Contact your local public health department to learn about its role in a local disaster, including the role of the nurses who work there.
I called the ……… county health department and spoke with a Mr. ………… He explained to me that he has a list of local nurses that he sends out an email to in order to ask for assistance when there is a need. Currently he is working out at the panhandle and stated that he does have a few volunteers from here who are helping to wrap things up at some of the shelters from the last hurricane. It’s nice that all the countries that have help available come in and help when there is a need. He said that sometimes they need nurses to help with flu shots and such and so he sends out an email and if you are available you just show up. He asked me if I knew anything about the Medical Reserve Corps (MRC). I had only just read about it in our material this week. As you guys may already know the MRC is a government organization comprised of a large network of volunteers that come together to improve the welfare and safety of a community (HHS, 2016). Mr. ………asked if I was interested in being a part of it. 

Would you be a good candidate to serve on a disaster team?
I do believe that I have something to offer the community, we all do. According to the information that Mr. …….. gave me, I have the opportunity to decline when I am needed if I have other obligations. So of course my job and my family come 1st. I did sign up to be part of the e-mailing group and Mr. ……. will be forwarding my information to another colleague in order to set up membership to the MRC.
Would your current employer be flexible in allowing you to participate in a disaster situation?
I am already part of the disaster team for work, so they would take priority. If I were PRN, I am sure they would be accommodating, but I am a full time staff member, and since being part of the MRC is voluntary, they would not be a priority for me at this time.
What is your personal preparedness to face a disaster?
We have a hurricane/disaster box prepared that has everything needed to help sustain us for 1 week. I have lived in FL for so long that I grew up with a family plan and so I carry that with me now that I have my own family. I have only ever evacuated once and having our preparedness box made that process a lot less stressful.

    • The role of the local and state health departments is to make sure everyone has good health. “Florida Health represents the vision that health belongs to everyone and every group—not just a state agency. It takes individuals, families, communities, and partnerships with local and state-level policy makers and stakeholders to create a culture of health in Florida” (floridahealth.gov, 2018). 
  • How do their goals of health promotion meet the objective of Healthy People 2020?
    • I think everyone’s goal in general is for the public to be in good health. Being in good health can reduce the number of hospital admissions and prolong your life. Ways to spread the word are communication! Social media, interacting with others, and just being aware of what’s out there. For instance, what kinds of food are we putting into our bodies, are we drinking enough water, and is the environment less polluted with toxins that can harm us.
  • How could nurses in the public health setting help clients to modify unhealthy behaviors at the primary and secondary prevention levels? Please provide an example for each prevention level.
    • Primary prevention would be education. Examples include educating the community about eating healthy or the dangers of smoking.
    • Secondary prevention would be reducing a disease or diagnosis. Having regular check-ups with the doctor or taking medicine pertinent to that disease would be examples.

Select an infectious disease and research the CDC website for information about the disease, its natural history, presenting symptoms, and outbreak characteristics. Identify an occurrence of the disease by searching the Internet for recent reports of this disease and compare that episode or occurrence with information from the CDC website.

Acute flaccid myelitis (AFM) is a rare infectious disease that affects the nervous system. The disease cause inflammation to spinal cord predominantly the gray matter, which cause the muscles and reflexes in the body to become weak. According to CDC, there are several probable causes of AFM such as, viruses like  poliovirus and non-polio enteroviruses, West Nile virus, and adenoviruses environmental toxins and genetic disorders.

The symptoms present with sudden onset of upper and lower extremities weakness, loss of muscle tone and reflexes. Some people will also experience facial drooping, ptosis, difficulty moving the eyes, slurred speech, difficulty swallowing, pain in the extremities, and inability to urinate. The most severe affect is respiratory failure.

According to CDC, from August 2014 through September 2018 there’s been a total of 386 confirmed cases, mostly cases involve children. Currently for 2018 with presenting onset of symptoms for September to October 16,2018; there are 62 confirmed cases in 22 state in the U.S.The patients’ symptoms have been most similar to complications of infection with certain viruses, including poliovirus, non-polio enteroviruses, adenoviruses, and West Nile virus. The diagnostic test ran all AFM cases have tested negative for poliovirus. “The increase in AFM cases in 2014 coincided with a national outbreak of severe respiratory illness among people caused by enterovirus D68 (EV-D68). Among the people confirmed with AFM, CDC did not consistently detect EV-D68 in every patient (Acute Flaccid Myelitis, 2018).

