End of life issues. 

With our aging population living longer with, in many cases, multiple chronic diseases, it is important we feel confident identifying and addressing end of life challenges and expectations.

Choose 1 focal point from each subcategory of practice, education, research and administration and describe how the APRN can provide effective care in end of life management, using the American nurses association position statement, recommendations for improvement in end of life management focuses on practice, education, research and administration. Listed below are steps that nurses can take to overcome barriers in healthcare practice.

Practice

1. Strive to attain a standard of primary palliative care so that all health care providers have basic knowledge of palliative nursing to improve the care of patients and families.

2. All nurses will have basic skills in recognizing and managing symptoms, including pain, dyspnea, nausea, constipation, and others.

3. Nurses will be comfortable having discussions about death and will collaborate with the care teams to ensure that patients and families have current and accurate information about the possibility or probability of a patient’s impending death.

4. Encourage patient and family participation in health care decision-making, including the use of advance directives in which both patient preferences and surrogates are identified.

Education

1. Those who practice in secondary or tertiary palliative care will have specialist education and certification.

2. Institutions and schools of nursing will integrate precepts of primary palliative care into curricula.

3. Basic and specialist End-of-Life Nursing Education Consortium (ELNEC) resources will be available.

4. Advocate for additional education in academic programs and work settings related to palliative care, including symptom management, supported decision-making, and end-of-life care, focusing on patients and families.

Research

1. Increase the integration of evidence-based care across the dimensions of end-of-life care.

2. Develop best practices for quality care across the dimensions of end-of-life care, including the physical, psychological, spiritual, and interpersonal.

3. Support the use of evidence-based and ethical care, and support decision-making for care at the end of life.

4. Develop best practices to measure the quality and effectiveness of the counseling and interdisciplinary care patients and families receive regarding end-of-life decision-making and treatments.

5. Support research that examines the relationship of patient and family satisfaction and their utilization of health care resources in end-of-life care choices.

Administration

1. Promote work environments in which the standards for excellent care extend through the patient’s death and into post-death care for families.

2. Encourage facilities and institutions to support the clinical competence and professional development that will help nurses provide excellent, dignified, and compassionate end-of-life care.

3. Work toward a standard of palliative care available to patients and families from the time of diagnosis of a serious illness or an injury.

4. Support the development and integration of palliative care services for all in- and outpatients and their families.

The advancement of elderly case has been determined to encompass assistance that is able and informed on the diseases prone to such age groups. It is crucial to determine the challenges and expectations required to efficiently perform those duties. This can only be properly accomplished with the right set of information accompanying the act and practice itself. It creates the need for research to accurately as possible find out what proper care can be applied. 

This can be accomplished by integration of the evidence obtained throughout all the sectors and levels concerned with that healthcare plan. This will allow seamless treatment and end of life care as it has properly seen and enforced expected conditions. This will lead to the development of newer methods that may be deemed efficient to handle such circumstances. This will lead to the promotion of the best quality practices across the dimensions and will include the assessment of physical, psychological, spiritual and interpersonal practices. It will ensure the use of the best to provide care.

In regard to decision making, the use of evidence based ethical practices will be used to avoid mistreatment of certain elderly patients. As such research ought to indicate their best interests in ensuring which manner of care being used is deemed ethical or respectable to the senior citizens. Support decision making is also encouraged to ensure their participation in whatever decision that they may come across.

In light of ethics, a measure of best practices should be placed to show quality and effectiveness of the care being provided and whether family is involved in treatment and decision making. It should also indicate the satisfaction rate of the patients, their families in the use of healthcare amenities to provide proper end-of-line care. Such research should be given the maximum support by the various stakeholders and beneficiaries of such programs for proper utilization.

