The provisions of the Affordable Care Act (ACA) directs all employers to provide minimum value and affordable medical cover to all full-time employees or else be penalized whereas the American Health Care Act (AHCA) removed the penalty on defaulting employers (Conley, Dygert & Sellstrom, 2016). Under the ACA, it is the people’s mandate to buy healthcare coverage whereas the AHCA abolished the penalty requiring individuals to purchase health cover. However, the AHCA issues penalties to individuals who fail to pay their healthcare coverage for more than 63 days in the last 12 months, and youth who are past the age limit under parental cover and did not enroll for health cover of that year (Conley, Dygert & Sellstrom, 2016).
The penalty is a 30% surcharge in premiums, which can last for a period of 12 months (Conley, Dygert & Sellstrom, 2016). On the aspect of coverage affordability, ACA provides for cost-sharing subsidies in low-income families or individuals to allow for exchange plans for up to an average of 70%. The premiums to be paid by such individuals and families have tax reductions to enable low-income earners afford healthcare insurance (Mach, 2017). On the other hand, the AHCA offers credits on the basis of age with fixed amounts such as below 30 years-$2000, 30-40 years-$2500, $3,500-50-60 years, and it faced out credits for people earning incomes that exceed $75,000 (Conley, Dygert & Sellstrom, 2016). The ACA removed additional charges for health status whereas AHCA charges 30% standard rate for people with preexisting health complication for the past 12 months (Mach, 2017).
Opinion on Why AHCA Failed in the first Passage Attempt
The most likely reasons for the failure of AHCA during its first passage attempt were due to its provisions that would impact negatively in low income families and the less-privileged individuals with regard to their preexisting health status. For instance, the AHCA lifted penalties that employers were to be charged if they defaulted paying the compulsory health cover for all their permanent employees. This implies that employees could pay their employees’ cover only when they wished, which could push the burden back to employees. The other reasons are that people with preexisting medical conditions would be required to pay more premiums, and abolished sharing of insurance cover, and charging premiums based on age instead of economic capabilities. This means that you can be charged little premiums even if you earn high, and you can be charged high premiums even if you poor depending on your age.

The four recommendations of the IOM report include: nurses should practice to the full extent of their education and training, nurses should achieve higher levels of education and training through an improved education system that promotes seamless academic progression, nurses should be full partners, with physicians and other healthcare professionals, in redesigning health care in the United States, and effective workforce planning and policy making require better data collection and information infrastructure(IOM, 2015). While after researching, there are strides being taken, I do not think that Georgia has met exceeded the goals but progress is being made. The accomplishments to date include:
· Established a workforce center. Held two doctoral symposiums that have brought students and faculty together to develop skills, present new work, and network. Convened advanced practice nurses (APN) from around the state to discuss the state of APN practice in Georgia and begin plans on how to move forward. Facilitated negotiations with the Secretary of State’s office, the Georgia Board of Nursing, and the National Council of State Boards of Nursing to obtain up-to-date workforce data from the recently-revamped re-licensure survey based on the national minimum data set.

Their report titled the “Future of Nursing: Leading Change, Advancing Health” set a path for advancing the responsibilities or role of nurses in the United States. The report proposes eight recommendations: a nurse ought to practice to the full extent of his or her education and training; and a nurse should attain higher levels of training plus education through improved educational system that enhances seamless educational progression. Thirdly, nurses ought to be full partners with other healthcare practitioners in redesigning the United States healthcare practice. Finally, there is need for better data collection plus improved information infrastructure for effective workforce planning as well as policy making (RWJF & IOM, 2010).
Since 2010, various states across United States have established coalition to help realize the recommendations of the Institute of Medicine and the Robert Wood Johnson Foundation report. One such action coalition is the Florida Action Coalition. This coalition, headquartered at the Florida Nursing Working Center, conducted a statewide study whose findings were used as a foundation for a training program of promoting nursing leadership. The coalition also established a program for emerging nurse leaders to provide a 90 day learning experience through group and individual coaching, online courses as well as webinars. The Florida Action Coalition’s website features graduates who are linked with new organizations searching for nurse leaders join their boards. So far, 18 out of 26 graduates of the initiative are working on policy making boards and other agencies. Finally, the coalition has partnered with the Florida Blue- a component of the Blue Cross and Blue Shield Association-to strengthen its ability to not only sustain itself, but also for statewide health improvement (FAC, 2018).

American Health Care Act (AHCA) presented by Trump’s administration and Obama’s Affordable Care Act (ACA) demonstrate different visions of the parties on the role of government, rights, and responsibilities of citizens in the sphere of health care. Whereas ACA is set to achieve a nearly universal access to health care, AHCA strives to limit federal spendings and optimize the resource usage. The failure of the first version of AHCA was attributed to the alleged jeopardy, in which it would have put the insurance system in the U.S. One of the main reasons was substantial limitations proposed for ACA including less federal financing and extended privileges of states for discontinuation of service for those having ACA insurance (Hirsch et al., 2017). In addition, it did not address the major problem concerning all the U.S. states such as the increase in health care costs.
Presently, under ACA the state provides subsidized health insurance for the family of four if its annual income is below 97,000 dollars. Under AHCA that limit was raised to 150,000 (Hirsch et al., 2017). Another difference is that individual insurance that was eventually introduced to ACA was absent in AHCA. In addition to that, ACA ranged subsidies by income level, whereas AHCA initially proposed to substitute them for the age-based system, where the amount of benefit increased with age. The reform proposed by Trump’s administration was set to allocate the 15 billion from the Patient and State Stability Fund to address multiple issues including provision of assistance to high-risk patients, widening access to prevention services, increasing insurance coverage, and so forth (Hirsch et al., 2017).

The AHCA was written to solve a crisis that doesn’t exist and accordingly accepts sacrifices it doesn’t need to ( Q1,2018). It’s akin to a doctor who wants to amputate a patient’s leg, a reasonable solution if gangrene has set in, but disastrous in case of a botched diagnosis. Everyone will face the specter of getting priced of health insurance altogether if they lose coverage. For them this is functionally a mandate, except instead of a financial penalty the AHCA punishes them by withholding access to care. I think one of the somewhat surprising elements of the bill was the fact that they kept in the essential health benefits, the ten classes of benefits that need to be covered just because it’s covered doesn’t mean that you’re going to be able to afford them. For the people who have coverage, meaning they are in an insurance plan, what is the extent of the out of pocket exposure they have to get the care they need. As it turns out, studies showing better health outcomes for those with insurance looked at individuals having private insurance. On the other hand, multiple studies, assessing the effect of the Medicaid expansion, found no improvement in health outcomes for those on Medicaid. In fact, other investigations have found worse outcomes for those with Medicaid as compared to those with no insurance at all.