Natural History of Influenzas

          The influenza virus was once called the Spanish Flu. This was because at the end of May 1918, the King of Spain, King Alfonso XIII, was the first to openly admit that a new disease had emerged in their country (Mamelund, 2017). But the disease did not originate in Spain. Other countries during that period throughout Europe including the United States were experiencing the so called “Spanish” Flu. Since World War I, much has changed in terms of understanding the flu and the origination of infectious disease. It is now known that the Flu is caused by a virus. Statistical studies since 2000 have found that higher mortality rates occur among the poor in comparison to the more affluent (Mamelund, 2017). People who are more susceptible to influenza are those who have poor nutrition, live in overcrowded areas, have secondary infections like pneumonia and other diseases (Mamelund, 2017).

Presenting Symptoms of Influenza

          According to the Centers for Disease Control and Prevention (CDC), Influenza is a contagious respiratory illness caused by influenza viruses. It can cause mild to severe illness and even cause death (Influenza, 2018).

  • Fever or feeling feverish/chills
  • Cough
  • Sore throat
  • Runny or stuffy nose
  • Muscle or body aches
  • Headaches
  • Fatigue
  • Some people may have vomiting and diarrhea (More common in children than adults)

Outbreak characteristics

          According to the CDC, people who have the flu are most contagious in the first 3 to 4 days after infection. People who are healthy can start infecting others, 1 day before symptoms develop and up to 5 to 7 days after getting sick (Influenza, 2018). In addition, some people can have the virus, have no symptoms and spread it to others.

          The World Health Organization’s Global Influenza Surveillance and Response System was established in 1952 (Influenza, 2018). The purpose is to reduce the spread of infection by vaccines.  Based on the surveillance system, Influenza activity in the United States remains low throughout the summer months and early October. Outpatient illness surveillance: The proportion of outpatient visits for influenza-like illness remained low and was 1.4% which is below the national baseline of 2.2%

  • Have there been challenges you have faced? If so, what strategies did you employ to overcome these challenges?’

I believe it is impossible to have any type of job that you would not have to find a way to overcome certain challenges. I am a nurse on a med surg floor right now and I would day the biggest challenge I have faced so far would be staffing and nurse to patient ratio. “Nurses are the providers with the greatest presence during health care delivery, and they provide the most holistic approach to patients” (Diana J. Mason, 2016, p. 161). With that being said, on the med surg floor I work on, we sometimes have 7 patients each and it makes it close to impossible for me to feel like I am being the nurse that I know I can be and provide the care that each and every patient deserves. Being a nightshift nurse I feel like they believe we are not busy or we have a lot of down time to be able to handle these 7 patients but I feel as I do not get to spend quality time and provide the care patients need having 7 patients. Prioritizing each patients needs is the only way I’ve been able to make it through nights that we have had 7 patients each. I make sure to show up at work at least 30 minutes early to look at my assignments and see what exactly it is that I am going to be working with. 

  • How do you envision your career moving forward in advocating for your patients? Your community? Yourself?

I feel like a large part of being a nurse is being a patients advocate. You have to listen to each patients wants and needs and be there to help lead them in the right direction of the care they need. It is important to listen to a patients needs and get whatever question they have regarding their health answered. Ive been a nurse for a little over a year now and I feel like I am becoming more and more of a patient advocate. I feel more comfortable and confident in the care I provide as well as educating patients and helping them form questions they may have for their doctors or other medical staff. I have recently signed on with a travel agency and I plan to start taking a few assignments here and there in hopes to help build skill and confidence in my nursing profession and be able to be more of an advocate for my community as well as myself.

The main challenge that comes to mind for me personally is juggling the patient load. I transferred from the pediatric floor to the med surg floor. When I worked on peds it was very rare that we would ever have more than 5 patients and they would usually call another nurse in. I know that babies and children’s conditions can be more severe at times because of how quickly they can decline, but I feel that on the med surg floor we should also have no more than 5 patients due to the actual work load each one requires. The average day on the med surg floor you will be taking care of 6 patients and 7 at times. This is a challenge to me because during the day time is when all the commotion is going on. It’s when doctors, therapists, and everyone comes and it’s also when you are typically passing the most meds. I think the patient load they give all of us nurses is neither fair nor safe. How can you adequately care for that many people and please all of them?  This is where patient satisfaction begins to fall because one of the patients will be calling you needing something but you are stuck with another patent and also have 10 other things you need to be doing at the same time. You want to be there for each patient as soon as they need you but it is nearly impossible. You want to provide the best quality of care possible. Every nurse plays a pivotal role in the measurement of quality (Mason, Gardner, Outlaw, & O’Grady, 2016 ). The only way to overcome these challenges is one for the hospital to hire more nurses which has not happened yet or two to hopefully have co-workers who do try to help you in overwhelming times. I am thankful to say that I do have co-workers who will step in and help when the moment gets crazy.

 I believe my career could only continue to improve by advocating for my patients and myself. It is important to always take time to be concerned and listen to what your patients are feeling and telling you. As the patients advocate you are the most important key to their quality of health while they are in the hospital.
