Monica Bullock    4 posts   Re: Topic 2 DQ 2  After learning about evidenced based practice in the work place and nursing practice I feel like my organization is making an effort to shift nursing practice. In dialysis a patient is complicated and they come in with many co morbities. On top of disease they come with many life issues such as no housing, minimal insurance, and a fixed income. In order for a dialysis patient to have a good patient outcome the staff must work as an interdisciplinary team to ensure success (Desai, 2008). Many practices in dialysis are based on evidenced based practice, for example fluid management and access care. I feel like my organization does a good job in educating staff about practices and changes in practice. Knowledge is list as one of the top reasons to effectively incorporate evidence into clinical practice (Garner, 2016). We have a lot of online training, but it would be helpful to have more continued education in person with our educators. I see the efforts of incorporating evidence based practice, but the struggle is the time to read and look over the information provided. On a typical day there is minimal down time making it difficult to do the online training and reading the information provided for the topic. On days off the information is not accessible, not saying that anyone would want to look at work related stuff on their days off. The biggest challenge to implementing evidence-based practice (EBP) within the organization according to my mentor is resistance from physicians and upper administration. She explains numerous individuals want to continue the path that they have always used. With change comes conflict. Overcoming the barriers experienced by the organization with implementing evidence-based nursing into practice has started by updating the policies and procedures to reflect current evidence-based practice measures. The nursing managers, directors, and quality/risk team hold meetings to review and revise practice standards to demonstrate present evidence-based nursing practice. When it comes to executing new policies, there is a level of opposition from nursing staff and providers alike. A possible solution as described by Straka, Brandt, and Brytus, (2013), could be monthly 30-minute lunch and learn continuing education sessions. These meetings would review topics such as searching and understanding literature, and leading staff through practice change (Straka et al., 2013). Getting staff to support EBP is also important to resolving the current issues. O.A.S.I.S. which means open your eyes, ask questions, search the literature, implement the findings and share the results is explained by Straka et al. (2013) to inspire nurses to examine clinical problems and examine the literature for best practices. Empowering staff is key to getting them to endorse an EBP culture.  Reference  Straka, K.                    Discussion: Nurses as Leaders in Health Care Reform

As health care delivery in the United States continues to evolve, either through mandates, improved technologies and training, or other drivers, nurses remain at the forefront in facilitating the success of new initiatives. In 2010, the Institute of Medicine formed a committee of experts to address the following question: “What roles can nursing assume to address the increasing demand for safe, high-quality, and effective health care services?” In the final report of the committee, The Future of Nursing: Leading Change, Advancing Health, the authors stated:

This report offers recommendations that collectively serve as a blueprint to (1) ensure that nurses can practice to the full extent of their education and training, (2) improve nursing education, (3) provide opportunities for nurses to assume leadership positions and to serve as full partners in health care redesign and improvement efforts, and (4) improve data collection for workforce planning and policy making. (p. 10)

For this Discussion, you will focus on the research recommendations presented by the committee concerning the role of nurses as leaders. Policy review doesn’t have to be as daunting a task as it sounds. By reviewing policies and procedures the organization is able to incorporate the current evidence-based practice and wed off the old ones.  Also, outdated policies can leave the organization at risk. old policies may fail to comply with new laws and regulations. They may not address the new systems or technology  Violations of the nursing standard of care can result in the actions against the individual, survey deficiencies especially if the nurse does not follow the facility policies and procedures in rendering the care. It is the facility responsibilities to educate staff and update their policies according to the new literature on Evidenced-Based Practice  Health care organizations need to consider multiple strategies to facilitate and promote evidence-based practice. Nimmy James    3 posts   Re: Topic 1 DQ 2  Providing quality care to the patient very important in nursing the Evidence-based practice (EBP) takes a major role for this outcome. “The EBP process is a method that allows the practitioner to assess research, clinical guidelines, and other information resources based on high quality findings and apply the results to practice” (Spyder, 2018).  In this study, 2100 questionnaires were given to registered nurses find out their thoughts regarding on EBP. The positive expression towards the EBP were more than 64% (Majid et al., 2011). But the nurse’s workload in daily routine causes many issues which they cannot keep up to date with new evidence. The nurses felt that EBP training, time availability, and mentoring by nurses with EBP experience would encourage them to implement EBP.  There are many issues and barriers that might hold nursing practice from achieving this goal. The first issue is the overload of information, means there are thousands of articles are published monthly which cause confusion and attain wrong data. Another issue is lack of time to adopt the EBP, lack of resources, and the difficulty in understanding statistical analysis (Majid et al., 2011).  Although nurses showed a positive attitude toward EBP, certain barriers were a delay. It is important that hospital management develop a comprehensive strategy for building EBP competencies through proper training and education. Kimberly Morris    1 posts   Re: Topic 9 DQ 2  One potential barrier that may prevent my evidence-based practice proposal from continuing its enthusiasm is the fact that the community in which I will be presenting may lose interest in the topic of lead. Because this topic has been brought to the forefront due to a recent crisis, once the initial public health hazard has been taken care of, the community may go back to being ignorant or not caring about the topic. One way in which I can battle this is to stress the fact that elevated blood lead levels are not an acute problem for the general public but instead they are a chronic problem. While the initial threat of elevated blood lead levels in children due to the public water change in Flint has passed, there are still may ways in which children can obtain lead. Pointing this out and stressing this in my education will hopefully allow me to continue an active interest within the community.  Another barrier I can see would be a one-size-fits-all approach to my chosen topic. Evidence-based practice changes are scientific based and often people take a strict adherence to the guidelines set forth within these proposals (Surface, 2009). These guidelines may not always fit with the audience you are working with and should be a revolving set of guidelines based on research and evidence. Just like my topic – 25 years ago obtaining elevated blood lead levels from imported toys and food items was not a big problem but in today’s global economy it has become an area of concern.  Reference  Surface, D.
