  Clinical-community linkages to help connect health care providers, community organizations, and public health agencies so they can improve patients’ access to preventive and chronic care services (Agency for Healthcare Research and Quality, 2013). Local communities having access to hospitals and clinics have a better chance of getting better quality health care. Hospitals having linkage to local communities allow the communities to have better communication with the hospital and they are able to provide health resources. Strategies that improve access to clinical preventive services (such as screening and counseling), community-level activities, and appropriate medical treatment have been shown to reduce and prevent disease in communities (Agency for Healthcare Research and Quality, 2013). There are numerous benefits of having linkage between hospitals and local communities. Communities have opportunities to participate in organizations that can help improve their health.

      There are some effective clinical-community linkages that help improve the health of people residing in the community. Patients get more help in changing unhealthy behaviors, clinicians get help in offering services to patients that they cannot provide themselves, and community programs get help in connecting with clients for whom their services were designed (Agency for Healthcare Research and Quality, 2013). Hospitals are able to provide important resources associated to specific health diseases. In order to provide these health benefits, hospitals are able to communicate with their local communities to help lessen the burden of unwanted health diseases. Furthermore, it is important that hospitals have the linkage with local communities to assess in medical and health concerns each individual may have.

Discussion two,

  Linkage between the community and the hospital, specifically during emergency situations is vital for many reasons including improved access to care services and improved communications during highly stressful situations. The linkage is not only important during these critical situations, but also on a day-to-day basis as hospitals can provide various programs to better influence the overall health and well-being of the community. Without a linkage the hospital will potentially suffer greatly, not only loosing funds, but also credibility when an emergency situation does occur. Additionally, hospitals have resources that are able to benefit the community in normal and crisis conditions.

Discussion three,

  When 9/11 happened I believe it showed how vulnerable our emergency management system was.  The first responders and management team didn’t have an effective communication system in place in order to make key decisions quickly (Khan, 2011). First responders also didn’t have proper equipment or training for such a large scale emergency. I believe the United States had nowhere to go but up after 9/11.

     Response times have improved since Hurricane Katrina, during the time that Hurricane Katrina happened, emergencies that giant weren’t considered a national issue but more of a local issue. Local officials had to ask for help instead of national agencies/government being proactive and lending help first. FEMA used to wait for the problem to become too big of an issue for local agencies to handle before getting involved, FEMA is now quickly engaged early on in the process (Mayer, 2008).
     Hurricane Katrina was unlike anything that the United States has ever seen, 120 mph winds & some areas flooded as high as 27 feet above ground (Mayer, 2008). About 1,300 people died because of Hurricane Katrina and an estimated $96 billion in damages (Mayer, 2008). Communication was poor on all sides, which made things even worse. 

Discussion Four,

  The federal government has improved its response posture since 911. This was an event that changed the manner in which the government approaches the entire concept of response. It has become more effective in response planning. In addition to this there is an emphasis on more of a preemptive approach as opposed to just focusing exclusively on response (Hamner, 2015). This is very important as in enables the government to actively prevent all preventable emergencies and in the instance that these emergencies do materialize respond to them as quickly and as strategically as possible.

The response posture of the federal government has improved since Katrina. It has not improved to a standard that can be deemed as ideal but it has all the same improved. There are contingency plans in place to help people affected by natural disasters. Katrina was so different because it highlighted the ineffectiveness of government agencies at the time to deal with natural emergencies of that caliber.

  How do you approach risk assessment in a hospital?

Risk Assessments are challenging is a hospital environment because there are exposures to a multitude of environments and situations. Hospitals adopt an Incident Command Systems that are specific for their hospital and healthcare situation. The security structures fall under the Incident Command System and included a variety of support staff. Each of the organizations will have to discuss risks and identify a comprehensive risk assessment. Approaching the risk assessment means to rapidly recognize the potential incident escalating into a more severe situation that can jeopardize patient and staff safety (P. 278). Preparing a team is the first step. This will allow for direct communication and promote diversity within the demographics of the hospital’s location. The team from there will identify resource organizations and security staff to manage the risks assigned. 

 Discussion two, 

  Risk assessment creates a way for hospitals identify and manage patient’s safety risks. The goal of risk assessment in a hospital is to limit or eliminate as much patient risk as possible. There are stages for a hospital risk team to follow for proper risk assessment, they are; identifying the hazards, evaluating the associated risks, controlling the risks & re-evaluating. These stages are extremely important when documenting risk assessment and trying to eliminate risks. When risks to a patient are noticed/detected, they need to be reported so they can be documented and assessed. Once assessed preventative measures should be put in place so it doesn’t reoccur. Feedback from patients is also important, sometimes they see more than hospital staff would see. It is important for a hospital to have feedback forms for patients to fill out in case of an at risk moment is noticed and that information goes through the proper chain of command.

