QUESTION

Review the four recommendations in the IOM report The Future of Nursing: Leading Change, Advancing Health and find your state’s action coalition. Discuss your state’s action coalition’s work as it aligns with these four recommendations. Is each recommendation being addressed? Based on your analysis of the website, how would you describe your state’s progress toward meeting the IOM recommendations?

ANSWER

The four recommendations of the IOM report include: nurses should practice to the full extent of their education and training, nurses should achieve higher levels of education and training through an improved education system that promotes seamless academic progression, nurses should be full partners, with physicians and other healthcare professionals, in redesigning health care in the United States, and  effective workforce planning and policy making require better data collection and information infrastructure(IOM, 2015). While after researching, there are strides being taken, I do not think that Georgia has met exceeded the goals but progress is being made.  The accomplishments to date include:

· Established a workforce center. Held two doctoral symposiums that have brought students and faculty together to develop skills, present new work, and network. Convened advanced practice nurses (APN) from around the state to discuss the state of APN practice in Georgia and begin plans on how to move forward. Facilitated negotiations with the Secretary of State’s office, the Georgia Board of Nursing, and the National Council of State Boards of Nursing to obtain up-to-date workforce data from the recently-revamped re-licensure survey based on the national minimum data set.

The ACA or the AHCA: Imperfect Solutions to Increasing Access to Health Care?

.

To support your work, use your course and text readings and also use the —————–University Online Library. As in all assignments, cite your sources in your work and provide references for the citations in APA format.

This week’s discussion focuses on the federal government’s role in protecting the public’s health using the ACA and current efforts to “repeal and replace” the ACA. Please note that the government’s role in protecting the public’s health did not begin with the ACA. In the late 1800s, Lilian Wald brought health care to the community. And in 1965, Congress passed bills authorizing Medicare and Medicaid. Since 1965, Congress has expanded government-sponsored health care to pregnant women and children.

Answer the following question:

· Compare the current provisions in the ACA with those in the failed first version of the AHCA. Share your opinions about why the AHCA failed in the first attempt at passage.

Continue to read daily from one of the policy briefs that are included in your weekly readings list. POLITICO Pulse is a daily briefing from POLITICO, and the Kaiser Health News Morning Briefing is a daily briefing from the Kaiser Family Foundation. Both focus on health care. Either one will keep you abreast of what is happening in health care policies and politics.

In addition, subscribe to the “HealthCetera” podcast and listen to the podcast weekly. This is a production of WBAI, free speech radio of Pacifica Foundation Radio in New York City. First Steps on Becoming a Grassroots Lobbyist/Advocate for Health Care Policy

This assignment has two parts, numbered below. Write each question as a new topic area, then follow with a paragraph or two to answer the question. You may find it necessary to search for answers to the questions outside of the assigned reading. Be sure to use APA guidelines for writing style, spelling and grammar, and citation of sources.

Tasks:

1. Imagine that you are going to make a visit to your representative in Congress. Develop a one-page document that supports your position on the AHCA that you would leave with your representative or his or her aide when you make your visit. This one-pager, also called a “leave behind,” should state your position clearly in bullet points and give your reasons why your position is preferred. This “leave behind” should have 1 inch margins and utilize 14-point font throughout. If you were making this visit in person, you would present your “leave behind” to the person with whom you speak during your visit. Include a second page that describes your rationale for the position in your “leave behind” page supported by at least two journal articles.

2. Write a two-page brief to describe the scenario surrounding your legislative visit. Understanding the political affiliation of your representative, include answers to the following:

· Was your member in support or in opposition to the AHCA?

· State three points that you would cover in support or opposition to your representative’s position.

· How does your nursing experience influence the advocacy position that you take on the AHCA?

Submission Details:

· Submit your response in a 4-page Microsoft Word document.

  • How do you judge Palin’s quote? [“And who will suffer the most when they ration care? Effective strategy to oppose Democrats’ plans for health care reform or unethical scaremongering?

Palin’s quote was not only inaccurate, it could be said it was a scare tactic.  For those relying on politicians to define changes, especially as they relate to a lengthy new health care act, I view it as a scare tactic to sway voters against the ACA.  Nowhere in the ACA or AHCA does it enforce a “death panel” (Mason et. al., 2016). This quote from a politician is why it is so important for nurses to understand politics in health care and provide accurate, unbiased, and honest information.

