My clinical rotation for this week is preoperation room. I saw many patients the floor was so busy in the morning. One of the case that I saw 64 years female, she came for Esophagogastroduoddenoscopy. She has history of HTN, hypothyroidism. Consent in, she took her medication for hypothoidism today and she is NPO midnight, and she stop asprin 14/09/2019. Her vital signs is TEM 97.2, pulse 79, BP 162/89, respiratory 20. O2 98%. The nurse ask pt about her name date of birth. And she ask about allergy, procedure and who is the doctor, also, when NPO, what medication she took and when the last time. Then she ask the pt to change dress. Then inserte the IV with the fluid. Bed in lower position and call light with pt.  Also take three consent from pt one for procedure one of anthesea and one for blood. Also ask pt to remove the jowlary and glass and dentior. And check the lab result if any abnormality and inform thr surgon and anethesea. Then give hand off report to OR nurse . note in the perioperation no time out only for some surgery. 

My nursing diagnosis is 

Fear and anxiety related to procedure as evidence by pt verbalize she is restless and not sleep two night and her Bp 162/89. 

My goal is 

Patien will able to report decrease anxiety and fear and blood decrease to normal level by 1122on 09/18/2019. 

Intervention:

Explain the procedure to the pt, answer her questions, put her in comfortable position. 

Goal met patent relax and she said feel better and anxiety decrease and I check her BP 128/80) this what i see and you can add more intervention and please follow each suction in rubric

Ineffective tissue perfusion related  to surgical procedure as eviedence by BP 100/67, and patient has dranage on the surgical site. 

My goal patient will verbalized f by 16:30, 09/25/19. 

The intervention: Monitor vital signs. Note color, warmth, capillary refill. Inspect dressing for excess drainage or bleeding or leak of CSF. Monitor intake and out put.

I observe three surgery one was about cerebral angiogram the patien female 78 years old intubation and she had drange 100ml. the second c5-6 arthoplasy. replace a damaged spinal disc in the neck with an artificial disc . he was Male 34 years old, the last one was lumbar spinal fusion female 67 years. I went last week to hospice and at the early morning I attended to nursing meeting they are discussed the patients health care and how are they doing after that I went with my nurse to visit two small town near to my city. we visited nursing home there are two patient in each home assignment for my nurse the nurse take their vital signs and ask the nursing home about their health  and how they feels one of the patient was very sick and he has difficult breathing his extremity cold and pale he is at the end of his life. and he has GI bleeding and rectal bleeding the nurse hospice nurse ask the home nurse to keep him NPO also we went to the home health nursing in Clyde city to visit the patient he was sleeping and the nurse assessed him. also I saw one patient pass away and their family called the nurse while we are in the way and the nurse responded fast and Changed her direction to see this patient first he was diagnosed with end stage of cancer and he has history of COPD and DM and Hypertension and I observed how the nurse did the assessment and support the family of death patient. after that she called the funeral home to take the die patient

the diseases like end stage of renal disease, lung cancer, alzahimer,demintia,….

in the hospice there are interdisplary staff are nurses social worker, aid service, volunteer, chapel faith, pharmacy,

the visit according to patient’s health status some of them need three days on the week some one day 

the nurse took the medications for the patient from hospice before she leave. like pain medication and she took dressing
