To be professionally accountable in nursing is necessary and vital for patient care and safety. According to The American Nurses Association Code of Ethics, the definition of accountability “ is to be answerable to oneself and others for one’s own actions.”
The nurse has to ensure that while delivering care, accountability is owed to themselves and the client, the client’s family, colleagues, the workplace and the profession.
An example of professional accountability is during the admission process of a client to a psychiatric unit for the first time. The client is involuntary and is not sure what this status means for them. With professional accountability, the nurse explains the involuntary status and also ensures that the family understands what this status is in a psychiatric facility. Due to the fact that the hospital is a teaching facility and nursing students are always present, each opportunity must be seen as a teaching moment, so nursing students can understand how professional accountability impacts the care of clients from the time of admission.
A senior nurse knows that establishing a therapeutic relationship allows the client to feel safe and being professionally accountable impacts both client and student especially during the assessment phase. Here the nursing process is key to clinical expertise and using the nursing process demonstrates that quality of care must start from the point of admission to discharge. This nursing process is kept in mind by a highly educated nurse who also uses evidence-based practice to deliver safe and effective care.
Evidence-based practice in professional accountability allows the nurse to have a better understanding of the practice and how accountability influences her to make decisions that fully impacts patient care.(Brower & Nemec,2017) An example of evidence-based practice in mental health is to form a client-centered plan of care to ensure all decisions are shared-decision making. The care team and the client seeking help with mental health issues come together to have client-centered care and self-directed care.
With accountability to the client and self, the nurse is an advocate for the client and ensures that the latest evidence is used to drive change for better patient outcomes. ( Brower & Nemec, 2017).
Professional Accountability is one of the best ways for a nurse to self regulate, be conscious of the care and their impact when imparting knowledge to the future generation of nurses.

Professional organizations are vital to nursing practice. They represent nurses in government at the national and state level. They are a great source of information regarding new research and changes made to current nursing practice. (Helbig, 2018) Evidence-based practice is driving nursing care by substantiating what we do; professional nursing organizations disseminate this information to practioners through nursing journals and periodicals.
Professional nursing organizations are advocates for nurses. They give nurses a voice in the public eye. On Tuesday, April 16, 2019 the Washington State Senate was considering a bill, SHB 1155. This bill supports providing nurses with uninterrupted breaks (rest periods). Republican senator Maureen Walsh during the discussion of this bill stated, “I would submit to you that those (small hospital) nurses probably do get breaks. They probably play cards for a considerable amount of the day.” Walsh was trying to argue that in these rural areas nurses don’t need their uninterrupted break periods. That it is too costly to provide these rest periods in these circumstances. The professional nursing organization, The Washington State Nurses Association, immediately responded. They called Walsh’s remarks “demeaning” and said there was “zero logic” in her statements. (Stracqualursi, 2019) This is a professional nursing organization defending nursing practice both in government and in the media. This statement made by Senator Walsh created a landslide of backlash on social media. It was the swift and rapid response by the Washington State Nursing Association that was heavy hitting and reputable to combat this horrific argument made by Senator Walsh.
Professional nursing organizations are involved in government, research and education. The American Nurses Association’s (ANA) goal is to support nurses and improve health care standards. This goal is realized through membership and collaboration.

Two days ago, I was involved in the care of an oncology patient. She was no longer able to speak and was becoming vividly altered in mentation. She had been alert and oriented in the previous week, but at the shift report on this day, I was informed that she had climbed out of bed and fallen. When I got to her, I discovered that her condition was becoming more acute and she was hallucinating and declining fast. I made the Charge nurse aware of her increasing anxiety and inability to stay in bed, requesting for a sitter to keep her safe, I was told that we had no resources and that I should consider using a posey restraint or ativan to settle her. My lady is 91 years old with acute alteration in the mental status due to her disease process, the last thing I wanted to do was to increase her anxiety with the posey, I did not want to end her life either by giving her ativan. She was severly anorexic and receiving PPN due to her NPO status. I went and stayed in her room and attempted to run to my other patients from there. I was finally able to consult with the shift coordinator who subsequently provided me with a sitter for this patient. She lasted the night with a 1:1 sitter, without exotic drugs and received the care she needed.

In my experience as a labor and delivery nurse I have had several opportunities to advocate for my patients. In one instance I had a laboring patient who was very adamant that she did not want an epidural. She wanted to experience a labor with minimal interventions. When her physician came to evaluate her, he was upset that she did not have an epidural. He started to try to intimidate and scare her into getting one.There was no medical necessity for it and she looked at me and started to cry. I verbalized to the physician that she did not want an epidural and I was going to be sure that whatever her wishes were, we were going to respect them, no matter how inconvenient he felt it was for him, as long as there was no medical contraindication. My patient delivered with no epidural and was so thankful and appreciative that I helped her not get bullied into an unnecessary procedure when there was no medical indication for it.

Nurses are responsible for delivering optimal patient centered care, and in order to so, nurses must advocate for their patients, especially patients that cannot advocate for themselves. The article “Critical Care: The Role of Nurses as Patient Advocates,” states that a nurse’s role in being a patient advocate includes being communicators, liaisons, educators, interpretors, and caregivers (“Critical Care: The Role of Nurses as Patient Advocates,” 2016). Not too long ago I was taking care of an older patient who did not have a cardiac history, and their pulse kept dipping down into the high 20’s. I contacted the patient’s attending doctor three times to show my concern, but the attending just kept saying to continue to monitor the patient. The patient seemed worried as well, and didn’t feel like they were getting the proper attention. Fed up with the doctor not willing to examine the patient, I called an RRT which forced the attending doctor and other members of the interdisciplinary team to examine the patient. After the RRT, it was determined that patient needed closer monitoring and they were sent to the PCU. The doctor seemed annoyed with my decision, but the nursing supervisor and RRT nurse commended my efforts for advocating for the patient. Had I not initiated the RRT, the patient could have possibly experienced cardiac arrest and/or loss of heart function.

However, my goal and passion has always been working as an L&D nurse. I found it challenging to fulfill my passion and goal since the hospitals I was looking to work at required a BSN. I have had many co-workers and former classmates complete their RN-BSN program through GCU; all have had wonderful things to say about the program and saw their careers soar. With their encouragement and time needed to save financially, I have decided that I needed to take the next steps in obtaining my BSN.
This is my first online class/program, so I am a little worried and concern about the structure of the program and expectations. I am also concerned with my ability to stay on task and complete everything I need to be successful in a timely fashion. But after reading 7 Habits of Highly Effective Nursing Students (Moore, 2015) and Balancing Work and Nursing School (All Nursing Schools, n.d.), I will create a working space for me to complete my school work, a calendar for assignments and proactively stay ahead of schedule.
I know that as long as follow Strategy #8-Focus on Your Nursing Career Goal (All Nursing Schools, n.d.) and have my goal and passion align, I will stay on track and be successful.
