. There are currently more than half a million people in America living with various forms of disabilities. Such people continue to remain in waiting lists to receive services due to the fact that Obamacare urges states to expand Medicaid coverage to able-bodied adults instead of prioritizing the services to those who need them the most (Hilberg, 2015).

My judgment is informed by a commitment to advance care of planning. For instance, recent estimates from the Urban Institute suggest that a big percentage of the individuals eligible to Obamacare Medicaid expansion are adults without children in their prime working years. This means that Obamacare does not provide medical services to the most vulnerable. Another report published by the Foundation for Government Accountability showcases how Medicaid rolls have erupted under Obamacare and also how individuals with disabilities have suffered owing to this.

The Role of Nurses in this Plight

It is important to note that nurses are always in the frontline in boardrooms and in leading healthcare organizations. Given their high level of education and their experience, they have the intelligence to observe, validate and the network to transmit messages that advocate for healthcare reform values. Taking this into consideration, it is not right for them to endorse a legislation even when it is not perfect. If they leverage their numbers and use their influence efficiently, they will be able to support only the legislative changes that strengthen the system instead of those that weaken it (Béland, Rocco & Waddan, 2016).

The above also applies to the recently failed American Care Act. They must rigorously oppose legislation that could undermine or compromise the goals of the healthcare system. Nurses must, therefore, make an extra effort to communicate, listen and collaborate with the new congressional leaders. I believe that by putting their voices together, they can become part of the solution when it comes to directing policy through political goodwill (Béland, Rocco & Waddan, 2016).

I believe Palin bringing up the ration of care for the sick, disabled and elderly was very important and a very effective way to grab the attention of many Americans. Not every American has a disable family member nor do they have to per say “deal”or worry with all of the medical and financial burdens these families have to take on. I believe Palin bringing this to many americans attention and speak for the people that are unable to speak for themselves was very effective when it comes to making it known they are also worthy of the care that every other “normal” American is able to receive.

It is hard to pick one thing that would inform someones judgement, but I would have to say analysis of facts. It is important to always do your own research and not base everything you believe off of what other people are saying. With that being said, I do still agree with Palin and her advocating for the sick, elderly and disabled people of America but doing my own research also taught me more about what Palin was referring to. I came across this statement “The health reform bill being considered in the House of Representatives says that a Comparative Effectiveness Research Center shall “conduct, support, and synthesize research” that looks at “outcomes, effectiveness, and appropriateness of health care services and procedures in order to identify the manner in which diseases, disorders, and other health conditions can most effectively and appropriately be prevented, diagnosed, treated, and managed clinically.” (2009, Politifact).  It was not solely about and nothing to do with evaluating patients for their worthiness.

Yes I believe it is very important for nurses to be involved in politics and endorsement of health reforms even if they are not considered to be perfect. Nurses are huge advocates and we have a voice that could speak out make a huge difference.

In the meantime, an emergency squad arrived and resuscitated the spectator. On Monday morning, you learn of the absence of the AED only to find out that it had been locked in the custodian’s closet. Some of the goals of the community towards the same are the collaboration with the community-based organizations to provide affordable health care, to improve the quality of health care and increase the access points for community engagement and especially in healthcare.

The key stakeholders in the healthcare and nursing project are the individuals in the community, the community-based organizations and the various leadership authorities such as the state and the federal government (Bourcier et al., 2015). The healthcare institutions in the community are also part of the stakeholders in the community engagement with the public health initiatives (Bourcier et al., 2015).

Some of the goals in care to the community, according to Kangovi et al. (2014), are as follows. Firstly, the community-based organizations are targeting towards individualizing healthcare and bringing it closer to the populace (Kangovi et al., 2014). The other goal in care is to develop a comprehensive and diverse workforce that will be instrumental to the healthcare observance in the community (Kangovi et al., 2014). Partnership with the community is the other goal in care to improve the social and health wellbeing of the community members (Kangovi et al., 2014).

Some of the initiatives underway are the development of the community-based organizations to act as bridges to access of health care by the public. The other initiative is the diversification of the healthcare and nursing professionals to equip them with skills that will enable them to be some of the key players in the provision of health care to the community.

  Identifying public health issues is an important task that requires attention in order to implement change. Often times, change is opposed and not welcomed. With reference to the situation presented, there are benefits and hindrances that could arise. The need for access to pedestrian walkways would be presented based on the safety that these walkways would offer as well as the health benefit that these walkways would offer. With the walkways, there would be no reason for people to be walking in the roadway or close to the road, risking being injured by a motorist. There is less risk of injury since the walkways would provide safe and level paved ground to walk on. The health benefit would come from the walkways giving persons wanting to walk, a safe spot to walk or run and not be in the way of cars traveling the roadways. Walking has many health benefits that include:

  • increased cardiovascular and pulmonary (heart and lung) fitness
  • reduced risk of heart disease and stroke
  • improved management of conditions such as hypertension (high blood pressure), high cholesterol, joint, and muscular pain or stiffness, and diabetes
  • stronger bones and improved balance
  • increased muscle strength and endurance
  • reduced body fat.

Two different hindrances that could be expected include cost and space. The need for the walkways would be a good tool for encouraging those in the city council and/or county commission to approve the funding. Space may be a problem if the roadways are close to housing and stores. Dealing with the right of way laws, and getting private citizens to be onboard with the construction could also become a problem. Educating the public on what these sidewalks could mean for the health of the community could potentially increase the approval as well as recruit others to promote the idea.

Changes in the community can be a difficult process. Because changes within the community do not affect just one person. However, a difference like imputing a sidewalk is a positive change that is more promising to not affect the community in a negative way. Two key factors that would facilitate a change are, health and safety. Not only will the sidewalks make the community look beautiful, they will aid in providing a safe effective way for people to exercise and commute. Utilizing these two factors, as well as educating can help persuade the community as to why these sidewalks are so important. The community team could also get a petition and signatures from around the community, showing support to present to whoever will be involved in the decision making. 

Any change will bring about issues. The main ones presented in this case are cost and not having enough to facilitate the change. Yet, to fix cost issues, the community could host a number of events from car washes, to selling food (cupcakes, cookies), have fundraisers and money from donors. Not necessarily having enough, meaning the community support or enough factors to persuade the change could hinder it as well. “Another distinguishing characteristic of community activism is that the primary commitment and motivation for change are generated from within the community of interest” (Mason, Garden, et. al, 2016, p.651). Without any support, commitment or motivation to aid in this change, the project may fail. Making use of the team by educating, getting a petition and having the community involved with the change, will better the chances for all.
