HOMEWORK ASSIGNMENT:

For Module 3, conduct a literature search utilizing peer-reviewed journal articles, books, professional organization Web sites and articles, etc., to continue working on your program/intervention plan (which you started in Module 2) and propose appropriate evaluation measures to evaluate effectiveness of your program.

Write a paper (3 pages in length) including a complete workplan that shows your implementation procedures to address the health behavior and describing your methods to evaluate your program/intervention. A workplan sample can be found at http://www.cdc.gov/OralHealth/state_programs/pdf/workplans.pdf

The following items will be assessed in particular:

  1. Use the feedback I gave you in the previous module to refine and update your goal and objectives.
  2. Include a complete workplan with the following components: goals, objectives, activities, data measurement tools, timeframe, responsible person, and measures of success.
  3. Describe how you will evaluate the effectiveness of your program. Make sure to include information on what evaluation tool you plan to use (e.g., questionnaire and survey).

Assignment Overview

The Session Long Project (SLP) is a progressive assignment that involves assessing a health behavior; researching and identifying an evidence-based model or best practice strategies to best address the health behavior; creating a plan (i.e., program or an intervention) and outlining implementation procedures; and suggesting appropriate evaluation tools to measure effectiveness of the proposed program or intervention. This will be developed in phases throughout the session. The task and assignment expectations for each segment of the project will be explained in each module.

Choose one of the following topics to focus on for the rest of the Session Long Project (SLP):

  • Teen pregnancy.
  • Older adults’      injury from falls.

You will be writing your Module 1 – 4 SLP assignments based on the one topic you have chosen from the list above.

For Module 1, conduct a literature search utilizing peer-reviewed journal articles, books, professional organization articles, government Web sites, etc. Write a 3-page background paper on the health behavior you selected. Make sure you meet the expectations in order to be eligible to earn full credit.

The following items will be assessed in particular:

  1. Identify one topic      and a target population (from the SLP topics above) to focus on. Describe      your target population and the health behavior(s) that relate to the topic      you have chosen.
  2. Based on your      literature search, assess and provide background information on the health      behaviors. Include statistics to explain the scope of the problem.
  3. Provide an      explanation as to why it is important to address the health behavior you      have chosen.
  4. What benefits or      consequences do the behaviors have on individuals’ health?

Health Behavior Slp2

By now, you have identified and done some literature search on  your Module 1 Session Long Project (SLP) topic/health behavior and  target population. In short, you have assessed the need for a program or  intervention to address the health behavior you previously identified.

For Module 2 SLP, you will again conduct a literature search  utilizing peer-reviewed journal articles, books, professional  organization articles, etc., to help you identify evidence-based  strategies to best address the health behavior in your target population  and create a program/intervention plan.

Write a paper (3 pages in length) summarizing your findings on  evidence-based strategies and a workplan to address the health behavior.  A workplan sample can be found at http://www.cdc.gov/OralHealth/state_programs/pdf/workplans.pdf

The following items will be assessed in particular:

  1. Using the same topic you have chosen from Module 1 SLP, develop a  program goal and at least two objectives (1 learning objective and 1  behavioral objective) for your target population. Your objectives must  be specific, measureable, achievable, realistic, and time-phased  (SMART). Make sure that you have read the required readings on SMART  objectives before developing your goal and objectives.
  2. Based on your literature search, provide a summary of evidence-based  strategies that have been shown effective in promoting or reducing the  behavior in your target population.
  3. Identify two evidence-based strategies that you will use to  implement your program or intervention. Provide an explanation why you  have chosen these strategies and defend your choices.
  4. For now, include a workplan table with your goals, objectives, and  activities/strategies. NOTE: Other parts of the workplan such as the  data measurement tools, timeframe, responsible person, and measures of  success will be completed in the next module SLP.