  • Reflect on what informs your judgment: commitment to advance care planning, analysis of facts, and/or political party loyalties?

When it comes to my judgement, research and understanding of the facts are key.  Had I simply listened to Palin, I would not have understood the changes in health care the ACA implemented.  Although far from perfect, this act did increase the number of Americans with health coverage to 20 million people and seeks to provide better health promotion, allowing nurses to practice to the full extent of their education and licensure (Collins, 2018).

  • Is it right for nurses to endorse health reform legislation even if the legislation is not perfect? Does this apply to the recently failed American Health Care Act?

Few things in life are truly perfect.  As a nurse, I have learned you simply cannot make everyone happy and that applies to healthcare.  Nurses should seek to endorse healthcare policies that allow them to practice as nurses; teaching patients to be as independently healthy as possible.  This certainly applies to the failed AHCA.  Again, the debate is not over universal health coverage, but as to who should pay for it.  Nurses have identified the need for access to health care since the beginning of time.  Advocating and endorsing legislative that is supportive of this idea should be the nurses priority.

I believe Palin bringing up the ration of care for the sick, disabled and elderly was very important and a very effective way to grab the attention of many Americans. Not every American has a disable family member nor do they have to per say “deal”or worry with all of the medical and financial burdens these families have to take on. I believe Palin bringing this to many americans attention and speak for the people that are unable to speak for themselves was very effective when it comes to making it known they are also worthy of the care that every other “normal” American is able to receive.

  • Reflect on what informs your judgment: commitment to advance care planning, analysis of facts, and/or political party loyalties?

It is hard to pick one thing that would inform someones judgement, but I would have to say analysis of facts. It is important to always do your own research and not base everything you believe off of what other people are saying. With that being said, I do still agree with Palin and her advocating for the sick, elderly and disabled people of America but doing my own research also taught me more about what Palin was referring to. I came across this statement “The health reform bill being considered in the House of Representatives says that a Comparative Effectiveness Research Center shall “conduct, support, and synthesize research” that looks at “outcomes, effectiveness, and appropriateness of health care services and procedures in order to identify the manner in which diseases, disorders, and other health conditions can most effectively and appropriately be prevented, diagnosed, treated, and managed clinically.” (2009, Politifact).  It was not solely about and nothing to do with evaluating patients for their worthiness.

  • Is it right for nurses to endorse health reform legislation even if the legislation is not perfect? Does this apply to the recently failed American Health Care Act?

Yes I believe it is very important for nurses to be involved in politics and endorsement of health reforms even if they are not considered to be perfect. Nurses are huge advocates and we have a voice that could speak out make a huge difference.

Week 4 Project

Reaching out a Solution

This assignment is designed to assist you in developing a thoughtful process for advocating about an issue as a nurse, from identifying a problem that needs to be solved through articulating a process for doing so.

This assignment consists of answering each of the questions listed below from the “Political Analysis and Strategies” (see attached chapter/document)chapter of your course textbook. Write each question as a new topic area; then follow with a paragraph or two to answer the question. Be sure to use APA guidelines for writing style, spelling and grammar, and citation of sources, if any used. This project should be no longer than 4 pages.

Let us assume that you are a school nurse in a high school. At a recent school athletic event, a spectator suffered a cardiac arrest in the stands. A coach of the home team went into the high school to fetch the automatic emergency defibrillator (AED) only to find out that it was not readily available. In the meantime, an emergency squad arrived and resuscitated the spectator. On Monday morning, you learn of the absence of the AED only to find out that it had been locked in the custodian’s closet. Reflect on the following questions outlined in the “Political Analysis and Strategies” chapter:

· What is the issue?

· Is it my issue, and can I solve it?

· Is this the real issue or merely a symptom of a larger one?

· Does it need an immediate solution, or can it wait?

· Is it likely to go away by itself?

· Can I risk ignoring it?

· What are the possible solutions? Are there risks to these solutions?

· What steps would you need to take in order to solve the issue?

· Does anyone else at the school need to be involved in the solution?

· Where is the power leverage in the school to reach the preferred solution?

