Advanced nursing practice is an essential component in the overall nursing and health care fraternity. The concepts used have been changing and are still changing over time. As there was a need and passion for improving on my skills and carrier as a nurse, I saw a need to join the South University for the advanced program in which I wanted to specialize in clinical nursing. As a clinical nurse specialist, I have been struggling in adaptation to meet challenges and the changing requirements of families. My main work as a clinical nurse specialist revolves around patient’s well-being and treatment.

In a hospital setting, there is an assignment that needs to be dealt with by clinical nurses, and others require non –clinical persons to ensure the safety of the patients in the access of health care services. There are roles to be played by managers who are not necessarily clinical officers. The managers have a moral and legal obligation in ensuring the patient’s care is of standards, and in the case of mediocre quality, they tend to improve them. The managers are in a position to dictate and authorize systems, policies, and the climate around the institution. The role of a clinical nurse about the role of (ANP) Adult nursing practitioners is that they order and perform routine screenings and accuracy in diagnostic of diseases and other illnesses to patients. Check-Ups to patients above 12 years old. They are involved in ordering laboratory tests and also participated in the determination of the appropriate form of treatment for the various diagnosis. The role of the two helps a vital role in the promotion of the patient’s outcome and safety as ordering laboratory tests a nurse diagnoses the ailment while the action of a hospital manager is to ensure the correct and right quality of service to the patients and efficient management.

An ANP role is that they focus mainly on adults from age 12 and above. The nurses are involved in dealing with chronic and acute illnesses (To practicing nurses to qualify as an ANP, passing in NCLEX-RN examination is mandatory for a certified registered nurse, while on top of that, there is a need to have experience of around and above two years. Concepts related to accreditation by the Commission on Collegiate Nursing Education (CCNE) or ACEN. This concept applies to people seeking masters, doctoral or postgraduate program (Sarah B, Keating; EdD; MPH; RN; C-PNP; FAAN & Stephanie S. DeBoor; APRN; ACNS-BC; CCRN,2017). After earning a certificate in adult nursing practitioners, the license must be renewed every five years. Most of the concepts used in advanced nursing practitioners are almost the same as those used in nursing during the analysis of diagnosis, treatment, chemical formulas of medicines, and body reactions to certain products towards certain drugs for medication. For specialty reasons, one can take the midwife field or registered nurse. I viewed the documentary titled “The Invisible Patients documentary” and an ethical dilemma that was experienced by the nurse practitioner is the decision about hospice care. The patient has a right to decide how they want their life to end in regard to resuscitation, and it is one of the nurse practitioner duties to educate the patient on what a do not resuscitate (DNR) means and the  accept their terminal prognosis or to have do-not-resuscitate (DNR) order (Bhatnaga & Lagnese, 2019). Patients should be allowed to receive all the care needed until the end of their life. The nurse practitioner in the video appears to be giving her personal views and wanting to listen to the patient and his wishes. Roger verbalizes his desire to live even on a respirator and it is not the duty of the nurse practitioner to discourage the patient from the right to desire life.

The nurse practitioner in the documentary as a disruptive innovator was her desire of delivering and have more nurses in primary care because they reach patients that are in unfortunate situations and needs the help of the nurse practitioner to provide care and advocate on their behalf whether by calling doctors or coordinating care on their behalf. An awareness of personal values generates more consistent choices and behaviors; it can also assist APNs to be aware of the boundaries of their personal and professional values so that they can recognize when their own positions may be unduly influencing patient and family decision making (Hamric, 2014). The nurse practitioner should separate their personal beliefs from situations that where an ethical dilemma is involved and follow the nurses code of conduct to make appropriate decisions.

Overcoming barriers when faced with ethical dilemma is being an active listener with good communication skills. We must listen to the patients concern and wishes and communicate with them at a level that is understandable and open to answer questions with clarity.

Week 3 – Ethical Dilemma

Please reply to the following discussion with one reference. 

Many ethical dilemmas are illustrated in the documentary The Invisible Patients.  The documentary follows Jessica, a nurse practitioner, as she confronts the challenges of caring for homebound patients.  One of her patients is Roger, a 30 year-old male with muscular dystrophy who is nearing end of life.

Jessica is faced with navigating the ethical issues pertaining to establishing a “do not resuscitate” order (DNR).  In the beginning of the film Roger is hesitant to sign a DNR.  He states that he is not ready to die and desires all available interventions that might allow him to continue living, even if that means “bringing him back.” 

Jessica feels that resuscitation would be a painful prolongation of the inevitable.  This stirs internal conflict for her.  She is faced with two opposing possible courses of action.  According to Hamric, an ethical dilemma occurs when a person is faced with two or more obligations that cannot be simultaneously met (2014).  This creates tension due to the differing and opposing demands of these obligations (Hamric, Hanson, Tracy, O’Grady, 2014)  Jessica has undoubtedly seen first-hand the violent and painful process of cardiopulmonary resuscitation, and she obviously knows that these efforts are usually futile, only resulting in unnecessary pain and suffering.  On the other hand, she is duty bound to respect the patient’s autonomy and his personal wishes. 

Jessica handles this situation very professionally.  She gently communicates to Roger and his family the reality of the progressive nature of Roger’s disease.  She tries to put aside her own emotions during the discussion.  Jessica asks open ended questions and empathetically listens to his responses, giving him positive, affirming non-verbal feedback.  In doing this, she is gathering information to help her navigate this ethical dilemma.   This end-of-life discussion was carried out subtly, tactfully, and at length over multiple visits to the patient in his own home. 

These home visits are themselves an example of the NP as a disruptive innovator.  Traditional medicine as practiced today takes place in a clinical setting or a tertiary care center (Hamric, Hanson, Tracy, O’Grady, 2014).  Jessica provides a service that is outside of the status quo by delivering superb patient-centered primary care in the patient’s own home.

I feel I would handle this situation in the same manner.  I believe that it is a fundamental right for people facing terminal illness to decide their own fate.  However, I also believe that fear of death and the unknown that follows too often drives these decisions.  I believe my role is to reassure, encourage, and help relieve this fear so the patient can feel at peace with a decision that reflects the reality of their situation.
