Involving families in care and education can enhance patient compliance. Family members can affect health of an individual based on their social characteristics, culture, the influences from their neighborhood, community and society (Sharma, 2013). Health seeking behaviors and preferences may also be influences by family structure and the changing dynamics of family composition can impact lifestyles and heath related risk factors (Sharma, 2013). Nurses have a powerful position to create a therapeutic relationship with patients and families and involving those who influence the health and wellbeing of an individual in efficient, quality health care (Ponte, et al., 2007). Nurses who are confident in their ability to acknowledge family structure and what patients and families need, can successfully implement actions to encourage high quality care. “The family influences on each other can be engaged to assist patients and family members make choices and changes for better health”

The definition of “family” has evolved over generations and expanded from the traditional male and female married couple and their children living under one roof to that of the modern way in which people are interacting with each other and carrying out relationships. Coinciding with the way people are relating to one and other today, the definition a family has broaden to “societal units of single-parent families; lesbian, bisexual, gay, trans gender, queer, and questioning (LBGTQ+) families; children raised by grandparents or other relatives; multi generational units; and people who are together out of friendship and love, not bloodlines. The form and function of the family dynamics directly influence the overall health of an individual. This is why understanding the different family structures and not assuming the “non-traditional” structure will enable the nurse to make accurate assessments on the health care needs of the individual as well as identifying gaps to meeting those needs and formulating a plan while honoring their preferences, beliefs and values. Understanding the family structure will give the nurse insight to the plan for aftercare and how well the goals will be met in the home setting. The family systems theory takes a look at the family as a whole system rather than individual members to study the family processes and problems. However, each family member has their own individual role, feelings, behaviors and goals that influence the rest of the family. Considering the family system will help nurses understand the interaction of the family because any “modification by an individual within the system can change the family’s finances, health, housing, communication, and dreams. Once a change is made within the member of the family then the dynamics in the whole system start to shift, which can result in either positive outcomes for everyone or create stressors and tensions for the other members to adapt to. Involving the whole family in the assessment and planning of care will create an environment of support and encouragement, promote education and increase the probability of healthier, positive behaviors.

The definition of the traditional family is “is the parents and offspring living together as a societal or social unit with the same ancestry” (Green, 2018). However, the definition of family that encompasses the different family structures today would be “societal units of single-parent families; lesbian, bisexual, gay, transgender, queer, and questioning (LBGTQ+) families; children raised by grandparents or other relatives; multigenerational units; and people who are together out of friendship and love, not bloodlines. Pets, often part of a household, may acquire the status of “family” members. Thus, family tends to connote a complex, diverse, and dynamic unit, often consisting of parents and children related by legal, genetic, or emotional ties” (Green, 2018). For example, I know some people who are unable to have children of their own and consider their pets as their children. I am sure many people have even heard the term ‘dog-mom’ being used throughout society as well. The family systems theory states that “Family members influence each other and the family as a whole” (Green, 2018), furthermore, each “…individual does not exist in isolation, but is part of a group or family system. Each family member experiences the togetherness, attachment, and intimacy of the family system, but the family member remains an independent thinker with individual purpose-driven actions” (Green, 2018). Whether or not a patient’s family is considered traditional or nontraditional, it is important for nurses to understand the family systems theory when providing and planning care because a patient’s family truly impacts their environment.

I think the definition of family is one that is likely to change per the patient and their individual take on the definition. What a family to me could be and is probably very different than what a family is to one of my patients. Family is either by choice, so the person or people have chosen this as their family and it could range from a best friend to a significant other or a family could be blood, the “biological” part of what makes people “family” and lastly, it could be a mixture of both choice and “biologic” that makes a family. It is important to acknowledge a non-traditional family because like I said before the people in the “family” does not matter but how they interact and the dynamic in which they live is what is more important than the who of the family. “A family systems approach to individuals and their families provides a means to better understand the interactions of a societal family unit ”  and family systems theories help us to understand what we, as nurses, need to look for when assessing and evaulating a family and their health.