Reaching a solution requires the use of power vested in the nurse. Review Box 9-1 (Sources of Power) and determine which type(s) of power the school nurse has in this situation. State your reasons for your answer.

Submission Details:

· Submit your response in a 4-page Microsoft Word document.

· Cite sources in the APA format on a separate page.

Nurses as Change Agents in the Community

As in all assignments, cite your sources in your work and provide references for the citations in APA format.

.

In the chapter, “How Community-Based Organizations Are Addressing Nursing’s Role in Transforming Health Care,”(SEE ATTACHED DOCUMENT HAVE THE CHAPTER TO READ))))) the authors describe the community as the focus of advocacy action for change. They list a series of questions that help community teams develop organizing principles to guide their interventions, and the aim is to attack social determinants of health to make community change.

Suppose a community team identifies lack of access to exercise modalities as a public health issue. If the team were to determine that pedestrian walkways need to be constructed, discuss at least two factors that would facilitate the development of the walkways and two that would hinder the development. For the hindrances, discuss ways that the team might work to overcome those hindrances.

 Cry

One of the strengths of the community such as in the community health directives is that the community gives a positive response and works in tandem with the community health workers as they transform public health in the society. Some of the assets that the community owns are the playgrounds, healthcare centers, and shopping centers. The assets of a community enable the community health providers to work effectively in the transformation of the public health standards.

The community has various needs as far as healthcare and nursing is concerned. Some of the needs of the community include access to the healthcare arrangements that the authorities such as the state and federal governments provide. The community also needs enough financial resources to afford maintenance of high standards in healthcare. Some of the goals of the community towards the same are the collaboration with the community-based organizations to provide affordable health care, to improve the quality of health care and increase the access points for community engagement and especially in healthcare.

The key stakeholders in the healthcare and nursing project are the individuals in the community, the community-based organizations and the various leadership authorities such as the state and the federal government (Bourcier et al., 2015). The healthcare institutions in the community are also part of the stakeholders in the community engagement with the public health initiatives (Bourcier et al., 2015).

Some of the goals in care to the community, according to Kangovi et al. (2014), are as follows. Firstly, the community-based organizations are targeting towards individualizing healthcare and bringing it closer to the populace (Kangovi et al., 2014). The other goal in care is to develop a comprehensive and diverse workforce that will be instrumental to the healthcare observance in the community (Kangovi et al., 2014). Partnership with the community is the other goal in care to improve the social and health wellbeing of the community members (Kangovi et al., 2014).

Some of the initiatives underway are the development of the community-based organizations to act as bridges to access of health care by the public. The other initiative is the diversification of the healthcare and nursing professionals to equip them with skills that will enable them to be some of the key players in the provision of health care to the community.

If the team were to determine that pedestrian walkways need to be constructed, discuss at least two factors that would facilitate the development of the walkways and two that would hinder the development. For the hindrances, discuss ways that the team might work to overcome those hindrances.

Identifying public health issues is an important task that requires attention in order to implement change. Often times, change is opposed and not welcomed. With reference to the situation presented, there are benefits and hindrances that could arise. The need for access to pedestrian walkways would be presented based on the safety that these walkways would offer as well as the health benefit that these walkways would offer. With the walkways, there would be no reason for people to be walking in the roadway or close to the road, risking being injured by a motorist. There is less risk of injury since the walkways would provide safe and level paved ground to walk on. The health benefit would come from the walkways giving persons wanting to walk, a safe spot to walk or run and not be in the way of cars traveling the roadways. Walking has many health benefits that include:

  • increased cardiovascular and pulmonary (heart and lung) fitness
  • reduced risk of heart disease and stroke
  • improved management of conditions such as hypertension (high blood pressure), high cholesterol, joint, and muscular pain or stiffness, and diabetes
  • stronger bones and improved balance
  • increased muscle strength and endurance
  • reduced body fat.

Two different hindrances that could be expected include cost and space. The need for the walkways would be a good tool for encouraging those in the city council and/or county commission to approve the funding. Space may be a problem if the roadways are close to housing and stores. Dealing with the right of way laws, and getting private citizens to be onboard with the construction could also become a problem. Educating the public on what these sidewalks could mean for the health of the community could potentially increase the approval as well as recruit others to promote the idea.