  Discussion three, 

  In settings such as a hospital or medical clinic, the public eye would assume that these particular communities are one of the safest buildings to be in during a disaster and you would think nonetheless that it will also have the minimalist in crimes. However, during disasters such as the terrorist attacks in the fall of 2001 to the earthquake in Haiti in early 2010 these mass casualties have caused hospitals to become victims as well and are known to consume their resources and be reduced in providing care to patients. In addition, many other forces have their own security tasks to assure and assess their own individual vulnerabilities, threats, and risks upon identification, which is why nearly every town has a police force. These forces will be of use to any situation they are to come across in the area and to evaluate how to care for these incidents (Reilly, 2011 p 276).  Therefore, by discussing the hospitals security and defense it will be of necessary use in protecting  the physical infrastructure within the medical setting and allows healthcare facilities to continue to provide the resources for patient care and delivery (Reilly, 2011 p 272).

 Discussion Four, 

  Hospital security is important to protect those treating the patients and the patients themselves. People feel safe in a hospital when an incident occurs, and it is very important to maintain the safety level in a hospital to ensure no harm comes to their patients. Almost every town has a police force, and this is crucial to keep their citizens safe, without it, the incidence rates in both the town and hospitals are fairly high. Hospitals need to put risk assessments in place along with active protocols to ensure everyone under their care is safe and sound. In truth, anyone can enter a hospital and make the place unsafe. That is why the book emphasizes on hospital safety and protection, to prevent more incidents from happening (Moran, 2011).

Within the United States, the triage has its purpose in serving within the emergency department (ED) by prioritizing the incoming patients and classifying patients who cannot wait to be seen. In addition, the triage nurse evaluates each patient, considering the safety of how long a patient should wait in the waiting room before being seen. Therefore, patients who are above a higher risk of having an emergency have a shorter wait time, in comparison to patients who are not at a high risk (Agency for Healthcare Research and Quality,2012).

On the other hand, a triage may seem as organized because there are staff involved in assessing who should be seen, however there are many ways a triage can be more effective for patients at high risk and low risk. Being that there are patients coming in and patients going out, the capacity level of the triage can be over than where the capacity level should be. Therefore, the patient flow in the emergency room is very important and by nurses and staff reducing time patients do spend in the ED will help reduce over capacity or overcrowding within the triage. So improving patient satisfaction and reduction, may reduce mortality and morbidity while keeping the patient flow (Jarvis, 2016).

The purpose of triage in the emergency department (ED) is to prioritize incoming patients and to identify those who cannot wait to be seen. The triage nurse performs a brief, focused assessment and assigns the patient a triage acuity level, which is a proxy measure of how long an individual patient can safely wait for a medical screening examination and treatment (Emergency Severity Index Implementation Handbook, 2012). Having an effective triage system is vital in helping those who are in emergency situations. Having implemented policies and strategies to better serve the public is a great way in having an effective triage. Triage nurses who are professionally trained is an important factor and having experience in the field is another factor in providing efficient and accurate service in need. Furthermore, having triage to provide service before coming in to hospitals or clinics is an essential asset in the medical field and gives patients information that can be used in the future.

Vulnerable populations can be described as “Any individual, group, or community whose circumstances create barriers to obtaining or understanding information, or the ability to react as the general population. … Circumstances that may create barriers include, but are not limited to age; physical, mental, emotional, or cognitive status; culture; ethnicity; religion; language; citizenship; geography; or socioeconomic status” (Emergency Preparedness for Vulnerable Populations: People with Special Health-care Needs). Knowing this definition you can see why they would be the ones to suffer in an emergency situation. Many of them have limited mobility which can cause issues when trying to evacuate an area. Those with language barriers may not necessarily understand directions that are being told to them, along with those who are hard of hearing may not be hearing instructions correclty. Furthermore, those with mental disabillities may struggle to grasp the gravity of emergency situations and either refuse to leave, or become violent with first resonders due to fear of the unknown. With the possiblity of all of these issues emergency planning must be done in great detail to ensure the safety of the vulnerable populations, along with the safety of those responding to them.

According to the Minnesota Public Health Emergency Training Series Module on vulnerable populations, “A vulnerable population is any individual or group of people with any physical, mental, emotional, cognitive, cultural, ethnic, socio-economic status, language, or other circumstance that creates barriers to understanding and/or barriers to the ability to act or react in the requested or anticipated manner.” (UM Public Health Staff, 2005). The key to understanding why these vulnerable populations are the ones to suffer most in an emergency incident is in the very definition of what makes them vulnerable in the first place. The fact that these individuals have a condition or lifestyle choice (sometimes not a choice) that prevents them from understanding and/or acting or reacting in the way that is expected is what makes them targets of being the most impacted from an incident to begin with. The best way that I can explain it is with an analogy that I have seen with expectations in a school setting. Imagine a teacher asking 4 students to climb the same tree, the 4 students are a monkey, a cat, a snake, and a fish. Some students simply cannot climb the tree without help. Now, imagine the same scenario, but instead of the teacher asking students, it’s an emergency worker explaining that the only way that your life will be saved is if you climb this tree. In emergency situations, the task being asked is not the problem, it’s a person’s ability to complete the task. There are some portions of every community that have differing abilities that will need help performing the tasks that need to be done.