With the development of the times, today’s family information is becoming more and more abundant. Family as a unit, each member is a part of the family. Everyone’s emotions, health, and habits will affect the entire family. The home is a place full of love, and also learning to communicate, understand, sympathize, collaborate, respect, and be respected in this place. Use the positive energy obtained at home to dissolve people in social contradictions with people, so that is why the concept of family is essential. “Nurses promote positive nurse-patient and nurse-family relationships. This incorporates an atmosphere of honesty, trust, caring, competence, and cooperation among all participants. The nurse guides assessments and conversation over sensitive topics, devoting attentive listening skills to all”. Health promotion of families includes use of referral services. For example, we know lots of people who are lonely and elderly do not need living in a nursing home. We can build active entertainment communities, including card games, mahjong, chess, games, etc. Open regularly and arrange, provided shuttle services. We can even help them supply some life needs and health education Missions. Unsuitable environments can cause many diseases. Provide a better communication environment for retired seniors and provide health prevention lectures can improve their health.

The concept of family health is important because behaviors from one family member influences the whole family; which plays a role in health maintenance and prevention of disease. The nurse can use structural, functional and developmental assessments to gain an understanding about families. Structural assessments look at individuals and where they are within the family’s current life cycle (Green, 2018). Functional assessments look at family routines along with verbal and nonverbal communication. Developmental assessments look at the development of the family: birth, marriage, death and separation (Green, 2018). By looking at the family structure, both traditional and nontraditional, the nurse takes into consideration family roles, behaviors, relationships, and lifestyle choices which have an influence on family and individual health. Risk factors that affect the health of families are also identified which include: violence, housing, sanitation, poverty, social isolation and biologic factors. The nurse is able to evaluate each family’s beliefs, preferences, needs and values in order to provide family-centered care (Green, 2018). Strengths and weaknesses are then assessed and identified by the nurse by listening and conducting an effective family interview in order to provide customized care. Depending on which family health deficits are identified, the nurse can identify which type of assessment is best fit to enable individuals to control their health.

Family Health Assessment Part I

Understanding family structure and style is essential to patient and family care. Conducting a family interview and needs assessment gathers information to identify strengths, as well as potential barriers to health. This information ultimately helps develop family-centered strategies for support and guidance.

This family health assessment is a two-part assignment. The information you gather in this initial assignment will be utilized for the second assignment in Topic 3.

Develop an interview questionnaire to be used in a family-focused functional assessment. The questionnaire must include three open-ended, family-focused questions to assess functional health patterns for each of the following:

Include the following in your paper:

  1. Describe the family structure. Include individuals and any relevant attributes defining the family composition, race/ethnicity, social class, spirituality, and environment.
  2. Summarize the overall health behaviors of the family. Describe the current health of the family.
  3. Based on your findings, describe at least two of the functional health pattern strengths noted in the findings. Discuss three areas in which health problems or barriers to health were identified.
  4. Describe how family systems theory can be applied to solicit changes in family members that, in turn, initiate positive changes to the overall family functions over time.

It moves beyond a focus on individual behavior toward a wide range of social and environmental interventions. The concept of “health” is no longer viewed in black and white thinking as a disease free individual vs. diseased individual. Health used to be viewed as remaining free from signs and symptoms of infectious diseases. Now, it has become more complex, taking an interest in each individuals state of well-being regardless of having a disease process or not. Health is now viewed in a holistic manner including the mental, emotional, spiritual and physical elements of ones being. Keeping one’s elements in balance through health promotion, disease prevention and risk reduction have now become the focus of health care. Illness may occur if one of these elements is thrown off. However, if someone has a diagnosed disease, that does not always mean they are ill or not “healthy”. If someone has a chronic illness such as, Diabetes but it is well managed, blood sugars remain stable, and the individual carriers out their activities of daily living with no complications and this person is happy with themselves then this person might consider themselves to be in their personal optimal state of health. Health promotion overall, has shifted from educating patients on the symptoms of an already existing condition and treating those symptoms but to that of an entire preventative approach. Nurses are an instrumental tool in promoting healthy living, identifying risk factors for individuals and educating patients on steps they can take to care for themselves and live their best lives disease free. With health care and medical technology rapidly changing, it is crucial that that nurses provide their patients with the leading evidenced-based-practices to produce the safest and best possible outcomes.

