Learning Objectives

Students will:

· Develop effective documentation skills for group therapy sessions *

· Evaluate the efficacy of cognitive behavioral therapy for groups *

· Analyze legal and ethical implications of counseling clients with psychiatric disorders *

Select two clients you observed or counseled this week during a group therapy session. Note: The two clients you select must have attended the same group session.

Then, in your Practicum Journal, address the following:

· Using the Group Therapy Progress Note in this week’s Learning Resources, document the group session (see attachment).

· Describe each client (without violating HIPAA regulations), and identify any pertinent history or medical information, including prescribed medications.

· Using the DSM-5, explain and justify your diagnosis for each client. (very important to use 

· Explain whether cognitive behavioral therapy would be effective with this group. Include expected outcomes based on this therapeutic approach.

· Explain any legal and/or ethical implications related to counseling each client.

· Support your approach with evidence-based literature.

Assignment 2: Practicum – Journal Entry

Reflect on a patient who presented with a cardiovascular, neurologic, respiratory, or gastrointestinal disorder during your Practicum Experience. Describe the patient’s personal and medical history, drug therapy and treatments, and follow-up care. Then, explain how the patient’s gender might have impacted or influenced her care. If you did not have an opportunity to evaluate a patient with this background during the last 10 weeks, you may select a related case study from a reputable source or reflect on previous clinical experiences.

Assignment: Practicum Journal Entry: Analyzing an Ethical Decision

In your role as a PMHNP, you will encounter several situations that will require your ability to make sound judgments and practice decisions for the safety and well-being of individuals, families, and communities. There may not be a clear-cut answer of how to address the issue, but your ethical decision making must be based on evidenced-based practice and what is good, right, and beneficial for patients. You will encounter patients who do not hold your values, but you must remain professional and unbiased in the care you provide to all patients regardless of their socio-demographic and ethnic/racial background. You must be prepared to critically analyze ethical situations and develop an appropriate plan of action. For this Assignment, you review the literature and discover the various ethical dilemmas PMHNPs encounter and how these issues are typically addressed in your state.

To prepare:

• Review literature for moral/ethical issues encountered by a PMHNP.
• Select one of the articles you found that was published within the last 5 years to use as a focus for this assignment.

Write a 2-page paper in which you do the following:

• Summarize the moral/ethical issue in the article (no more than 1 paragraph).
• Describe the moral and ethical dilemmas surrounding the issue.
• Analyze the ethical issue and compare them to the state health laws and regulations in your state.
• Outline the process of ethical decision making you would use to address this ethical dilemma.

Getting Started

As a prescriber, you will be bombarded with information about new medications and may even feel pressure to prescribe them.  This information and pressure may come from drug reps, journal advertisements, and even patients.  The journal club/article review is meant to help you learn to make your own judgments regarding the importance of a new medication (or new indication).

Upon successful completion of the course material, you will be able to:

• Take a primary research study and make an informed decision regarding the significance of the study.
• Apply study results to practice.

Resources

• Textbook: Pharmacotherapeutics for Advanced Practice Providers
• Lexi-Comp (available via Up-to-Date) or other suitable drug/disease reference.
• Suggested treatment guidelines/recommendations for this workshop
• Chosen article and any other necessary references

Instructions

1. Review the rubric to make sure you understand the criteria for earning your grade.
2. Chose an article of interest to you from one of your professional journals and critique it.  As indicated in the syllabus, the desired article is one of the primary research.  It should involve statistical analysis of a pharmaceutical agent that is being compared to another medication or a placebo. The article can pertain to a new medication, or a new indication for an existing medication but it must be studied in subjects requiring treatment, not healthy volunteers. 
3. Include at least the following in your analysis of the article (see example here):
   1. Title of the article (including journal reference)
   2. Study objective(s)
   3. Study design
      1. Overall design
      2. Study endpoints (primary, secondary, etc.)
      3. Inclusion/exclusion criteria
      4. Subject baseline characteristics
   4. Results
      1. Efficacy and safety outcomes, if applicable
      2. Statistical significance (p-values, confidence intervals, etc. as applicable)
   5. Personal conclusions
      1. Personal thoughts on the study
   6. Impact to practice
      1. How might the study change the way we practice?
4. The article review should be written using APA style.
5. When you have completed your assignment, save a copy for yourself and submit a copy to your faculty by day seven of the workshop (Turnitin enabled).
