This will aid to enhance a much better outcome for these patients.Their trust in the profession of nursing will be strengthened. The issue here is about honesty and integrity in our work as nurses. We are expected to research enough materials and get an excellent grasp of the researched topic while giving credit to the sources of our acquired wisdom. As we practice doing this at this level of our education, we will easily transfer it to our professions as nurses. As we ethically practice this, we will be able to stand with those great giants in healthcare, and deliver the best possible care with integrity to those that have come to us with trust in their time of illness or trauma.

Two days ago, I was involved in the care of an oncology patient. She was no longer able to speak and was becoming vividly altered in mentation. She had been alert and oriented in the previous week, but at the shift report on this day, I was informed that she had climbed out of bed and fallen. When I got to her, I discovered that her condition was becoming more acute and she was hallucinating and declining fast. I made the Charge nurse aware of her increasing anxiety and inability to stay in bed, requesting for a sitter to keep her safe, I was told that we had no resources and that I should consider using a posey restraint or ativan to settle her. My lady is 91 years old with acute alteration in the mental status due to her disease process, the last thing I wanted to do was to increase her anxiety with the posey, I did not want to end her life either by giving her ativan. She was severly anorexic and receiving PPN due to her NPO status. I went and stayed in her room and attempted to run to my other patients from there. I was finally able to consult with the shift coordinator who subsequently provided me with a sitter for this patient. She lasted the night with a 1:1 sitter, without exotic drugs and received the care she needed.

Nurses are responsible for delivering optimal patient centered care, and in order to so, nurses must advocate for their patients, especially patients that cannot advocate for themselves. The article “Critical Care: The Role of Nurses as Patient Advocates,” states that a nurse’s role in being a patient advocate includes being communicators, liaisons, educators, interpretors, and caregivers (“Critical Care: The Role of Nurses as Patient Advocates,” 2016). Not too long ago I was taking care of an older patient who did not have a cardiac history, and their pulse kept dipping down into the high 20’s. I contacted the patient’s attending doctor three times to show my concern, but the attending just kept saying to continue to monitor the patient. The patient seemed worried as well, and didn’t feel like they were getting the proper attention. Fed up with the doctor not willing to examine the patient, I called an RRT which forced the attending doctor and other members of the interdisciplinary team to examine the patient. After the RRT, it was determined that patient needed closer monitoring and they were sent to the PCU. The doctor seemed annoyed with my decision, but the nursing supervisor and RRT nurse commended my efforts for advocating for the patient. Had I not initiated the RRT, the patient could have possibly experienced cardiac arrest and/or loss of heart function.

When her physician came to evaluate her, he was upset that she did not have an epidural. He started to try to intimidate and scare her into getting one.There was no medical necessity for it and she looked at me and started to cry. I verbalized to the physician that she did not want an epidural and I was going to be sure that whatever her wishes were, we were going to respect them, no matter how inconvenient he felt it was for him, as long as there was no medical contraindication. My patient delivered with no epidural and was so thankful and appreciative that I helped her not get bullied into an unnecessary procedure when there was no medical indication for it.

I have had many co-workers and former classmates complete their RN-BSN program through GCU; all have had wonderful things to say about the program and saw their careers soar. With their encouragement and time needed to save financially, I have decided that I needed to take the next steps in obtaining my BSN.
This is my first online class/program, so I am a little worried and concern about the structure of the program and expectations. I am also concerned with my ability to stay on task and complete everything I need to be successful in a timely fashion. But after reading 7 Habits of Highly Effective Nursing Students (Moore, 2015) and Balancing Work and Nursing School (All Nursing Schools, n.d.), I will create a working space for me to complete my school work, a calendar for assignments and proactively stay ahead of schedule.
