            The organization that I had the pleasure of doing my Capstone practicum at was the Broward College Health Sciences Simulation Center (HSSC). The mission of this organization is to provide quality, interprofessional, experiential learning, using the best high-fidelity healthcare simulation practices. The HSSC focuses in preparing students and other healthcare professionals to be confident, compassionate, and competent in their field. This organization provides a safe and controlled environment where the faculty can offer guidance. They practice using tools, such as SBAR, that will be used in the real-world so there is a feel of better preparation.

            Healthcare system is an organization of people and resources to deliver services to meet the health needs of a targeted populations (Pina et al., 2015). This simulation center helps prepare the future healthcare professionals by guiding them through scenarios that they may come across in the field. It is such a benefit for students to come to this organization to utilize the equipment and gain knowledge and skills. Since the mission is to prepare healthcare students, while I was completing my Capstone I was able to see the relationship between the faculty and students which showed professionalism and direction. They were honoring their mission and philosophy which is to prepare these students to be safe health professionals.

Regulation

      The healthcare profession has numerous regulatory bodies ensuring patient safety is maintained at a high standard.  One such organization is The Joint Commission (TJC), an organization established in 1951 that provides voluntary accreditation through a very in-depth evaluation of compliance with standards of care of an organization (Wrzesniewski, 2017).  Continued accreditation establishes an organization’s determination to provide evidence-based, safe, quality care.  In 2001 TJC introduced a new requirement of a formal patient safety program (Huber, 2018).  The continuously elevated level of standards and safety measures required by TJC ensures safety is always at the forefront of health care.  Although medication administration error goals can be to keep errors to a minimum, it is impossible to avoid errors where the human element exists (Wang et al., 2015).  Hospitals can establish the building blocks needed to provide care of the highest standards through the safety framework required by TJC accreditation.  However, to assure continued safety and compliance it is important as a hospital culture to maintain standards and not push staff compliance before inspections (Wrzesniewski, 2017).  The standard of care and safety measures should be instilled in healthcare workers.  Policies and procedures should be adhered to and followed to the letter to avoid staff confusion and straying from the goal of safety.  Safety in an organization is a culture and must be preserved at its highest form. 

As nurses, we follow organizational policies and procedures, question when necessary, and advocate for change when applicable. Nurses are the last line of defense in medical errors.  This responsibility should not be taken lightly and a conscious continuous effort should be made to apply evidence-based practice when providing care to patients.  Patient safety should be the number one priority in any health care and to any given healthcare provider as we have all taken an oath to above all not do any harm to our patients.

 The relationship between national and global health and nursing leadership is extremely important. Health equity is essential for people to have the opportunity to live productive and healthy lives and achieve their full potential (Huber, 2018). Sadly, in the global and national health arenas, this is not so, and there are grave disparities in access to healthcare. The nurse leader has a duty and responsibility to advocate in his or her area of expertise. It is imperative that nurse and healthcare leaders address these disparities at not only on a national level but also on a global one Huber, 2018).

       For instance, on a national level, one example is the undocumented population. Health disparities often occur due to lack of health insurance and access to healthcare (Huber, 2018). In the United States alone there are more than 12 million individuals without access to healthcare, many with chronic or long-term ailments (Sorrell, 2017). Lack of decent health coverage is an ethical issue because adequate healthcare is a basic human right and need (Sorrell, 2017). Healthcare should be fair, safe and impartial, and should improve the health of the population (Huber, 2018). One of the drawbacks is that healthcare and nursing leaders have lack of guidance concerning the authorization and delivery of healthcare to this population (Sorrell, 2017).

       How should the nursing leader advocate for these immigrants?  Nurse leaders and other healthcare providers need to publicly recommend and support policies that provide lasting and impartial health provisions for these individuals (Sorrell, 2017).  Nurse leaders need to join with lawmakers and organizations such as the Center on Health Equity Action for System Transformation or the Health Equity Task Force to help develop a health equity centered system (Families USA, 2019).

  Mr. Jones’ current non-compliance with treatment appears to be due to a disturbed body image and lack of education regarding the aging process. Although development of arthritis is not a normal change of aging, it is something that may happen due to the natural alterations in the musculoskeletal system (Tabloski, 2014). Bones become stiff, weaker and more brittle as people age and vertebral disc thinning is likely (Tabloski, 2014) . Osteoarthritis affects over 50% of individuals over age 65, restricting self-care ability – even more so when the recommended treatment regimen is not followed (Tabloski, 2014) . The nurse caring for Mr. Jones should continue to educate him on the risks of non-compliance such as falls and hip fracture. Talking to Mr. Jones about alternatives to pain medication (such as lifestyle changes like exercise, weight loss and healthy diet) may prove to be helpful in this situation. Mr. Jones believes that if he uses a cane to get around, he will be seen as undesirable to women and that is affecting his willingness to comply with his treatment. Reassigning the patient to (or collaborating with) a younger female nurse who is empathetic to how he feels may be helpful and he may decide to listen to her advice. The nurse could also collaborate with the physical therapist to teach Mr. Jones some exercises that he can do in the privacy of his home since he is concerned with his image (Kotter-Grühn, 2015).
