Family Health Assessment Part I

Understanding family structure and style is essential to patient and family care. Conducting a family interview and needs assessment gathers information to identify strengths, as well as potential barriers to health. This information ultimately helps develop family-centered strategies for support and guidance.

This family health assessment is a two-part assignment. The information you gather in this initial assignment will be utilized for the second assignment in Topic 3.

Develop an interview questionnaire to be used in a family-focused functional assessment. The questionnaire must include three open-ended, family-focused questions to assess functional health patterns for each of the following:

  1. Values/Health Perception
  2. Nutrition
  3. Sleep/Rest
  4. Elimination
  5. Activity/Exercise
  6. Cognitive
  7. Sensory-Perception
  8. Self-Perception
  9. Role Relationship
  10. Sexuality
  11. Coping

Select a family, other than your own, and seek permission from the family to conduct an interview. Utilize the interview questions complied in your interview questionnaire to conduct a family-focused functional assessment. Document the responses as you conduct the interview.

Upon completion of the interview, write a 750-1,000-word paper. Analyze your assessment findings. Submit your questionnaire as an appendix with your assignment.

Include the following in your paper:

  1. Describe the family structure. Include individuals and any relevant attributes defining the family composition, race/ethnicity, social class, spirituality, and environment.
  2. Summarize the overall health behaviors of the family. Describe the current health of the family.
  3. Based on your findings, describe at least two of the functional health pattern strengths noted in the findings. Discuss three areas in which health problems or barriers to health were identified.
  4. Describe how family systems theory can be applied to solicit changes in family members that, in turn, initiate positive changes to the overall family functions over time.

Cite at least three peer-reviewed or scholarly sources to complete this assignment. Sources should be published within the last 5 years and appropriate for the assignment criteria.

“According to the World Health Organization (n.d.a), health promotion is the process of enabling people to increase control over, and to improve, their health. It moves beyond a focus on individual behavior toward a wide range of social and environmental interventions. The concept of “health” is no longer viewed in black and white thinking as a disease free individual vs. diseased individual. Health used to be viewed as remaining free from signs and symptoms of infectious diseases. Now, it has become more complex, taking an interest in each individuals state of well-being regardless of having a disease process or not. Health is now viewed in a holistic manner including the mental, emotional, spiritual and physical elements of ones being. Keeping one’s elements in balance through health promotion, disease prevention and risk reduction have now become the focus of health care. Illness may occur if one of these elements is thrown off. However, if someone has a diagnosed disease, that does not always mean they are ill or not “healthy”. If someone has a chronic illness such as, Diabetes but it is well managed, blood sugars remain stable, and the individual carriers out their activities of daily living with no complications and this person is happy with themselves then this person might consider themselves to be in their personal optimal state of health. Health promotion overall, has shifted from educating patients on the symptoms of an already existing condition and treating those symptoms but to that of an entire preventative approach. Nurses are an instrumental tool in promoting healthy living, identifying risk factors for individuals and educating patients on steps they can take to care for themselves and live their best lives disease free. With health care and medical technology rapidly changing, it is crucial that that nurses provide their patients with the leading evidenced-based-practices to produce the safest and best possible outcomes.

Healthcare has shifted from improving sanitation, to disease prevention, to health promotion. In “…the early 19th century, infectious diseases were widespread and difficult to contain largely because of the lack of proper sanitation methods, including insufficient disinfection of drinking water and improper removal of human waste. During this time, health was viewed in basic terms as being free of disease” (Falkner, 2018). However, there has been a shift in the definition of health, especially with healthcare focusing on improving access to primary and preventative health services. “The goal is to create a culture of health in which health promotion and disease prevention is the focus rather than seeking treatment after disease has set in” (Falkner, 2018). Nurses are always thinking about ways to get in front of a problem before a problem gets to a patient, and this is especially important when providing care with the improved vision on health promotion and disease prevention. Nurses should understand the provisions in Healthy People 2020: and nurses are pivotal in educating patients to take control of their health to maintain a healthy state and thus prevent disease. Wellness, which is the “State of well-being that is not necessarily equated with the absence of disease, ability to adapt, and continue to function and live life well are contributing factors to wellness” (Falkner, 2018), goes hand-in-hand with health. The hospital system I work for incorporates these concepts by providing more holistic options to patients and employees. For example, there are mediation stations throughout the hospital. Another example is in the ICU where I work; recently some of the nurses that I work with and me all found a ton of evidence-based research on the benefits of essential oils for critically ill patients. Now we have lavender oil and cardamom/grapefruit oil available on our unit and use them regularly in certain patients. We are currently running our own unit-based project to see if there is a decrease in sedation use when essential oils are on board. While it is crucial for nurses to continue to use evidence-based practice, it is exciting to be able to incorporate more holistic/wellness modalities in delivery of nursing care; especially because these more wellness-based modalities are becoming increasing studied and accepted in evidence-based practice.

Traditionally, health was defined as the absence of illness with great emphasize on the role of clinical diagnosis and intervention. Early 1990s, the main cause of death were infectious diseases such as pneumonia, caused by viruses and bacteria. Therefore, the definition of health during the 1990s was based on freedom from illness. Due to medical advancement over the years, infectious diseases can be cured and prevented. The ensuing discussion describes the changes in the concept of health. With the control of infectious diseases, in modern days the main cause of poor health is life-styles (Polsen & Borui, 2015). Therefore, the concept of heath has shifted its focus from illness to wellness, a positive broad health approach that includes intellectual, social, physical, and emotional well-being. Wellness refers to putting more effort to live your life to the fullest potential and conducting activities that prevent diseases. Overall wellness is an active process of change to enhance complete physical, social, and mental wellbeing. Traditionally, health was considered the opposite of illness, therefore, people were concerned with treating diseases rather than promoting health. World Health Organization changed the view of health from disease to a full life perspective. Thus, health promotion has become a priority in the modern society. Currently, health promotion encompasses efforts to promote health while preventing injuries and diseases through education, good working conditions, and policies that facilitate a healthy lifestyle. Nurses need to implement health interventions through evidence-based practices because these practices foster for efficient, effective and safeness in the health sector. Nurse’s aims to improve caregiving and enhance high quality intended patient’s outcomes through evidence-based practice (Steven, 2013). In conclusion, the concept of health and health promotion has transformed over time due to the changes in the predisposing conditions. Traditionally, the concept of health was concerned with treating illness, however, this increased the incidence of infectious diseases. Due to the high incidence of infectious diseases, the concept of health changed into the state of well-being and promoting health.

The three levels of health promotion are easy to explain. Primary health promotion is going to be any type of preventative methods. For example, immunizations of children or the yearly flu shot. This is preventing the sickness and allowing the continuance of wellness within the person. Secondary health promotion is through health screenings and early treatment of diseases. For example, a patient goes into a walk-in clinic to be seen for flu-like symptoms. The walk-in clinic itself is considered a secondary health facility because they do minor health screenings and can prevent the worsening of certain diseases. Lastly, the tertiary health promotion according to Grand Canyon University is, “is to help the patient achieve some semblance of normalcy and acclimate back into their lives and society.” (Falkner, “Health Promotion in Nursing Care”, 2018). Tertiary facilities include rehabs, hospitals, or even hospice care centers. They provide care to manage symptoms and promote the patient to try to get back to normal life and become well again. The levels of prevention can determine how much education a person or patient needs because each prevention level determines where the patient is within their healthcare journey. It also determines how much lifestyle changes may need to be made to become functional within society again.