Healthcare has shifted from improving sanitation, to disease prevention, to health promotion. In “…the early 19th century, infectious diseases were widespread and difficult to contain largely because of the lack of proper sanitation methods, including insufficient disinfection of drinking water and improper removal of human waste. During this time, health was viewed in basic terms as being free of disease” (Falkner, 2018). However, there has been a shift in the definition of health, especially with healthcare focusing on improving access to primary and preventative health services. “The goal is to create a culture of health in which health promotion and disease prevention is the focus rather than seeking treatment after disease has set in” (Falkner, 2018). Nurses are always thinking about ways to get in front of a problem before a problem gets to a patient, and this is especially important when providing care with the improved vision on health promotion and disease prevention. Nurses should understand the provisions in Healthy People 2020: and nurses are pivotal in educating patients to take control of their health to maintain a healthy state and thus prevent disease. Wellness, which is the “State of well-being that is not necessarily equated with the absence of disease, ability to adapt, and continue to function and live life well are contributing factors to wellness” (Falkner, 2018), goes hand-in-hand with health. The hospital system I work for incorporates these concepts by providing more holistic options to patients and employees. For example, there are mediation stations throughout the hospital. Another example is in the ICU where I work; recently some of the nurses that I work with and me all found a ton of evidence-based research on the benefits of essential oils for critically ill patients. Now we have lavender oil and cardamom/grapefruit oil available on our unit and use them regularly in certain patients. We are currently running our own unit-based project to see if there is a decrease in sedation use when essential oils are on board. While it is crucial for nurses to continue to use evidence-based practice, it is exciting to be able to incorporate more holistic/wellness modalities in delivery of nursing care; especially because these more wellness-based modalities are becoming increasing studied and accepted in evidence-based practice.

Traditionally, health was defined as the absence of illness with great emphasize on the role of clinical diagnosis and intervention. Early 1990s, the main cause of death were infectious diseases such as pneumonia, caused by viruses and bacteria. Therefore, the definition of health during the 1990s was based on freedom from illness. Due to medical advancement over the years, infectious diseases can be cured and prevented. The ensuing discussion describes the changes in the concept of health. With the control of infectious diseases, in modern days the main cause of poor health is life-styles (Polsen & Borui, 2015). Therefore, the concept of heath has shifted its focus from illness to wellness, a positive broad health approach that includes intellectual, social, physical, and emotional well-being. Wellness refers to putting more effort to live your life to the fullest potential and conducting activities that prevent diseases. Overall wellness is an active process of change to enhance complete physical, social, and mental wellbeing. Traditionally, health was considered the opposite of illness, therefore, people were concerned with treating diseases rather than promoting health. World Health Organization changed the view of health from disease to a full life perspective. Thus, health promotion has become a priority in the modern society. Currently, health promotion encompasses efforts to promote health while preventing injuries and diseases through education, good working conditions, and policies that facilitate a healthy lifestyle. Nurses need to implement health interventions through evidence-based practices because these practices foster for efficient, effective and safeness in the health sector. Nurse’s aims to improve caregiving and enhance high quality intended patient’s outcomes through evidence-based practice (Steven, 2013). In conclusion, the concept of health and health promotion has transformed over time due to the changes in the predisposing conditions. Traditionally, the concept of health was concerned with treating illness, however, this increased the incidence of infectious diseases. Due to the high incidence of infectious diseases, the concept of health changed into the state of well-being and promoting health.
