Holistic care includes biological, social, psychological and spiritual needs. (Zamanzadeh, Jasemi, Valizadeh, Keogh, & Taleghani, 2015). Biological and physiological needs are integrated into day to day life.
However, due to her living situation joining different groups and communities is not practical since they are always on the move. Luckily, there are many communities that take place on the Internet and she is efficient with the computer. Some groups she can join online include support group, exercise group, a book club, and a cooking group.
These things would help to manage her age related changes with a social aspect.
Exercise, diet, and support groups would help her manage her breathing difficulties, GERD, and arthritis. While they are stopped she could visit local parks and churches, as well as group activities such as bingo. MY ANSWER:
The management of chronic conditions is becoming an integral part of medical care in most parts of the world. The elderly, in particular, are living with one or more chronic illnesses for very long periods. Access to care is not easy, given that most of them are not insured. The situation gets worse when the patient is from an ethnical minority background. Since it can be hard to reach such patients, the first strategy that both the medical staff and the care givers of the patient should put in place is fostering positive communication. Putting a lot of focus in disease prevention, health promotion and early detection is a very effective strategy for enduring successful chronic disease management. This is especially helpful for the frail seniors (Hitchcock, 2016). It is, therefore, important for the care givers to listen carefully, show compassion and work with the patient to ensure care goals.
The second strategy would be to embark on a patient-centered approach. Such an approach is supposed to be responsive to, and respectful of, the preference, needs and values of the patient. Physicians should never make assumptions about the treatment option that the patient would choose. Rather, they should discuss also the available options. Doctors should also recognize when such senior patients need help in managing their chronic illness (Iecovich, 2018). Again, it may be important to carefully watch the interactions between different medications that are used to treat different conditions. The physicians and the patient should further come up with an action-plan and always keep it up to date.
While caring for the elderly patients can pose a big challenge for physicians, it does not mean that it is an impossible mission. With the right strategies in place and with the coordination of the doctors, the patients and their caregivers, it will be easier to manage such chronic conditions.

Non-compliance with medication and dietary regimens has caused Mrs. T.T.’s blood sugars and wound healing to be compromised. Getting this patient on the right track to facilitate a healthier way of eating which in turn would help with the wound healing process, decrease her HGB A1 ,and promote weight loss. Maintaining and conforming to a healthier eating plan and exercising at least three times a week should boost her wellness. Mrs. T>T’s efforts to conform to these lifestyle changes will not be in vain if the end result increases her longevity as well as her quality of life. First lab values should be obtained to get a baseline. Secondly, teaching her how to accurately and frequently obtain her blood glucose level and how and when to use her insulin is a fundamental part of this wellness process. Providing education as well as demonstrating healthier ways to prepare food is a vital component as it relates to wellness. Mrs. T>T. should be taught how to dress her wounds as well as being able to detect, if any new wounds are present and when to contact the physician or wound nurse. A Wellness Program for the Elderly
Target Population: The target population for this wellness program will be the elderly people like —– who suffer from arthritis and do not have the right health management resources at their disposal.
The unmet needs of the population: After having a conversation with —-, so many things surfaced. For example, he told me that the medical staffs do not give patients like him the needed medical care, meaning that there is a big gap in healthcare. He also extended a lot of gratitude to his caretaker saying that were it not for her, his condition would have been far much worse. While this was true, I also felt that — is not given the care that is required since he constantly complains of severe joint pains and has difficulties doing the daily tasks such as standing up on his own and bathing himself. He is just a representation of thousands of other elder patients like him who have absolutely no one to take care of them and are uninsured.
Holistic interventions to address the identified needs: The following are the strategies that will be included in the program. First and foremost, professionals will be hired to engage such elderly persons with exercises such as aerobics, range of motion exercises strengthening and enduring exercises (Mok, 2018). In the program, the patients will be taught on the best ways to manage their pain given that arthritis is a life-long condition that can only be managed using the right treatments and protocols (Roedl, Wilson & Fine, 2016). Furthermore, I would engage doctors in the program so that they can be able to get a first-hand glance of the problem on the ground.
Implementation of the program: The proposed program will be developed in phases. The first phase will be the trial phase and if all goes well, full implementation will follow. This initiative is targeted at helping both organizations and elderly patients into their journey towards living a life with arthritis.

The older adult should be 65 years or older. Use the format provided to record the responses. A list of questions is available for you to start with. Include 2–3 questions of your own to get a complete picture of the older adult. Summarize your findings and also contrast the responses with findings in your readings and other current literature.
Download a patient questionnaire. This form should be used as an example.
After gaining permission, conduct a physical and mental functional assessment of the older adult you have chosen. Review your readings for the process of functional assessment.
Make use of the tools discussed this week to complete a comprehensive assessment of your patient. Search the Internet for resources on these tools.
Make sure the older adult is clearly identified on the tools. Do not include their name, but do include professional or other designation, and age. Your name should also be identified on the tool. (This should be a part of your Appendix.)