As a nurse, one is required to note that the reality of a person’s death is something that is difficult for both the patients and their families. As a nurse, one is needed to be in a position to offer guidance to these people. In this case, he or she should be comfortable discussing about death and offer appropriate comfort to the families. In addition, the nurse should collaborate with other professionals to ensure that the most accurate information is provided. In the subcategory of education, the focal point to consider is number 1. Nurses are required to offer emotional, physical, spiritual, and practical care for patients in each phase of life (Jors et al., 2016). They spend more time with patients nearing end of life in comparison to other health professionals. For this reason, they are required to receive the training and practical experience needed to conduct the duties in the best way possible. In the research subcategory, the focal point to consider is number 1. In this case, there has been a change in medicine towards evidence based medicine. The field of palliative care covers a group of various ailments. Hence, the future of evidence based medicine needs to be diverse to make sure that all patients are able to benefit as much as possible. Lastly, in the administration subcategory, the focal point to be considered is number 1. In most cases, palliative care takes place up to the time when the patient dies. Afterwards, the process comes to an abrupt end. However, it is important to consider that the family members still require help in coping with the demise. For this reason, there should be strategies of ensuring that care is extended to family members after death.

           Nursing practice, education, and research are important in managing patients at the end of life. American nurses association position statement is aimed at providing direction for nurses to help in supporting patients as well as the families in acknowledging and adapting to the patient’s impending death. Nurses plays crucial role in determining the symptoms of the patients, taking measurements within their scopes to help in the administration of the drugs, offering other measures for the reduction of symptoms, and working together with other healthcare providers to improve patients’ comfort as well as the understanding and adaptation of the families (American Nurses Association (ANA), 2016).

           While caring for patients at the end of life care, nurses are required to be compassionate through recognizing that death is near and to convey such information to the families. Therefore to better their practices while managing care to the patients in the end life, nurses have to strive to achieve the standards of primary palliative care so that healthcare professionals possess the basic knowledge of palliative nursing to help in the improvement of care and families. In this case, nurses together with other healthcare professionals must make an effort to ensure that there is an expansion in palliative care services. It is important to ensure that there is an improvement in the healthcare quality at the end of life so that there will be no need for physician-assisted suicide (Ding, Saunders, Cook, & Johnson, 2019).

           Concerning education, individuals involved in the practice of secondary or tertiary palliative care will have special education and certification. Through an expansion of palliative care services, nurses and healthcare professionals have to develop perfect post-graduate healthcare training to help them have basic skills required for palliative care. There are some principles of palliative care for example those which involve the enhancement of the symptom evaluation and management, using the goals of the client and family vs. the problem-focused care as directive; being well informed about prognosis; delivery of the message with sympathy and understanding; and the process of acknowledging the spiritual, psychosocial, and cultural aspects of caring for patients with severe injuries. Therefore, it is important as a nurse to ensure that learning or training on these principles is well covered to improve the palliative healthcare services (Thomson, Faahpm, & Patel, 2013).

           About research, it is important to integrate evidence-based care across a dimension of end of life care. Research work is important in finding more effective types of palliative care. Through research work, management of the case becomes easier since it is associated with a clear understanding of the end of life choices and the methods of communicating the choices to the patients and their families (Ding, Saunders, Cook, & Johnson, 2019). Concerning administration, it is crucial to work towards the standards of palliative care available to the patients and the families from diagnosis of severe illness or injury. The issue of caring for a patient towards end of life requires interdisciplinary specialty as well as effective decision making with includes even families to help in improving the well-being of the persons with severe sickness. Through the use of support from all levels of the healthcare system through effective communication skills regarding major concerns, it becomes easier to overcome the challenges associated with meeting the standards of palliative care (Kelley, 2015)

The United States Preventive Services Task Force (USPSTF) is an independent panel of experts in primary care and prevention that systematically reviews the evidence of effectiveness and develops recommendations for clinical preventive services. The task force, a panel of primary care physicians and epidemiologists, is funded, staffed, and appointed by the U.S. Department of Health and Human Services’ Agency for Healthcare Research and Quality. The USPSTF evaluates scientific evidence to determine whether medical screenings, counseling, and preventive medications work for adults and children who have no symptoms (Kennedy-Malone, Fletcher, & Martin-Plank, 2014, pp. 31-47).
 

Colon cancer screening by colonoscopy, occult blood testing, or sigmoidoscopy in adults 50 to 75. Colorectal cancer is common and preventable in adults >50 years. The USPSTF recommends
screening for colorectal cancer using fecal occult blood testing, sigmoidoscopy, or
colonoscopy, in adults, beginning at age 50 years and continuing until age 75 years. The risks and benefits of these screening methods vary (Kennedy-Malone, Fletcher, & Martin-Plank, 2014, pp. 31-47).
